He had a history of third-degree AV block in December 2023, where etiology could not be determined even after extensive diagnostic workup. Following a pulmonary embolism that occurred afterwards, incidental findings of unclear generalized lymphadenopathy as well as splenic and liver lesions were seen on CT scan.For further investigation, a bronchoscopic ultrasound-guided biopsy of the mediastinal lymph nodes was performed. Histological workup revealed non-necrotizing epithelioid cell granulomas, consistent with sarcoidosis. The patient's case was discussed at our multidisciplinary conference for interstitial lung diseases (ILD board).In summary, based on all findings, a diagnosis of pulmonary sarcoidosis with spleen, liver, and heart involvement was made. Retrospectively, a cardiac manifestation of the diagnosed sarcoidosis was most likely already present in December 2023, when the AV block was diagnosed.Due to the multiorgan manifestation, a combination therapy of prednisolone and methotrexate for at least one year was recommended.Sarcoidosis is a granulomatous disease of unknown etiology and, as a multisystem disease, exhibits various manifestations with heterogeneous presentations. This case illustrates that cardiac sarcoidosis should always be considered as a differential diagnosis in the case of a new-onset AV block.
","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":" ","pages":"297-300"},"PeriodicalIF":1.7,"publicationDate":"2026-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"145937395","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, the presence of an underlying rheumatologic disease significantly increases the risk. ILD associated with rheumatic disorders is linked to substantial morbidity and mortality, underscoring the importance of early recognition and prompt therapeutic intervention. Clinical presentation is highly heterogeneous, ranging from asymptomatic or subclinical disease to acute hypoxemic respiratory deterioration, which often delays diagnosis. Typical symptoms include exertional dyspnea and chronic non-productive cough, while advanced stages may manifest with digital clubbing, cachexia, and cyanosis.Pulmonary function testing, particularly body plethysmography and measurement of diffusing capacity for carbon monoxide (DLCO), together with high-resolution computed tomography (HRCT), represent central diagnostic tools. Characteristic radiologic patterns such as usual interstitial pneumonia (UIP) and non-specific interstitial pneumonia (NSIP) allow for disease stratification and prognostic assessment. Multidisciplinary discussion within an ILD board involving rheumatologists, pulmonologists, and radiologists is essential for accurate classification and individualized treatment planning. High-dose glucocorticoids are reserved for acute or subacute inflammatory presentations, whereas long-term steroid therapy should be minimized due to adverse effects. Current management strategies combine immunomodulatory agents, including conventional and biologic DMARDs, with antifibrotic therapies in progressive fibrosing phenotypes. Close interdisciplinary follow-up is crucial to detect progression early and optimize patient outcomes.
","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":"80 4","pages":"302-317"},"PeriodicalIF":1.7,"publicationDate":"2026-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147719363","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Its goal is to standardize long-term management in order to optimize survival, functional status, and quality of life. The guideline is addressed to professionals in pulmonology, internal medicine, surgery, and general practice, as well as to transplant centers and inpatient institutions in the German-speaking countries of Germany, Austria, and Switzerland. The guideline is led by the Deutsche Gesellschaft für Pneumologie und Beatmungsmedizin e. V. (DGP).Methodologically, it is based on systematic literature searches and GRADE-based evidence assessment, combined with the development of an interdisciplinary and multiprofessional consensus involving patient representatives.The guideline emphasizes lifelong, individualized follow-up care founded on close cooperation between transplant centers, community physicians, and patients. Core topics include immunosuppression - specifically the selection, combination, and monitoring of calcineurin inhibitors, antimetabolites, and mTOR inhibitors - as well as strategies to minimize adverse effects and nephrotoxicity.Additional key areas are the prevention and management of infections, including the prophylaxis of opportunistic infections (such as CMV, Aspergillus, and Pneumocystis jirovecii), and vaccination strategies. The guideline also provides recommendations for standardized patient monitoring through bronchoscopy, biopsy, and spirometry within structured follow-up intervals and preventive examinations.Further topics include acute cellular rejection, non-adherence, chronic graft dysfunction, and comorbidities such as diabetes, chronic kidney disease, osteoporosis, and tumor prevention.
","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":" ","pages":"234-270"},"PeriodicalIF":1.7,"publicationDate":"2026-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147461310","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Demographic parameters, symptoms, use of outpatient/inpatient care services and subjective satisfaction with care were recorded.In total, 3345 people (average age 49 ± 13 years; 81.5% women) completed the survey. 83.8% reported a medically confirmed long COVID diagnosis, with a further 12.2% reporting a post-vac syndrome. The average duration of symptoms was 2.8 ± 1.1 years, with only 36.4% reporting an improvement in their symptoms over time. Almost nine out of ten patients (89.1%) were on long-term sick leave (average 1.8 ± 1.3 years), 70.8% reported total or partial incapacity for work and 46.4% applied for a pension. General practitioner care was the first point of contact for 75.7%. Over the course of the illness, 93% consulted more than three and 21.5% more than ten different doctors. Personal financial contributions were high: 41.4% invested more than € 1,000 and 11.3% more than € 10,000 in diagnostics or therapy. 60% received a rehabilitation intervention. Overall, 97.2% rated their care as \"poor\" or \"very poor\".This survey highlights a high and persistent burden among long COVID patients, as well as significant socioeconomic consequences, accompanied by a predominantly negative evaluation of the current care situation. Improvements require structured, easily accessible, and cross-sectoral services. Improving the primary care system, establishing clear referral pathways, and (where clinically indicated) integrating rehabilitative interventions into an interdisciplinary care concept could help to improve the care situation of patients with long COVID.
","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":" ","pages":"283-289"},"PeriodicalIF":1.7,"publicationDate":"2026-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13090092/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"145498129","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Laboratory findings revealed a mildly elevated C-reactive protein (CRP), leukocytosis, and marked eosinophilia, while procalcitonin, liver, and renal parameters were within normal limits. Thoracic CT imaging demonstrated cylindrical bronchiectasis in the right upper lobe with mucoid impaction.Serological testing showed a markedly elevated total IgE level as well as increased Aspergillus-specific IgE and IgG antibodies. Bronchoalveolar lavage (BAL) revealed lymphocytic alveolitis with positive Aspergillus antigen and galactomannan detection, confirming the diagnosis of allergic bronchopulmonary aspergillosis (ABPA).This case illustrates that considering atypical or rare diagnoses beyond standard treatment patterns can be crucial in patients with complex or refractory disease courses.
","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":" ","pages":""},"PeriodicalIF":1.7,"publicationDate":"2026-03-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147583579","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, the prevalence of ME/CFS among PCS rehabilitation outpatients remains unclear. Medical records of 216 PCS rehabilitation outpatients (57% female; age 47.7±12.5; January 2021 to April 2022) were retrospectively reviewed. During rehabilitation and at a six-month telephone follow-up, ME/CFS was diagnosed using the Canadian Consensus Criteria (CCC). Demographics, body mass index (BMI), FAS, and 6MWT were compared between phenotype and non-phenotype groups using logistic regression, repeated measures ANOVA, and chi-square tests (α=0.05). Of 216 patients, 15 (93% female; age 40.6±10.7; BMI 25.7±5.6) met ME/CFS criteria, yielding a prevalence of 6.9%. Compared with non-ME/CFS phenotype, ME/CFS phenotype patients were significantly younger (p=0.01) and predominantly female (p=0.003). Baseline FAS was significantly higher (35.8±6.4 vs. 27.8±8.6, p=0.001) and did not improve (Δ +1.3±4.5 vs. Δ -5.1±6.2, p<0.001). Baseline 6MWT was significantly lower (479±132 m vs. 540±96.1 m, p=0.02) and both groups improved over time, but between-group change was not significant (p=0.49). Approximately 7% of PCS in outpatient rehabilitation exhibit ME/CFS, characterized by severe, persistent fatigue, female predominance, and attenuated functional gains. While the FAS is a practical screening tool, confirmation via CCC remains essential. Future studies should validate these findings and explore tailored rehabilitation strategies for patients with ME/CFS.
","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":" ","pages":""},"PeriodicalIF":1.7,"publicationDate":"2026-03-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147583582","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
