Nevertheless, the efficacy of endocrine therapy is imprecise and linked with unfavorable outcomes. Consequently, surgical intervention remains the favored approach for addressing cases of undescended testis. Aim of the work: We aimed to compare the outcomes of single scrotal incision orchiopexy versus the traditional inguinal approach in the treatment of children suffering from the palpable low-lying undescended testis [PUDT]. Patients and Methods: This is a prospective randomized controlled comparative study that included 40 patients presented with palpable low-lying undescended testis to the pediatric surgery outpatient clinic and admitted to the pediatric surgery unit at the general surgery department of Al-Azhar University Hospitals, Assiut. Patients were randomly divided into 2 equal groups 20 patients each: Group 1: underwent the traditional inguinal approach for orchiopexy. Group 2: underwent single scrotal incision orchiopexy. Results: As regards the operative time, we found that the mean operative time was higher in group 1 than in group 2 [30.5, and 22 minutes respectively], with a statistically significant difference between the two groups [P value = 0.001]. As regards the postoperative complications, we found that five patients [25%] in group 1 developed complications which is higher than that of group 2 in which the total reported complications were in 3 [15%.] patients. In terms of Wound infections and dehiscence, they were the most common postoperative complication in our study which represent 45% of the total reported complications. We reported four cases in the traditional inguinal approach orchiopexy group and 2 cases in the single scrotal incision orchiopexy group. Patients with large overhanging abdominal panniculus should benefit from this treatment. Pregnancy changes the size and contour of the abdomen; so, abdominoplasty is often requested during a Cesarean delivery. In late pregnancy, especially in multiparous women, the abdominal wall muscles are subjected to increased strain, and the rectus muscles divaricate in the midline, generating diastasis recti. Panniculectomy is a popular women's aesthetic surgery. It can be long-lasting if maintained with exercise and a balanced diet. Aim of the work: The goals of this study are to assess the advantages, disadvantages, cosmetic outcomes, and complications of combining panniculectomy with Caesarean Section. Patients and Methods: A prospective randomized clinical controlled trial where 50 of Gravid women shared and equally distributed to two groups, each group includes 25 pregnant women who had already made the decision to have an elective Cesarean section at Al-Azhar University's Department of Obstetrics and Gynecology. Results: In terms of time needed to complete a procedure, there was a massive gap between the two groups. The two groups did not differ significantly with respect to wound complications, post-operative body mass index. The Panniculectomy with CS group had a mean of 8.72 ± 1.24. [range: 5-10] in terms of patient satisfaction. Conclusion: There's an argument that doing both operations at once would save time and money. It does not appear to be in the best interests of most patients, however, due to the higher risk of postoperative complications and un-esthetic outcomes. Progranulin [PGRN], a growth factor, has major biological impact on several cancer types. Aim of the work: To measure levels of progranulin in the serum of adult patients with acute myeloid leukemia [AML] before and after treatment. Next, to correlate their serum levels to prognosis. Patients and Methods: Our prospective observational study was conducted on 40 adult patients with AML recruited from Clinical Hematology and Oncology hospital in Maadi Military Complex before and after chemotherapy treatment during six months. 15 adult healthy people with comparable age and sex were subjected as a control group. The patients received conventional chemotherapy protocols according to NCCN guidelines for AML treatment. Either routine laboratory investigations or disease-specific labs were performed. Progranulin level in serum was assessed using ELISA. Results: Serum Progranulin levels are significantly higher for AML patients than control group. There is statistically insignificant difference of serum Progranulin levels after chemotherapy treatment compared to its level at diagnosis among AML patients. Aim of the work: The aim of this research had been to examine potential of remote ischemic preconditioning on myocardial and renal protection and patient survival after elective open AAA repair. Patients and Methods: This randomized double blinded clinical trial comparing AAA repair with remote ischemic preconditioning versus conventional AAA repair [control] was done in cardiothoracic and vascular surgery center Mansoura University from October 2022 to May 2023 after approval from institutional review board of Mansoura faculty of Medicine with obtaining a written informed consent from all studied cases. Results: There had been statistically significant variation among studied groups as regard 30-day mortality. High-type fistula-in-ano is especially hard to manage due to its position. Standard surgical methods are associated with many adverse events such as incontinence and repeated infections. Fibrin glue is a kind of biological glue which facilitates tract obliteration, with preservation of anal sphincter. Aim of the work: This study aims at comparing the efficacy of fibrin glue versus fistulotomy in the treatment of high perianal fistulas. Patients and Methods: In the present study, 30 patients diagnosed with a high-type perianal fistula were divided into two groups; group [1] undergone standard fistulotomy, and group [2] undergone fibrin glue. Operative outcomes, post-operative improvements and complications were compared. Results: Patients in group [2] had significant short operative time [31.1 ± 12.5 min. in group 1 vs. 19.7 ± 1.62 min. in group 2; P, 0.004]. Group 2 had a significantly shorter hospital stay length than group 1 [1.93 ± 0.79 days for group 1 and 1.31 ± 0.35 days for group 2; P, < 0.001]. No significant difference was shown between studied groups regarding pain improvement, wound infection, wound discharge and incontinence at one week following surgery. However, group 1 patients had a significant [P, <0.001] less healing time than group 2. At six months following surgery, patients in group 1 reported higher rate of infection [four patients vs. none in group 2; P, 0.03] and higher recurrence rate [six in group 1 vs. one patient in group 2; P, 0.03]. Conclusion: The use of fibrin glue injection is a viable, valuable, and effective alternative for treating fistula-in-ano. For cosmetic purposes, dermatologists usually recommend topical keratolytics, laser treatments, or chemical peels. The treatment of AN with a topical retinoid called adapalene has a high rate of success. Metformin, an insulin sensitizer, was proposed as a therapy option for AN by focusing on the insulin resistance connected to AN. Aim of the work: This study was done to compare the efficacy and safety of topical 30% metformin cream with topical 0.1 adapalene cream. Patients and Methods: There were 40 instances of AN in the most recent study. Two sets of cases, each with 20 patients, were randomly assigned: Group A [the metformin group] and Group B [the adapalene group]. The patients had a thorough dermatological examination along with a general clinical evaluation. The initial assessment included the calculation of HOMA-IR, assessment of disease severity, digital photographs, and dermoscopic assessment. After completion of treatment, clinical response and the degree of improvement were determined in the cases within the two groups. Follow-up was done for 2 months after treatment. Results: All clinical and dermoscopic results evaluated following treatment show statistically significant variance across the tested groups in terms of the degree of improvement. In the adapalene group, the extent of improvement was greater. The mean ANASI score for the metformin group decreased from 32.4 to 27.8 pre-and post-operatively, while for the adapalene group, it decreased from 26.8 to 18.4. The Adapalene group had a larger percent change, although there is no statistically significant distinction between them. Conclusion: Adapalene 0.1% cream was more successful than Metformin 30% cream in treating acanthosis nigricans with minimal and tolerable side effects.
