{"title":"Positive Child Relationship Behavior Toward Mothers in 3-7-Year-Old Children with Mental Disorders.","authors":"Wolfgang Briegel, Paulina Mederer","doi":"10.1024/1422-4917/a001048","DOIUrl":"10.1024/1422-4917/a001048","url":null,"abstract":"<p><p><b></b> <i>Objective:</i> A paucity of research exists on the subject of positive child relationship behavior toward parents, particularly in children with mental disorders. <i>Method:</i> We invited the mothers of children 3 to 7 years with mental disorders according to the ICD-10 to complete questionnaires on positive child relationship behavior, child behavior problems, maternal psychopathology, dysfunctional parenting, and maternal sense of competence. <i>Results:</i> 71 children (56 boys and 15 girls, mean age = 64.07 [<i>SD</i> = 11.75] months with their mothers (mean age = 33.94 [<i>SD</i> = 6.49] years) participated in this pilot study. Children with primarily internalizing disorders did not differ from the general population regarding the frequency of positive relationship-relevant behavior toward their mothers. Conversely, children with externalizing disorders and those with a residual diagnosis exhibited significantly lower frequencies of positive behavior toward their mothers. Dysfunctional parenting behavior and maternal psychopathology did not correlate with the frequency of children's positive relationship behavior toward their mothers. {"title":"Depressive Störungen – von der ICD-10 zur ICD-11.","authors":"Svenja Geissler, Gerd Schulte-Körne, Ellen Greimel","doi":"10.1024/1422-4917/a001053","DOIUrl":"10.1024/1422-4917/a001053","url":null,"abstract":"<p><p>Depressive Disorders - From ICD-10 to ICD-11 <b>Abstract:</b> This article presents the main changes in the classification of depressive disorders associated with the transition from ICD-10 to ICD-11. We evaluate the changes regarding their relevance for diagnostic assessment during childhood and adolescence and the current state of research. Some changes were implemented in the categorization of disorders: In ICD-11, depressive disorders are listed as a disorder group separately from bipolar disorders and now include both dysthymic disorder as well as mixed depressive and anxiety disorder. With the introduction of ICD-11, the diagnostic criteria for depressive episodes were revised, however, specific considerations for diagnoses in children and adolescents remain unaddressed. The ICD-11 newly introduces the diagnosis of premenstrual dysphoric disorder as a depressive disorder, in parallel to DSM-5. In addition, it introduced seven diagnostic specifiers that describe depressive disorders beyond the diagnosis. The clinical descriptions and diagnostic guidelines were comprehensively updated with the revision of the ICD criteria and supplemented with developmental characteristics, which are particularly relevant for the assessment of children and adolescents. However, the guidelines are not yet included in the German draft version currently available online. {"title":"Ausprägung und Korrelate des Rauchverhaltens bei Patientinnen und Patienten mit Substanzkonsumstörung in einer jugendpsychiatrischen Spezialambulanz.","authors":"Marko Teichmann, Yulia Golub, Veit Roessner, Sören Kuitunen-Paul","doi":"10.1024/1422-4917/a001061","DOIUrl":"10.1024/1422-4917/a001061","url":null,"abstract":"<p><p>Prevalence and Correlates of Smoking Behavior Among Patients with Substance Use Disorders in a Specialized Adolescent Psychiatric Outpatient Clinic <b>Abstract:</b> <i>Objective:</i> Germany lacks recent data on the incidence and severity of tobacco use and tobacco use disorders (TUD) in high-risk populations, especially among adolescent outpatients with substance use disorders (SUDs). <i>Method:</i> We collected data from <i>N =</i> 134 adolescents in a specialized SUD outpatient unit for adolescent psychiatric patients using structured interviews (consumption and disorder symptoms) and a sociodemographic caregiver questionnaire (education, subjective financial status, and housing status). Descriptive data and linear regression models present possible associations between consumption, disorder diagnoses, and social demographics. <i>Results:</i> There were high 30-day prevalence rates for smoking (95 %), daily smoking (79 %), heavy or very heavy smoking (61 % and 23 % among smokers, respectively), and past-year TUD (77 %). Interpersonal Psychotherapy for Adolescents (IPT-A) has proved effective internationally, but data from German clinical settings are lacking. This pilot study aimed to examine the feasibility and effectiveness of the German version of IPT-A for adolescents with depression. <i>Methods:</i> Twenty-four outpatients (17 female, 7 male) aged 12 - 18 years (<i>M</i> = 15.96, <i>SD </i>= 1.66) with a primary diagnosis of depression received 12 - 16 sessions of IPT-A. We assessed feasibility via dropout rate. The primary outcome measure was the Children's Depression Rating Scale-Revised (CDRS-R), assessed at pretreatment, posttreatment, and 6-month follow-up. Additionally, questionnaires were included (Beck Depression Inventory II [BDI-II], Global Assessment of Functioning Scale [GAF], Conflict Behavior Questionnaire [CBQ-20]). <i>Results:</i> The dropout rate was 25 %. Depressive symptoms, as measured by the CDRS-R, decreased significantly over time with a large effect size, and reductions were maintained at the 6-month follow-up. <i>Conclusions:</i> The findings indicate that IPT-A is feasible in the German clinical care context and can reduce depressive symptoms in adolescents. It examines alternative hypotheses for the increase in various diagnoses (gender dysphoria, autism spectrum disorder, attention deficit/hyperactivity disorder) and discusses topics ranging from changes in diagnostic criteria, the neurodiversity movement, and the expansion of diagnostic concepts (\"concept creep\") to looping effects. {"title":"Attitudes of Psychotherapists Toward Outcome Measures and Routine Outcome Monitoring in Child and Youth Psychotherapy.","authors":"Sebastian Ertl, Theresa Emser, Sarah Garsche, David R Kolar","doi":"10.1024/1422-4917/a001071","DOIUrl":"https://doi.org/10.1024/1422-4917/a001071","url":null,"abstract":"<p><p><b></b> <i>Objective:</i> In this observational study, we investigated which therapeutic characteristics affect the attitudes of psychotherapists working with children and adolescents toward Outcome (OM) and Routine Outcome Monitoring (ROM), and how implementation is affected by previous experience and attitudes toward administering OM/ROM. <i>Methods:</i> We assessed the attitudes of licensed German psychotherapists and psychotherapists in training working with children and adolescents toward the OM/ROM in an online survey conducted between June 2023 and March 2024 (<i>N</i> = 126). <i>Results:</i> Compared with psychoanalytic-oriented therapists, therapists with an orientation in cognitive behavioral therapy (CBT) presented more positive attitudes, but we found no significant difference between orientations in CBT and psychodynamic psychotherapy. Therapists working in institutionalized outpatient settings reported more positive attitudes than those working in private practices. We found no effect of previous experience in administering OM/ROM for either implementation barriers or facilitators. However, more positive attitudes toward OM/ROM are associated with higher expectations regarding both support in therapeutic processes and support in clarification. <i>Conclusion:</i> OM/ROM systems should consider assessing concepts from different psychotherapeutic orientations.
