This systematic review aimed to describe the range and effectiveness of workplace-based interventions for menopause. MEDLINE, PubMed, Embase, CINAHL, Cochrane Library, Web of Science, PsycINFO, EconLit, and SCOPUS were searched from the inception until April 2022. Quantitative interventional studies evaluating physical/virtual workplace-based interventions aiming to improve well-being, work, and other outcomes, that involved women in menopausal transition, or their line managers/supervisors were eligible for inclusion. Two randomized controlled trials and three uncontrolled trials, comprising 293 women aged 40-60 years and 61, line managers/supervisors, were included in the review. Results were narratively synthesized due to the heterogeneity of interventions and outcomes and we found that only a limited range of interventions have been evaluated for their ability to support women going through menopausal transition in the workplace. Self-help cognitive behavioural therapy (CBT); Raja yoga; and health promotion (involving menopause consultations, work-life coaching and physical training) improved menopausal symptoms significantly. Self-help CBT was associated with a significant improvement in mental resources for work, presenteeism, and work and social adjustment. Awareness programs significantly improved knowledge and attitudes of both employees and line managers/supervisors about menopause. The interventions have mostly been evaluated in small studies with selected populations but have improved menopausal symptoms and work outcomes. In this study, only the data from 829 women who identified as postmenopausal were analysed.</p><p><strong>Main outcome measures: </strong>Quantitative and qualitative data.</p><p><strong>Results: </strong>Regarding women's attitudes towards the menopause before they went through it, 18.0% were accepting of it, 15.8% were dreading it and 5.1% were looking forward to it. 38.1% of women felt that the menopause was difficult, 24.6% felt it was very difficult and 20.7% felt it was fine. 94.1% of women had never been taught about the menopause at school, and 49.0% did not feel informed at all about the menopause. More than 60% started looking for information regarding menopause as their symptoms started. The qualitative thematic analysis of the participants' responses produced six themes: the need for education, knowledge and understanding of symptoms, why is getting treatment so difficult, feelings and attitudes towards the menopause, the impact of menopause on a woman's life, the importance of the media - are they getting it right.</p><p><strong>Conclusion: </strong>Women's lack of education and their healthcare professionals' lack of adequate training on the menopause means that women enter this critical life stage uneducated and unsupported. It is vital that everyone is taught about the menopause and that general practitioners receive proper training. Here, we present a case of a postmenopausal woman who presented with features of virilization like alopecia and hirsutism. During its workup, two malignancies were diagnosed at a very early stage. Microscopic focus of endometrial adenocarcinoma in a polyp and similar focus of endocervical adenocarcinoma in the subsequent hysterectomy specimen were noted. They suggested that hormone replacement therapy (HRT) use increased the risk of cardiovascular disease and venous thromboembolism including pulmonary embolism and deep vein thrombosis The findings led to a decline in HRT prescriptions and negative publicity about the use of HRT for women with significant menopausal symptoms. Subsequent studies have shown that the risk of thrombosis with HRT relates to whether estrogen is combined with a progestogen and the route of administration of estrogen. In healthy women with no background medical problems, transdermal hormone replacement is not associated with an increased risk of thrombosis. However, much less is known about the safety of various HRT preparations in women with a high background risk of thrombosis. These cases can often be challenging for clinicians with uncertainties around testing for thrombophilia, use of anticoagulation and striking a balance between the risks and benefits of prescribing HRT. This article will review the mechanism of thrombosis with differing types of HRT and present the evidence from the relevant trials. Hormone replacement therapy (HRT) is the most effective treatment option available for women experiencing classical menopausal symptoms, such as vasomotor symptoms and sleep disturbance. If HRT is to be considered, this should be based on an individualised risk assessment. Decisionmaking around HRT prescribing can be challenging in women who have a complex medical history, including women with a history of a hormone-dependent cancer. An ever-increasing number of cancer survivors now live long enough to reach the natural age of menopause or they may experience menopausal symptoms earlier as a side effect of treatment (surgery, chemotherapy or radiotherapy). When considering treatment options for menopausal symptoms, the oncologic risk of HRT – the potential to cause cancer recurrence and progression – is difficult to assess. Individualised decision-making is needed, but the evidence on which this should be based is not always clear. Collaboration with oncologists and surgeons is needed to ensure that long-term patient safety is not compromised for short-term symptom control. Guidance published recently in a joint statement from the Royal College of Obstetricians and Gynaecologists (RCOG), the British Menopause Society (BMS) and the Society for Endocrinology provides an evidence base to help clinicians advising women experiencing symptoms of menopause. It is important that women have the right information and are supported by appropriate clinicians when making decisions about their treatment and care. Alongside this, the National Institute for Health and Clinical Excellence (NICE) published helpful guidance that provides a framework for shared decision-making and advocates the use of clinical decision aids to support a collaborative approach between the clinician and the patient. In these tales, two cases are presented, relating to women with a history of pelvic cancer (endometrial and bladder cancer). Both women presented for advice in relation to managing menopausal symptoms. This study aimed to determine the relationship between urinary incontinence with sexual function and quality of life.</p><p><strong>Method: </strong>It was a cross-sectional study using cluster sampling on 433 postmenopausal women in Tabriz-Iran, 2021-2022. Data were collected using questionnaires of socio-demographic and obstetrics characteristics, female sexual function index (FSFI), Menopause-Specific Quality of Life (MENQOL), and Urinary Incontinence Diagnosis (UIDQ). Independent sample t-test and general linear model (GLM) were used to compare the quality of life and sexual function score between the two groups of women with and without stress, urgency, and mixed urinary incontinence.</p><p><strong>Results: </strong>Independent sample t-test showed a significant difference in the quality of life and sexual function between women with and without stress, urgency, and mixed urinary incontinence (<i>p</i> < 0.05). Based on the GLM with adjusting the socio-demographic and obstetrics characteristics, women without stress incontinence reported a lower mean score of MENQOL (B = - 23.38; 95% CI = -30.1 to -16.6; <i>p</i> < 0.001) and a higher mean score of sexual function (B = 4.5; 95% CI = 2.1-7.0; <i>p</i> < 0.001) compared to women with stress incontinence; a lower MENQOL score and a higher sexual function score indicate better condition.
