Lesions were reassessed and classified according to the updated Orphanet classification system for dysraphisms. Cases of open dysraphism, intermediate forms, Currarino syndrome, caudal regression syndrome and postnatally diagnosed cases were excluded. Prenatal imaging findings, pregnancy outcomes, postnatal diagnostic concordance, neurosurgical management and functional outcomes were analyzed.

Results: Seventy fetuses with a prenatal diagnosis of CSD were included. Saccular forms of CSD accounted for 85.7% (n = 60) of cases, including 33 (47.1%) cases of limited dorsal myeloschisis, 18 (25.7%) cases of posterior meningocele and nine (12.9%) cases of conus spinal cord lipoma. Most (85.7%) lesions were located at the lumbar or sacral level. Associated structural cerebral anomalies were identified prenatally in 17.1% (n = 12) of cases. There were 59 (84.3%) live births and 11 (15.7%) terminations of pregnancy, including eight cases with major cerebral anomalies. Among liveborn infants with available data on postnatal surgical interventions (n/N = 44/59), postnatal surgery was performed in 90.9% of cases. The prenatal diagnosis was adjusted postnatally in 4/39 (10.3%) cases with available postnatal outcome data. At 36 months, independent ambulation was achieved in 94.4% (17/18) of children with follow-up data available at this time point. In contrast, urinary dysfunction was present in 38.9% of cases at 36 months, while gastrointestinal disorders were less frequent (22.2%).

Conclusion: Using the Orphanet classification, prenatal ultrasound examination allows reliable anatomical characterization of CSD in 89.7% of cases. Motor outcomes are generally favorable, whereas urological morbidity is observed in nearly half of cases at 18 months. These are important elements to consider in prenatal counseling. © 2026 The Author(s). Gemelli' IRCCS, Rome, Italy, between January 2014 and June 2022 with a histological diagnosis of HGSC or invasive LGSC, who had DICOM images available from a preoperative ultrasound examination. The extracted radiomics features belonged to two different families: intensity-based statistical features and textural features. Feature selection was performed via univariate analysis using the Wilcoxon-Mann-Whitney statistical test, followed by multivariate selection using the Boruta algorithm, Pearson correlation analysis to reduce collinearity and Akaike information criterion stepwise regression. Selected features were used to train logistic regression models. To distinguish between HGSC and LGSC, two logistic regression models were developed: a radiomics model based solely on radiomics features, and a clinical-radiomics model combining radiomics features with patient age. For comparison, two additional models were constructed: a radiomics-ultrasound model incorporating radiomics features and ultrasound variables that were significantly different between the two histotypes, and an ultrasound model based exclusively on these ultrasound variables. The area under the receiver-operating-characteristics (ROC) curve (ROC-AUC) and the area under the precision-recall (PR) curve (PR-AUC) were calculated. Modeling results were internally validated via the bootstrap resampling method, and optimism-corrected metrics were computed.

Results: A total of 153 patients (120 with a histological diagnosis of HGSC and 33 with a histological diagnosis of invasive LGSC) were recruited, providing a total of 332 images for analysis. After feature selection, three textural features, 'gray-level co-occurrence matrix informational measure of correlation 2' ('F_cm.info.corr.2'), 'gray-level run length matrix run percentage' ('F_rlm.r.perc') and 'gray-level size zone matrix zone size variance' ('F_szm.zs.var') were used for the radiomics model; one textural feature (F_szm.zs.var) and age were used for the clinical-radiomics model; one textural feature (F_cm.info.corr.2) and one ultrasound variable (color score) were used for the radiomics-ultrasound model; and three ultrasound variables (color score, presence of papillary projections, presence of the ovarian crescent sign) were used for the ultrasound model. Gestational age was estimated using crown-rump length (CRL), measured independently by two experienced sonographers blinded to the other's results, and by an AI system applied twice per examination. Comparisons were made using mean absolute error (MAE) and Bland-Altman analysis. A hierarchical measurement error model was used to account for uncertainty in CRL-based estimates and to estimate accuracy relative to a latent true gestational age, with Bayesian inference.

Results: Overall, 285 women with a singleton pregnancy were included. The AI model estimated gestational age in all cases, with a median acquisition time of 23 s for the first AI measurement. The MAE between AI- and CRL-based gestational age was 2.1 (95% CI, 1.9-2.3) days for single measurements and 2.0 days when averaging both AI estimates. Limits of agreement were wider for AI-based estimates than for CRL-based estimates, indicating lower precision. When accounting for measurement error in CRL, the estimated MAE for AI was lower (1.8 days for single and 1.6 days for duplicate measurements). CRL-based estimates showed lower variability than AI-based estimates. Within-operator variation was the main source of error in AI measurements.

Conclusions: AI-based gestational-age estimation performs well in the first trimester but is less precise than gestational-age estimation using CRL measured by experienced sonographers. Measurement error models give a clearer picture of performance by accounting for variability in CRL and show that the accuracy of AI-based estimates is better than suggested by simple comparisons. © 2026 The Author(s). The study population comprised women with a singleton pregnancy identified as being at high risk for preterm PE (estimated risk ≥ 1:100) according to the first-trimester FMF triple test, who subsequently received low-dose aspirin prophylaxis. A locally estimated scatterplot smoothing (LOESS) curve was applied initially to explore the relationship between the incidence of preterm PE and the estimated risk. Based on the trend observed in the LOESS curve, participants were categorized into five estimated risk groups: 1:2 to 1:5, 1:6 to 1:10, 1:11 to 1:20, 1:21 to 1:50 and 1:51 to 1:100. Logistic and Cox regression analyses were performed to identify independent predictors of preterm PE and to evaluate the risk of delivery with PE, respectively.

Results: Among 2919 high-risk pregnant women receiving aspirin prophylaxis, 280 (9.6%) developed PE, including 107 (3.7%) who developed preterm PE. Compared to women with an estimated risk of 1:51 to 1:100, those with estimated risks of 1:2 to 1:5, 1:6 to 1:10 and 1:11 to 1:20 had a 16-fold (aOR, 15.845 (95% CI, 8.166-30.746)), eight-fold (aOR, 7.674 (95% CI, 3.925-15.004)) and three-fold (aOR, 3.454 (95% CI, 1.802-6.622)) higher odds of developing preterm PE, respectively, whereas no significant difference was observed in those with an estimated risk of 1:21 to 1:50 (aOR, 1.343 (95% CI, 0.722-2.499)). On Cox regression analysis, chronic hypertension was associated with a three-fold higher risk of delivering with PE (adjusted hazard ratio (aHR), 2.979 (95% CI, 2.154-4.120)), while MAP MoM > 1.149 increased the risk by 66% (aHR, 1.659 (95% CI, 1.304-2.111)).
