The level of protection of vaccines administered together with the COVID-19 vaccine is the subject of research. We aimed to examine intensive care patients with Sinovac and Biontech vaccines. \u0000Material and Method: After the approval of the ethics committee, 45 patient data were included in our study. Vaccination status in intensive care units was examined and patients were grouped into Sinovac and Biontech. Age, sex, comorbid conditions, and mortality rates were compared statistically. With this comparison, the effects on mortality and clinical features were evaluated. \u0000Results: Forty-five patients between March 31 and December 31, 2021 were included in the study. In these patients, patients who received 2 doses of vaccine in the period before hospitalization according to the intensive care evaluation criteria were examined. Mortality rates were higher in patients who received 2 doses of Sinovac vaccine in patients hospitalized in the intensive care unit compared to those who received 2 doses of Biontech vaccine. \u0000Conclusion: It was observed that the vaccination status of patients who were vaccinated with COVID-19 and taken to the intensive care unit affected mortality. Studies with larger numbers of patients are needed.

Large-scale vaccination of risk groups and later the general population is the single most effective public health measure for mitigation of the COVID-19 pandemic. \u0000Methods and Materials: This prospective cross sectional study was carried out at Dhaka North City Corporation dedicated COVID-19 hospital, Mohakhali, Dhaka, enrolling 50 (fifty) vaccinated RT-PCR positive COVID-19 patients. \u0000Results: Majority of the patients were >50 years old. Most of the patients had bilateral lung involvement and ground glass was the predominant CT pattern. 20% patients had consolidation in HRCT scan. Most of the patients (46%) had mild disease and only 18% patients had severe disease. Severe cases were more common (77.8%) in older (>50years) patients. 20% of the patients had 50-75% and only 6 % patients had more than 75% of total lung involvement. 52% patients had less than 25% lung involvement. Lung involvement was significantly higher in older patients. Patients had to stay an average of 6.2 days in hospital. Older patients had to stay more days in the hospital than younger patients. 2% (1) of COVID-19 patients died after admitting into our hospital. \u0000Conclusion: Vaccination can effectively reduce mortality and morbidity of COVID-19 patients by reducing the active number of cases as well as severity of the disease.

Classic Sweet syndrome is not associated with malignancy or drug exposure and typically involves elderly females. A 67-year-old female with a past medical history of multiple sclerosis and chronic kidney disease presented with worsening erythema of bilateral lower extremities, and diffuse, tender pustular lesions in her extremities, eye lids, nares, oral commissures, and tongue. A biopsy revealed marked neutrophilic inflammation suggestive of Sweet syndrome and she was started on high dose methylprednisolone which immediately improved her condition. As with most progressive inflammatory diseases, early recognition and early treatment improves the prognosis. This case is unusual because of mucosal involvement at presentation. Classic Sweet syndrome can often be diagnosed by exclusion after failed treatment for either infectious or autoimmune disorders.

While tuberculosis is not uncommon in pregnancy, active, disseminated tuberculosis is exceedingly rare and is associated with poor maternal and fetal outcomes including a ninefold increase in miscarriage. Symptoms of both disseminated tuberculosis in pregnancy can be vague and nonspecific making it difficult to diagnosis. A 20-year-old, immune competent female presented with worsening dyspnea over two months and cavitary lesions on chest imaging. She had been treated with oral antibiotics two months prior and was noted to have an early intrauterine pregnancy at the time. She subsequently miscarried spontaneously. PCR (polymerase chain reaction) testing from brochoalveolar lavage isolated Mycobacterium Tuberculosis. A liquid biopsy (Karius) was positive for Cytomegalovirus, Herpes Simplex Virus 1, and Candida Albicans. The patient had a prolonged and complicated treatment course and ultimately was discharged to inpatient rehabilitation. Disseminated tuberculosis is associated with significant mortality and morbidity warranting prompt treatment with clinical suspicion. Treatment should start when active TB is diagnosed in pregnancy regardless of the trimester. The prenatal care giver is in a unique position to screen and support women who otherwise may not interact with a structured healthcare system.

The gold standard for hyperpigmentation therapy is hydroquinone (HQ) but its long-term use can cause side effects such as irritation, rebound phenomenon, and even ochronosis. Black soybeans contain active ingredients that can prevent the increase of tyrosinase enzyme and the amount of melanin so that it can be used as an alternative therapy for hyperpigmentation. This study aims to prove that the administration of 30% black soybean extract cream was as effective as 4% HQ cream in inhibiting the increase of tyrosinase enzyme expression and the amount of melanin in guinea pig skin exposed to UVB rays. Methods: This study used a post-test only control group design method with 30 guinea pigs as samples. The sample was divided into three groups of 10 each. The P1 group was given the base cream, the P2 group was given the 4% HQ cream, while the P3 group was given the 30% black soybean extract cream. Each group was exposed to UVB rays with a total dose of 390mJ/cm2 for 2 weeks, after that the guinea pig skin tissue was taken and histopathological preparations were made for examination of tyrosinase enzyme expression and the amount of melanin. The data obtained were analyzed using one way ANOVA, then the differences between groups were analyzed using the Least Significant Difference (LSD) test. Results: The results showed that the mean of the tyrosinase enzyme expression and the amount of melanin in the black soybean extract group P3 (12.880±3.327% and 3.16±1.486) were not significantly different from the HQ4% group P2 (9.690±2.176% and 2.18±1.117), while the mean of the base cream group P1 (25.78±5.392% and 12.080±3.150) was the highest among the three groups. The LSD test showed a significant difference between the base cream group (P1) and the other groups (p<0.01), but there was no significant difference between the 4% hydroquinone group (P2) and the 30% black soybean extract treatment group (P3) (p >0.05). Conclusion: The conclusion of this study was that 30% black soybean ethanol extract cream inhibited the increase of tyrosinase enzyme expression and the amount of melanin as effective as 4% HQ cream on guinea pig skin exposed to UVB light.

Both peripheral and in traventricular administration of these cytokines causes acute phase symptoms, e.g. fever, activation of the hypo -thalamic-pituitary-adrenal axis and psychological depression. The amygdala belongs to the structures of the lim bic system involved in the regulation of the immune response. Increased activity of immune system may lead to changes in the role of amygdala, medial prefrontal cortex, anterior cingulate cortex or insula.
