{"title":"Trends in keratinocyte cancers in England, 2013-22: basal cell carcinoma incidence, cutaneous squamous cell carcinoma incidence and nonmelanoma skin cancer mortality.","authors":"Khaylen Mistry, Nick J Levell, Dimitrios Karponis, Marlies Wakkee, David C Whiteman, Charlotte Proby, Zoe C Venables","doi":"10.1093/bjd/ljag315","DOIUrl":"10.1093/bjd/ljag315","url":null,"abstract":"
Background: Basal cell carcinoma (BCC) and cutaneous squamous cell carcinoma (cSCC), collectively termed keratinocyte cancers (KCs), are the most common cancers in White populations, and their incidence is increasing globally. KCs are substantially under-reported, and high-quality long-term data on patient demographics, incidence and mortality trends remain limited.

Objectives: To describe the characteristics of patients with KCs, and to assess incidence and mortality trends in England between 2013 and 2022.

Methods: We analysed all new KCs diagnosed in England from 2013 to 2022 using data from the National Disease Registration Service. Tumours were counted using per patient per annum (PPPA) methodology, whereby each individual could contribute one BCC and one cSCC per year. European age-standardized incidence rates (EASRs) and age-standardized mortality rates (ASMRs) were calculated using the 2013 European Standard Population, while age-specific incidence rates (ASIRs) were calculated using Office for National Statistics mid-year population estimates for the corresponding age group. Joinpoint regression was used to calculate annual percentage change (APC) and assess trends in EASRs, ASIRs and ASMRs by sex and age per 100 000 person-years (PYs).

Results: Compared with the first-all-time registration method, the PPPA approach identified 67% more BCCs and 42% more cSCCs over the study period. In 2022, PPPA EASRs were 295 per 100 000 PYs for BCC and 102 per 100 000 PYs for cSCC. BCC EASRs increased initially before stabilizing from 2015 [APC -0.2%, 95% confidence interval (CI) -0.8 to 0.3]. In contrast, cSCC EASRs increased rapidly from 2013 to 2015 (from 79 to 89 per 100 000 PY; APC 6.1%) and continued to increase between 2015 and 2019 (from 89 to 97 per 100 000 PY; APC 2.3%). The nonmelanoma skin cancer (NMSC) ASMR increased significantly from 2013 (APC 4.0%, 95% CI 2.5-5.7), with rates rising approximately twice as fast in male individuals (APC 4.4%, 95% CI 2.4-7.1) compared with female individuals (APC 2.2, 95% CI 0.3-4.6). For cSCC, ASIRs plateaued in middle age from 2015 onwards but continued to increase in younger (< 60 years) and older (≥ 80 years) individuals.

Conclusions: The findings highlight the importance of accurate tumour counting methods for healthcare planning. The incidence of cSCC continues to rise alongside increasing NMSC mortality, underscoring the need for strengthened prevention strategies, early diagnosis and timely access to quality healthcare.
","PeriodicalId":520789,"journal":{"name":"The British journal of dermatology","volume":" ","pages":"772-781"},"PeriodicalIF":9.6,"publicationDate":"2026-10-19","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148612255","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The aetiology of keloid formation is multifactorial, encompassing genetic predispositions, dysregulated inflammation and aberrant mechanical forces. Despite advances in therapeutic modalities, current clinical management strategies, such as silicone gel sheets, intralesional corticosteroids, surgical excision and adjuvant radiotherapy, often yield variable efficacy. With recurrence rates frequently exceeding 50% following monotherapy, preventing recurrence remains a major clinical challenge. This review comprehensively examines the mechanobiology of keloid formation, exploring how physical tension and extracellular matrix stiffness dictate disease progression. A central focus is the mechanosensory-immune axis, which perpetuates a vicious, self-reinforcing mechanics-inflammation-fibrosis feedback loop. We systematically decipher the key signalling cascades responsible for this mechanotransduction, specifically highlighting the transforming growth factor-β/Sma- and Mad-related protein network, integrin-focal adhesion kinase (FAK)-mediated signalling, intracellular calcium flux and the critical Hippo-Yes-associated protein (YAP)/PDZ-binding motif pathway. Furthermore, the role of the mechanosensitive ion channel Piezo1 and the convergence of mechanical and immunological signals via the Janus kinase-signal transducer and activator of transcription (JAK-STAT) axis are evaluated in depth. By elucidating these complex networks, we identify promising, mechanism-informed therapeutic avenues, including FAK, YAP and JAK inhibitors. We highlight the paradigm-shifting potential of targeting both membrane and nuclear mechanics, intracellular mechanical memory, to explain and curb the clinical intractability of keloids. Translating these mechanobiological insights into biomarker-stratified clinical trials will drive a critical shift toward precision mechanomedicine, holding profound potential to dismantle mechanical memory, prevent recurrence and improve patients' quality of life.
","PeriodicalId":520789,"journal":{"name":"The British journal of dermatology","volume":" ","pages":"735-749"},"PeriodicalIF":9.6,"publicationDate":"2026-10-19","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148595463","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
