There are some evidence of successful non-surgical treatment of this type of discrepancy after puberty by using maxillary expansion techniques. The Miniscrew-assisted rapid palatal expander (MARPE) method is a newly introduced method that has attracted the interest of many orthodontists. This study aimed to evaluate the therapeutic results of this technique in a systematic review. \u0000Method: In this study, various databases including PubMed, Scopus, and ISI (Web of science) were queried with proper keywords, provided by PICO strategy of research, from 1980 to December 2020. Relevant articles were collected with restrictions on the English language. The full text of papers with all inclusion criteria was assessed. The therapeutic outcomes of MARPE were evaluated in the selected studies. \u0000Results: A total of 14 studies were included in the systematic review. Totally, 5 case report studies, 8 retrospective studies, and 1 case series study were reviewed. Cone-beam computed tomography was used as an outcome measure in all studies. In all reviewed studies, maxillary expansion occurred with high success rates in patients. Moreover, respiratory characteristics were also assessed in two studies measuring respiratory muscle strength, airflow and nasal and nasopharyngeal airway volume, showing benefits of MARPE. A study also examined three-dimensional soft tissue stereophotogrammetry. Based on these studies, the indication for using this method was a maxillary transverse deficiency as well as upper and lower arches crowding. No serious complications were reported in any of the reviewed studies. However, most studies did not perform long-term follow-ups and the age range of the subjects was mainly between 12 and 24 years. Kharazifard","doi":"10.18502/jcr.v8i1.7151","DOIUrl":"https://doi.org/10.18502/jcr.v8i1.7151","url":null,"abstract":"Background: medication related osteonecrosis of the jaw (MRONJ) is a chronic condition of the oral cavity resulting in mucosal ulceration and exposure of underlying necrotic bone, and the ensuing secondary complications. The aim of this study was to evaluate the relationship between vitamin D deficiency and osteonecrosis of the jaw related to bisphosphonates. \u0000Materials and methods: This was a case-control study. The samples were 20 patients taking bisphosphonates, that 10 of them were with MRONJ and 10 were non-MRONJ. Clinical examination of patients was performed to diagnose jaw osteonecrosis. Demographic data of the patients were recorded including age, sex, type of drug, duration and cause of drug intake, and measurement of serum vitamin D levels. Data were analyzed using SPSS software. \u0000Results: In the patients without MRONJ, the mean age was 60.60 (±14.975) years, and in the patients with MRONJ, the mean age was 68.30 (69.92) years. As a whole, of the 16 female patients in this study, 10 cases (62.5%) were non- MRONJ and 6 cases (37.5%) were suffered by MRONJ. All of the male patients presented with MRONJ. In control group mean of vitamin D was 63.990 ng/ml (±29.796) and in case group mean of vitamin D was 29.510 ng/ml (±23.723). It seems that olfactory dysfunction is one of the early occurring symptoms of this viral infection and many patients just show this symptom after they got infected. Considering so, olfactory dysfunction especially a decline in olfaction could potentially be used for screening purposes and preventing the disease to spread. \u0000Methods: 50 PCR-verified SARS-CoV-2 infected participants were assessed about their olfactory function adequacy using a Modified Version of the University of Pennsylvania Smell Identification Test (UPSIT) for the Iranian population called Iran Smell Identification Test (Iran-SIT). Participants scores were compared against normal population scores in this test and possible correlations of age and scores were explored as well \u0000Results: Participants with SARS-CoV-2 infection generally obtained lesser scores in the mentioned test which means they experience a decline in olfactory function more, significantly. Aging also has a negative correlation with olfaction adequacy. \u0000Conclusion: Based on this article’s finding, olfactory function decrease is more frequent among SARS-CoV-2 infected people and potentially could be a suggestive indicator for screening programs. Reconstruction of such injuries become a major challenge for maxillofacial surgeons. Clinical experiments indicate that one of the serious problems associated with conventional plate systems is the frequent incidence of complications such as screw loosening, plate exposure and plate fractures. To improve the performance of reconstruction system with new procedure. A 42-year-old male patient suffering from Ameloblastoma tumor in the lateral large defect was selected as case study. Initially, after cutting the cancerous tissue, a titanium conventional plate (TCP) model had been utilized as mandibular reconstruction system which failed due to plate exposure. Patient's CT-scan images were prepared, and geometry and shape of the plate were evaluated using Computer-Aided Design & Computer-Aided Manufacturing (CAD/CAM) and Additive Manufacturing (AM) technology. Then, its effect on the biomechanical performance of the failed system TCP model was investigated by finite element method (FEM). Fibula Free Flap FFF model as alternative and improved reconstruction system was selected. FEM evaluation of two models showed inevitable results which tip the scales in the favor of FFF model. The maximum Von-Mises stress had been exerted at the interface between screw-cortical bone. In TCP model, the peak value of Von-Mises stress exerted at the interface between screw-bone was 110 MPa, which exceeded the yield strength of the cortical bone, while, this factor fell to 68 MPa in FFF model. Furthermore, comparison with TCP model, the sensitivity of the plates and screws to the chewing load variations in FFF model decreased 20%. To perform this retrospective study the prevalence of impaction, angulation, and the level of the eruption were assessed. Jafari, Hamed Zandi Esfahani","doi":"10.18502/JCR.V7I1.4006","DOIUrl":"https://doi.org/10.18502/JCR.V7I1.4006","url":null,"abstract":"Introduction: The purpose of this study is to determine the incidence of velopharyngeal insufficiency (VPI), fistulae and recurrence development in patients seen by the Isfahan Cleft Care Clinic and also determine the association of gender, age at repair, and cleft type with the incidence of each. \u0000Methods and materials: In this retrospective study, 320 children who had undergone a primary cleft palate surgery and who had come to the Cleft lip and palate in Isfahan in 2010-2017 were studied based on the medical records and Information about each patient. Then all patients were evaluated by standard speech assessment methods for severity of hypernasalitis and screened for clinical manifestations of fistula and recurrence. \u0000Results: According to the results of this study, the incidence of velopharyngeal insufficiency after initial repair was 78.1% and most of these patients had severe form. The results also showed that by increasing age at surgery also increased the intensity of velopharyngeal insufficiency, and the severity of this complication has nothing to do with gender. In the case of fistula and recurrence of cleft showed that, in patients who are undergoing the new procedure to be significantly less than other patients. \u0000Conclusion: According to the study, palatoplasty complications such as velopharyngeal insufficiency, fistula, and recurrence were less common in patients treated with microsurgery, Therefore, it can be concluded surgical repair of cleft palate should be performed before 12 months ages and in microsurgery methods.
