Spontaneous idiopathic tinnitus is a significant interdisciplinary therapeutic problem. In elderly patients it most frequently coexists with sensorineural hearing loss. The chief idea of tinnitus retraining therapy (TRT) in treatment of chronic tinnitus consists of following strategies: low level and broad band noise surroundings, diversion of the attention to other things and psychological counseling and therapy. The purpose of this study was to verify the benefits and ramifications of tinnitus retraining therapy in elderly patients suffering from chronic tinnitus with sensorineural hearing loss. Methods 30 subjects aged 65-90 years suffering from chronic tinnitus and sensorineural hearing loss were questioned about features of the tinnitus using a set of standardised questions. All of them were fitted with modern digital hearing aids and questioned about subjective hearing results after a month of follow-up. Main result 24 of the patients declared to have had considerable improvement in tolerance of the tinnitus. Main conclusion Fitting with hearing aids is an effective way of treatment in the majority of elderly patients with chronic tinnitus. Assessment of atopic status was based on medical history, physical examination, skin-prick testing, total IgE levels and nasal smears. Audiometry and tympanometry have been performed. Audiometric conductive hearing loss was found in 26.7% patients with perennial rhinitis and in 10% patients with seasonal rhinitis. Type B tympanogram were found in 20% of the patients with perennial rhinitis and in 3.33% of the patients with seasonal rhinitis. Type C tympanogram were found in 20% of the patients with perennial and in 6.67% with seasonal rhinitis. Secretory otitis media and retraction pockets were more common in patients with perennial rhinitis allergy. Local allergic reaction in the middle ear and dysfunction of the Eustachian tube can be the most responsible for these disorders. Tympanostomy and ventilation tubes were needed in secretory otitis and myringoplasty in the cases of the retraction pockets. Main efficacy criterion was defined as the change in computed tomography of paranasal sinuses, measured in V. Lund scale. In fenspiride group, overall score changed from 8.54 to 7.57 points (0.97 points i.e. 11.4% decrease, p=0.008), while in placebo group no statistically significant change was observed (change from 9.25 to 9.27 points--0.02 points i.e. 0.2% increase). Percentage of patients who experienced worsening (defined as at least 1 point increase between the beginning and the end of the study) in the placebo group was twice as big as in the fenspiride group (35.4% and 15.2% respectively; p=0.025). Clinical symptoms of chronic sinusitis--sneezing, itchy nose and purulent character of sputum--were decreasing in the fenspiride group significantly more than in the placebo group. Data from previous studies demonstrated beneficial effect of fenspiride treatment of patients with chronic sinusitis. The aim of our study was to reveal the role of the adenoids as a source of cellular components in the mucosal secretion. The adenoid removed because of its hypertrophy and the samples of surface secretions taken by \"imprint method\" described by Ebenfelt et al. from the group of 38 children were examined. By flow cytometry the lymphocyte subsets with following antigens: CD3, CD4, CD8, CD19, CD23, CD16+56, CD45RA, CD45RO, HLA-DR were analyzed. The results were compared between the groups of younger and older children and in the group of concomitant otitis media with effusion (OMS) and pure adenoid hypertrophy. The percentage of lymphocyte subsets in adenoid and secretion were similar, except for lymphocytes Th with memory phenotype which were more numerous in the adenoid and lymphocytes B CD23+ which were more numerous in the secretion. In the adenoid the percentage of T and Th lymphocytes increased with age and the percentage of B lymphocytes decreased. In the secretion the percentage of lymphocytes Th and B CD23+ was higher among older children. In cases of otitis media with effusion higher percentage of lymphocytes B (CD19+ and CD19+CD23+) and lower of lymphocytes Ts and Th with naive phenotype were observed in the secretion. In both cases the wound was healed by first intention. The cannula was not removed in the patient after CHEP, although the wide of neolarynx and spirometry parameters showed such a possibility. That patient with a huge obesity developed progressive coronary disease after surgery. Sometimes he needs to open the plugged cannula. The spirometry parameters confirmed that the patient after CHP could be decannulationed in the third month after surgery. Because of his psychological resistance that was delayed to the sixth month. The deglutition is efficient in both patients. The oesophagus tube was removed in sixth and seventh weeks after the operation. The patient after CHEP was carried out the objective evaluation of deglutition in the videoroentgenokinematography which showed a small aspiration. In spite of that he didn't develop any pulmonary complication during 2.5 years after surgery.
