Complicated metacarpal fractures are more common than any other type of fracture. The general goal of treating all fractures is for the patient to achieve normal motion. However, the ideal stabilization technique is still debated. For internal fixation, Kirchner wires [Wires] or can be used, each offering several advantages. No previous study has compared the use of Kwire with miniplates in the treatment of unstable metacarpal fractures. Therefore, we conducted this prospective intervention study to evaluate the outcomes of using Kwire and miniplates in the treatment of unstable metacarpal fractures . Aim of the work: To contrast the functional and radiological outcomes of MPIF and also KWIF for the treatment of unstable metacarpal fractures; and to determine which fixation method [Kirschner wire or mini plates] provides the best functional and radiological outcome in the treatment of metacarpal fractures. Patients and Methods: This prospective interventional investigation was performed at the Al-Azhar university Hospital in Damietta, in the department of orthopedic surgery. This study was conducted on 40 patients with unstable metacarpal fractures. Results: There were statistically significant variations among the two groups regarding operative time [minutes], blood loss [milliliters] & hospital stay [day]. The hand grip, DASH, and TAF scores did not differ significantly among the two groups. There was no statistically significant variation in result among the two groups. In regards to complications, there was not a significant distinction among both groups examined. Conclusion: In the treatment of unstable metacarpal fractures, both the Kirschner wire and the Mini Plate proved to be reliable options. Aim of the work: This study compares open and laparoscopic hernia repair procedures performed in hospitals. Patients and Methods: In this retrospective cohort study, the open and laparoscopic approaches to inguinal hernia repair were compared and evaluated for their effects on post-operative pain, hospital stay, operating room time, seroma, urinary retention, paresthesia, numbness, wound infection, and recurrence. From June 2020 to June 2022, 30 patients underwent hernioplasty surgery for a unilateral primary inguinal hernia. A laparoscopic hernioplasty [TAPP] was performed on Group A, while an open hernioplasty was performed on Group B. Results: There was a statistically significant difference between the two groups regarding the surgical time [p-value = 0.0001] and hospital stay [p-value = 0.0001]. While 20% of group B reported paresthesia and numbness, group A had no complications in terms of paresthesia and numbness [P=0.22]. There was no statistically significant difference between the two groups in terms of seroma and infection results [P-Values =0.329 and 1, respectively]. 13.3% of group A patients experienced recurrence of their hernias compared to group B's 6.7% [p-value =1]. Patients and Methods: seventeen new-born infants with bilateral congenital choanal atresia were managed endoscopically by a transnasal repair together with submucosal resection of the posterior part of the septum. On CT scan, 14 infants had mixed atresia, and 3 infants had pure bony atresia. Using a 2.7 mm, 0° sinuscope, the site of the atretic plate and the adjacent part of the posterior septum were infiltrated. A vertical incision was taken at the bony cartilaginous junction of the septum using a round/sickle knife. The bony septum was drilled out along with the atretic plates of both sides. The two septal mucoperiosteal flaps were then approximated and the soft tissue that was occluding the choana was then excised on both sides. The remaining bones of the atretic plates were then removed from the remaining superior, lateral and inferior walls to widen the neo-choana and create a wider than normal choana Results: A follow-up period for at least one year, revealed that 10 cases remained patent for adequate size bilaterally, four cases were found with bilateral concentric narrowing but still adequate for breathing, one case required revision surgery of the right side, two cases required bilateral revision surgery. We evaluated the frequency of aberrancy in AL patients. Aim of the work: Evaluation and analysis of the incidence of aberrant phenotypes in mononuclear cells in AL patients and their relation to hematological parameters. Patients and Methods: This observational descriptive study was conducted on 150 newly diagnosed cases of AL over the period from August 2017 to March 2021. Flow cytometry was performed by Becton Dickinson [BD] FACS Calibur/BD FACS Canto flow cytometers using monoclonal antibodies against cell markers. Results: The overall frequency of aberrancy was 38.7% [58/150] in all AL patients. Aberrant antigenic expression was more frequent in acute lymphoblastic leukemia [ALL] than acute myeloid leukemia [AML], T-ALL with aberrant expression was 66.6% of all T-ALL patients, while B-ALL with aberrant expression was 45.8% of all B-ALL patients, and AML with aberrant expression was 31.1% of all AML patients. However, it is still not existing. Aim of the work: To measure the serum level of plasma myeloperoxidase [MPO], and its correlation with respiratory function as well as its predictive power of asthma and its severity. Patients and Methods: The study recruited 130 patients with asthma, with age and sex-matched other 130 healthy controls. Patients were clinically evaluated; respiratory functions were performed and blood samples were drawn to estimate the serum levels of MPO. In addition, sputum was assessed for cellular content. Asthma severity was determined according to available guidelines and both mild and moderate forms were considered as [non-severe form of asthma]. Receiver operation characteristic [ROC] curve was built to assess the predictive power of MPO. Results: There was female-sex predominance in the study and control groups [65.4% and 70.0%, respectively]. 15.4% had mild, 35.4% had moderate and 49.2% had severe asthma. MPO levels were significantly higher in the study than the control groups [3310.35±373.39 vs. 1900.32±333.57 pg/ml, respectively]. The serum levels of MPO were positively correlated with asthma severity, all cellular content in BAL and body mass index. However, it was inversely correlated with respiratory function. MPO were significantly higher in severe than non-severe forms [3666.59±736.37 vs. 2964.91±554.47 pg/ml, respectively]. MPO had a good predictive power of asthma and its severe form [the AUC was 0.950 and 0.819 respectively]. Conclusion: Serum levels of MPO can used as a good biomarker for diagnosis of asthma and its severity. Eclampsia, pulmonary edema, HELLP syndrome, cerebral hemorrhage, severe renal failure, placental abruption, and disseminated intravascular coagulation are among the maternal consequences of pre-eclampsia [DIC]. Aim of the work: To analyze the relationship between maternal problems and uterine artery Doppler measures in women with severe pre-eclampsia. Patients and Methods: 200 pregnant patients from Sayed Galal and Shebin El-Kom teaching hospitals participated in a case-control study. Two groups of pregnant women were used in this study. Group I [Case group] consisted of 100 pregnant women with viable singleton pregnancies and no other obstetric complications or morbidities aside from preeclampsia. There were 100 expectant women in Group II [the Control group] who did not have pre-eclampsia. At admission, uterine artery Doppler was performed, and the postpartum result was assessed. Results: Uterine artery When compared to the control group, the case group's Doppler had a much higher resistance, which was linked to an increase in eclampsia, oliguria, renal failure, HELLP, and blood transfusions. As a result, it could be valuable to examine chemical markers of accessory glands as a means of identifying these changes. Aim of the work: This study aims at evaluating the effect of obesity on markers of accessory glands [seminal vesicles and prostate], epididymis and seminal parameters [volume, concentration, motility, morphology, viability] in infertile men. Patients and Methods: This study is a comparative crosssectional study that included 50 males complaining from infertility, who were divided in two groups according to BMI; control group: BMI [18-25 kg/m] and case group: BMI ≥ 25 kg/m. Semen analysis and markers of accessory gland were analyzed for all patients. Results: The semen volume, sperm count was significantly decreased in obese adults as compared to controls. The total motility, progressive motility ratio and ratio of immotile sperms all were affected among obese infertile men. Serum fructose, citric acid and alpha glucosidase levels were all affected among obese individuals with statistically significant difference [P= <0.001]. Conclusion: Obese men showed decreased sperm and prostatic function as compared with controls. Markers of oxidative stress are linked with progressive adiposity. So that, avoiding excessive body weight in infertile men at the appropriate time could lead to an improvement in both infertility and prostate disorders.
