At the same time, the development of high-altitude spas was closely linked to the treatment of tuberculosis. One of the reasons for locating sanatoriums away from large cities was to minimise the risk of infection. Tuberculosis sanatoria had the image of being dangerous places that should be avoided by the local population. Their construction was often accompanied by debates that delayed or prevented them from being built. In one of the most famous health resorts of the Habsburg Monarchy, the Semmering region, there were massive conflicts with representatives of tourism interests. Although the most important medical experts in the Habsburg Monarchy spoke out in favour of the construction and its safety, the authorities could not be convinced. A contemporary newspaper carried the headline: \"The battle for Semmering\". The conflicts surrounding the establishment of tuberculosis sanatoriums are shown, and the struggle for the first sanatoriums in the Austro-Hungarian Empire is made visible. The establishment of the first tuberculosis sanatoriums was accompanied by irrational fears, which could not be dispelled by expert opinions and statistical data. In the Semmering region, local representatives felt that tourism interests were under threat. This study examined the work of Leopold Schrötter von Kristelli, a laryngologist and social physician, within the context of the sanatorium movement. Schrötter von Kristelli made great efforts to establish a tuberculosis sanatorium and encountered significant opposition in his endeavours. The essay also highlights the important work of the Austrian Central Committee for Tuberculosis under the leadership of the social physician Ludwig Teleky. Teleky encouraged several important medical associations to investigate \"danger of spreading tuberculosis through sanatoriums in their vicinity\" in order to dispel these fears and prove the safety of such institutions. The College of Physicians in Vienna drew up a memorandum addressed to the relevant authorities demanding approval for the institution, but to no avail.</p>","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":" ","pages":"134-141"},"PeriodicalIF":1.7,"publicationDate":"2026-02-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"145795994","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, performances of theses scores in pneumonia and COVID-19 as well as the impact of Omicron as predominating virus mutation on prediction performances remained to be determined.In this single-center retrospective study, the prognostic value of different scores in the periods pre-omicron (03/20-12/2021) were compared to those of the omicron period (01/2022-03/2023) in patients hospitalized for COVID-19.Of 622 study patients, 348 (56.2%) had pneumonia. Mortality was 16.2% (101 patients), with a significant difference between periods (71 (23.1%) versus 30 (9.5%), p<0.001). In patients with pneumonia, predictive performance of PSI was significantly better compared to CRB-65, CURB-65 and qSOFA (differences between AUCs PSI vs CRB-65 0.066 (95% CI 0.02-0.11, p=0.006); PSI vs CURB-65 0.047 (95% CI 0-0.09, p=0.042); PSI vs qSOFA 0.124 (95% CI 0.05-0.2, p=0.001) but not when compared to the 4C Mortality Score. There was no difference in predictive performances between periods for any score, also after excluding patients with treatment restrictions.All scores had good predictive performances for low risk and moderate for mortality in patients with COVID-19, with PSI being the best predictor for patients with pneumonia. 4C Mortality Score was a good predictior in all patients. There were no significant differences between the predictive performances of the scores in pre-omicron and omicron periods of the pandemic.</p>","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":" ","pages":"108-113"},"PeriodicalIF":1.7,"publicationDate":"2026-02-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144251970","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We report an older patient with Klippel-Trenaunay-Syndrome who presented with huge angiomatous lesions of the left upper extremity, slowly progressive dyspnea due to chronic thromboembolic pulmonary hypertension (CTEPH). Since birth his left hand, arm and shoulder were covered by venous convolutes. CT-scan showed angiomatous lesions of the thoracic wall and signs of pulmonary hypertension. Accordingly, right heart enlargement with D-sign was found in echocardiography. Right heart catheterization confirmed pulmonary hypertension with a mean PAP of 43 mmHg and a PVR of 7.5 WU. Pulmonary angiogram showed peripheral obstructions confirming the diagnosis of CTEPH. Therapeutic anticoagulation was established. As pulmonary endarterectomy was not feasible, medical treatment with riociguat was initiated and the patient underwent 5 sessions of BPA addressing 10 pulmonary segments. This resulted in significant improvement of dyspnea, right heart function, parenchymal perfusion, as well as reduction of PVR to 5 WU. We present this case because in KTS patients thromboembolism and development of CTEPH has to be kept in mind. Therapeutic options are PEA surgery, interventional with BPA, and medical treatment.</p>","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":" ","pages":"67-70"},"PeriodicalIF":1.7,"publicationDate":"2026-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144986804","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
