In the 1970s, Bayes de Luna introduced the definition of interatrial blocks (IABs), which negatively influence atrioven - tricular (AV) synchrony and left atrial (LA) activation. We aimed to assess the potential of LA strain technology in evaluating the mechanics of LA in patients with correct conduction and IABs. Additionally, we measured the pa - rameters of regurgitation in pulmonary veins (PV), which depend on the type of interatrial conduction. Material and methods: The study group comprised 51 patients (26 male, 25 female) with symptomatic COVID-19 and si - nus rhythm. Our study analyzed their medical history, electrocardiography (ECG) and echocardiography, including the LA strain parameters. Results: Global peak atrial longitudinal strain (PALS) depended on P-wave duration, LA volume, left ventricular ejection fraction (LVEF) and inferior pulmonary veins (PV) regurgitation parameters. Glo - bal peak atrial contractile strain (PACS) statistically depends on the LVEF, LA volume and the P-wave morphology. Conclusions: The presence of IABs negatively influences PACS and PALS. Examining LA strain is complementary to accurate ECG, which may be helpful in everyday clinical practice, particularly in diagnosing heart failure with preserved ejection fraction (HFpEF) and as a predictor of new episodes of atrial fibrillation (AF).","PeriodicalId":52409,"journal":{"name":"European Journal of Translational and Clinical Medicine","volume":"52 11","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-01-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139447449","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

There is a scarcity of evidence-based therapies for those with ejection fraction (EF) ≥ 40%. The effect restoring regular ventricular response in patients with HF with EF ≥ 40% and concomitant permanent AF is unknown. Me-thods: This was a retrospective case-series study. 14 patients with symptomatic HF with EF ≥ 40% and permanent AF who had undergone permanent His bundle pacing (pHBP) were identified and enrolled. For 9 patients pHBP was a primary strategy, for the remaining patients it was an upgrade from right single chamber ventricular pacing. All patients underwent a follow-up visit 3 months after the procedure. Results: The severity of HF based on the New York Heart Association (NYHA) class was significantly reduced post-pHBP (mean 2.5 vs. 1.0, p-value < 0.001). Left ventricular ejection fraction significantly increased (mean increase 8.5%, p < 0.001) Similarly, significant de - crease in the left ventricular end-diastolic diameter was observed after pHBP (mean decrease 5.4 mm, p < 0.001). The degree of mitral regurgitation after three months was lower (mean grade 2.4 vs. 1.2, p < 0.001). Guiding these patients to rehabilitation areas is essential for the success of the process. The present case refers to a multiple trauma patient who suffered a car accident which had great impact on his upper left limb. The patient was treated at Hospital de São José, and later transferred for the Centro de Medicina e Reabilitação do Sul to start his rehabilitation. The complete description of the process in Occupational Therapy/Hand Therapy starts here, with the intention of evaluating the impact of this type of intervention on a multiple trauma patient, by assessing the evolution of the open wound, scar, edema, sensitiveness, joint range, muscular activation, limb functionality as well as Instrumental and Daily Life Activities. Despite improvements at motor and sensory levels, these were not as expected, as a result of the severity of the injuries and the late rehabilitation beginning. Nonetheless, at the functionality level the patient was able to achieve improvement levels that allowed a greater independence and a return to his job. A timely reference for treatment, in this case of multiple trauma in the upper limb, could have allowed for a greater success in the rehabilitation, at the same time, a more effective communication among all the structures involved in the case since the moment of the accident until rehabilitation would also have led to a faster closing of the wound, which would have a positive influence in the results. Although, and even with the late start, the Hand Therapy intervention was paramount for the patient’s independence.","PeriodicalId":52409,"journal":{"name":"European Journal of Translational and Clinical Medicine","volume":"26 3","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-11-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134957157","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Case Presented: Patient - 70-year-old female with metastatic CRC and peritoneal carcinomatosis. Prior to surgery, received 12 of chemotherapy FOLFOX + bevacizumab. CRS + HIPEC was performed followed by an uneventful recovery. After surgery, she has received chemotherapy with FOLFIRI. The disease progressed 14 months later. The patient has continued therapy with FOLFIRI, unfortunately, the disease has progressed and oral therapy with ftorafur has commenced. Patients’ overall survival so far is 48 months. Objective: A male child with newly diagnosed and unresectable OPG is presented to 1) review the earliest use of Antineoplastons A10 and AS2-1 for the treatment of brain tumors and 2) demonstrate the efficacy of Antineoplastons in the treatment of OPG. On April 18, 1988, a seven-year-old male child presented to the Burzynski Clinic (BC) with headaches. Based on prior, non-enhanced MRIs and biopsy, an unresectable suprasellar OPG was diagnosed. Antineoplaston therapy was started on a “proof of principle” basis. Tumor response was measured by magnetic resonance imaging (MRI) scans of the brain. Results: A brain MRI, performed on May 31, 1989, demonstrated an enhancing suprasellar nodule measuring 4.37 cm2. On August 24, 1990, brain MRI demonstrated a 0.96 cm2 enhancing nodule, indicating the achievement of a partial response (PR). Following higher dose intravenous (IV) and oral Antineoplastons, brain MRI performed on January 24, 1997, demonstrated a residual 0.4 cm2 enhancing nodule, indicating an enduring PR. All Antineoplaston therapy was discontinued on August 21, 2000. At last follow-up, > 35 years since the start of Antineoplaston therapy, the patient was healthy and showed no evidence of tumor progression. “Proof of principle” Antineoplaston therapy utilized in a seven-year-old male with unresectable OPG produced an objective response (OR) and prolonged overall survival. To the very best of our knowledge, none of these two rare conditions have previously been documented among Nigerians, within the existing literature. Case Report: Herein, is a rare case of concurrent hypoprolactinemia and IBMD triggered by marijuana abuse, in a 21-year-old undergraduate Nigerian male, who is a regular/heavy marijuana smoker of thirteen months duration. He had presented in our medical facility with complaints of recurrent excruciating lower back pain of three weeks duration which intensified with walking and while undertaking weight-bearing activities. He attested to having and seeking medical attention for, excessive sweating, and insomnia symptoms before the onset of current presenting symptoms. Results of investigations showed positive urine test for marijuana, hypoprolactinemia, distortions of biomarkers of bone metabolism, and radiologic features consistent with IBMD, Having found no other discernible cause of the low back/hip pain and hyoprolactinemia, he was diagnosed clinically with hypoprolactinemia and IBMD secondary to marijuana abuse. This warranted hospital admission where he obtained standardized specialist medical care and was subsequently discharged in good clinical condition with an uneventful follow-up period. Conclusion: This case highlights the dangers of metabolic aberrations due to marijuana abuse and the need to always maintain a high index of suspicion when confronted with it to avoid unnecessary medical protocols.","PeriodicalId":52409,"journal":{"name":"European Journal of Translational and Clinical Medicine","volume":"15 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136142954","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study aims at finding the association between erectile dysfunction and male infertility in Ibadan. Methods: This was a retrospective comparative study that analysed records of all men with male infertility between May 2016 and April 2023 at the surgical outpatient in a tertiary hospital in Ibadan. Data included demography, social habit, the severity and prevalence of ED, and type of infertility. The data were analysed with a Statistical Package for the Social Sciences (SPSS) version 25. Logistic regression analysed the association between male infertility and erectile dysfunction. A p-value of < 0.05 was regarded as statistically significant. Results: A total of 109 cases of male infertility of which 65 (59.6%) had ED. The overall mean age for primary and secondary infertility were 37.62 ± 6.95years and 40.62 ± 8.322 years respectively (t=2.892; p< 0.004). The mean age for absence and presence of ED were 39.08 ± 6.61years and 39.56 ± 7.94years respectively (t=0.340; p= 0.734). There is association between alcohol consumption and ED (χ2=6.969; p< 0.008). Similarly, statistically significant association between the ED and the type of infertility as mild, mild-moderate, and moderate ED were commoner among men with primary infertility and all men with severe ED had secondary infertility (χ2=12.283; p<0.015). Purpose to investigate the influence of the work environment of the intensive care unit on nurses and to identify the factors that play a decisive role in job satisfaction. Materials and Methods: Our study involved 100 intensive care unit nurses who completed a questionnaire focusing on nurses’ job satisfaction. Tests used are T-test, Mann-Whitney U (test of two independent samples), Anova, KruskalWallis to test distributions. The non-parametric tests analyzed with two independent variables T-test, Anova, Pearson, one-sample t-Test respectively. Results: Employees belonging to the age group of 40–49 are less satisfied with their work than employees from 30–39 years. Also, nurses with the most years of experience have statistically lower job satisfaction in terms of extrinsic job characteristics than groups with less work experience. Finally, nurses with more children are less satisfied with their work than those with fewer. Conclusion: Definitions of job satisfaction include a global affective approach with an approach to determine the overall level of job satisfaction while taking into account different individual and organizational factors that influence nurses’ job satisfaction. Contributions to Practice: The findings of this study can contribute to the improvement of working conditions and factors affecting the job satisfaction of intensive care unit nurses.","PeriodicalId":52409,"journal":{"name":"European Journal of Translational and Clinical Medicine","volume":"66 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-29","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135294503","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The study was designed to find the pattern and associated factors of maternal and newborn morbidities in Gombe, northeast Nigeria test. Materials and Methods: Patients were prospectively interviewed with the use of a structured questionnaire and medical case records were also reviewed. Data obtained was entered into SPSS version 21 for analysis. Results: A total of 250 women were studied. Most (53.6%) were aged 26–35 years. The majority (97.2%) were married. Most (40.0%) were unemployed and of the Fulani ethnicity (36.4%). Urban dwellers were in the majority (55.2%). Approx. 78% had at least a secondary level of education. Most (64.4%) were multiparous, had 2 or more living children (72%), and 76% with less than 8 ANC visits. Distribution of MOD showed C/S (52.4%), SVD (36%), and Instrumental delivery (1.6%). Maternal morbidity was recorded in 22% with PPH (47.3%), Genital injury (30.5%), and Genital sepsis (16.4%) most common. Newborn morbidity was recorded in 23.6% with neonatal asphyxia (25.4%), neonatal jaundice (25.4%), and LBW (22%) most common. Women who had less than 8 ANC visits (p = 0.05) and underwent instrumental delivery (p = 0.002) were more likely to suffer morbidity. Those who had Caesarean Section were less likely to have a maternal morbidity (p = 0.002). Whereas, neonates born to primiparous women were more likely to suffer a morbidity (p = 0.042). There was no significant relationship between number of ANC visits (p = 0.512) and MOD (p = 0.680). Conclusion: PPH, genital injuries, and genital sepsis were the commonest maternal morbidities while Neonatal asphyxia, jaundice, and LBW were commonest in the newborn. Less than eight ANC visits and instrumental delivery increased the likelihood of maternal morbidity, while delivery to primiparous mothers increased the likelihood of newborn morbidity.
