This study aimed to report the real-world single-center experience of using VDD leadless pacing (Micra AV) compared to conventional transvenous DDDR pacing in patients with high-grade AV block in Hong Kong. Methods: A retrospective review of consecutive patients who underwent Micra AV between October 2020 and April 2022 compared to patients with matched age and comorbidities who received conventional DDDR pacing between January 2017 and April 2022. Patients with intermittent or permanent high-grade AV block were included. Results: The outcomes of 22 Micra AV patients were compared to 22 patients who received conventional DDDR pacemakers. The electrical performance was stable during follow-up in 95.5% of patients who received Micra AV. AV synchrony was maintained in 76.2% and 77.8% of patients who received Micra AV at baseline and 6 months, respectively. The mean procedure time was significantly shorter in the Micra AV group than in the conventional group (40.0 ± 14.1 minutes vs. 71.6 ± 33.7 minutes) (p < 0.001), and the length of stay post-implantation of pacemaker in Micra AV group was also shorter (median 1 day vs. 7 days). Obesity is linked to increased endothelin-1 levels, which contribute to endothelial dysfunction, in fl ammation and insulin resistance. This study aimed to investigate the association between serum endothelin-1 levels with obesity among Indonesian females. Methods : This was a cross-sectional study. The subjects were from the population of Universitas Gadjah Mada Health and Demographic Surveillance System (HDSS) in Sleman, Indonesia. We collected venous blood, measured obesity parameters and performed the standardized questionnaire for 124 males and females. Endothelin-1 level was measured from serum by ELISA-method. Endothelin-1 level was compared between subjects according to obesity category and correlated with parameters of obesity. Results : There were 124 subjects enrolled, consisting of 70 females and 54 males. Endothelin-1 level was comparable between both sexes. Females had signi fi cantly higher proportion of central obesity compared to males (70.0% vs. 33.3%, p < 0.001). Females with central obesity and increased waist-to-height ratio had a tendency to have higher endothelin-1 levels. Females with body-mass index (BMI)-derived obese class II had the highest level of endothelin-1 (1.98 ± 1.09 pg/ mL), which was signi fi cantly higher compared to obese class I females (1.48 ± 0.35 pg/mL, p ¼ 0.026) and normoweight females (1.53 ± 0.41 pg/mL, p ¼ 0.030). Furthermore, in females, endothelin-1 level was signi fi cantly correlated with waist circumference (r ¼ 0.331, p ¼ 0.005), hip circumference (r ¼ 0.237, p ¼ 0.048), waist-to-hip ratio (r ¼ 0.246, p ¼ 0.040), waist-to-height ratio (r ¼ 0.354, p ¼ 0.003), BMI (r ¼ 0.248, p ¼ 0.038) and body adiposity index (BAI) (r ¼ 0.277, p ¼ 0.020). This correlation was not found among males. {"title":"Procedural and Clinical Outcomes of Dedicated Venous Stents in Acute and Chronic Venous Obstruction: A Retrospective Single Center Study","authors":"G. Tan, Madeleine G Yang, B. Dedicated venous stents have been developed to improve the ef fi cacy and safety of the venoplasty procedure. We report the procedural as well as clinical outcomes of these new generation stents in the treatment of acute and chronic venous obstruction. Method : This is a single centre retrospective cohort study of patients who received venous stenting for either acute or chronic venous obstruction. Baseline demographics, procedural details, clinical outcomes including 30-day all-cause mortality, 90-day primary patency rate, and change in Villalta score were reported. Result : 41 patients with venous obstruction were included in this study. The mean Villalta score before the procedure was 13.8 ± 3.8. Extrinsic compression of the affected vein was found in 95.1% of cases. Procedural success was achieved in 97.6% of cases, with 2 ± 1 stents of mean diameter of 15.3 mm ± 1.9 mm and total stented length of 179.3 mm ± 85.3 mm used per case. There were no procedural complications. 90-day primary patency rate was 90.2%. The Villalta score decreased by 9.2 ± 0.7 to 4.6 ± 2.4 at 90 days. {"title":"Precision Medicine for Cardiac Ion Channelopathies in Hong Kong: From Case Reports to Identification of Novel Genetic Variants and Development of Risk Prediction Tools using Population-based Datasets","authors":"Sharen Lee, N. Tse","doi":"10.55503/2790-6744.1506","DOIUrl":"https://doi.org/10.55503/2790-6744.1506","url":null,"abstract":"Congenital cardiac ion channelopathies refer to a set of inherited conditions characterized by abnormalities in the structure and/or function of ion channels, their associated proteins or other signalling components, predisposing affected individuals to life-threatening ventricular tachyarrhythmias and therefore sudden cardiac death. This is a literature review focusing on the progress of clinical research on congenital cardiac ion channelopathies in Hong Kong, from case reports in the 1990s to population-based studies in the 2020s. Locally, patients with Brugada syndrome, long QT syndrome and catecholaminergic polymorphic ventricular tachycardia have been studied. Leveraging the power of linked electronic health records data in the public sector, the epidemiology, clinical characteristics, genetics, genotype e phenotype relationship and predictive factors of arrhythmic events have received attention. Future efforts should focus on multidisciplinary collaborations between clinicians, scientists and data scientists the use of genomic data combined with clinical data for personalised risk prediction. With the Government ' s drive for innovations and recent announcement of the Strategic Development of Genomic Medicine in Hong Kong, future efforts should be focused on the development of a national registry linking the databases and standardizing the data fi elds and reporting in different centres in Hong Kong, other cities in the Greater Bay Area and the wider mainland.
