The squamous cell carcinoma or anaplastic carcinoma recognized in the neck nodes suggest, that the primary focus is localised in the head and neck region.

Aim: The schedule of diagnostic procedure aiming at finding the primary focus in CUP-syndrome is presented. The knowledge of carcinoma cells spreading paths between particular neck regions is crucial for effective diagnostics of tumor localization in upper aerodigestive tract.

Results: Among 3320 oncological patients treated between 1993-2003 in Department of Otolaryngology, Head Neck Oncological Surgery 32 were diagnosed as CUP syndrome. In 17 patients the primary localisation was revealed during the follow-up period in: nasopharynx, palatine tonsil, hypopharynx, testis and breast. In 15 patients the primary focus was never found. The radical neck dissection followed by radiotherapy or chemotherapy was performed in patients with CUP. In cases when primary tumor was found, the radical surgery or radiotherapy was additionally applied.
","PeriodicalId":520738,"journal":{"name":"Otolaryngologia polska = The Polish otolaryngology","volume":" ","pages":"77-83"},"PeriodicalIF":0.6,"publicationDate":"2005-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"40931031","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"[Oncological failure after partial laryngectomy for laryngeal carcinoma].","authors":"Anna Pajor, Jerzy Kopczyński, Maciej Gryczyński","doi":"","DOIUrl":"","url":null,"abstract":"

The aim of the study was an evaluation of oncological failures after partial laryngectomies for treatment of laryngeal carcinoma. A retrospective analysis was done on 325 partial laryngectomies performed during 18 years (1980-1997) at Otolaryngology Department Medical University of Lódź. Seventy four failures (22.8%) were observed, among them local recurrences--in 43 patients (13.2%), nodal recurrences--in 9 patients (2.8%), distant metastases--in 6 patients (1.8%), second primary malignant neoplasms--in 11 patients (3.4%) and spread of neoplastic disease--in 8 patients (2.5%). Local recurrence was statistically the most often type of failure--58.1% in comparison with frequency of the others (p<0.001). The failures, according to type of partial laryngectomy, occurred rarely after cordectomy--17.5%, the most frequently were noted after supraglottic horizontal laryngectomy--42.3% (p<0.01) and fronto-lateral/fronto-anterior laryngectomy--30.9% (p<0.05). Majority of failures were diagnosed within first 2 years after primary treatment--55.4% of all failures in comparison with third (12.2%), fourth and fifth years (5.4%), but they were also often observed over 5 years (27%) (p<0.05). A statistical difference in percentage of failures in the periods until 5 years and over 5 years was noted only for cordectomy (23.8% and 4.9% respectively, p<0.001) and supraglottic laryngectomy (56.3% and 12.5% respectively, p<0.05). Systematic control of patients after partial laryngectomies is necessary all over their life.
","PeriodicalId":520738,"journal":{"name":"Otolaryngologia polska = The Polish otolaryngology","volume":" ","pages":"55-61"},"PeriodicalIF":0.6,"publicationDate":"2005-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"40930525","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The observations of such Polish physicians as Antoni Bryk (1820-1881), Jan Sedziak (1861-1932) and Zygmunt Srebrny (1860-1941) were very valuable. The good points and shortcomings of galvanocautery and principles of post-operational procedures are pointed out. The use of galvanocautery in otorhinolaryngology especially in strictures of external auditory meatus, aural polyps, aural granulations, in hypertrophy of nasal concha, nasal polyps, in atresia of nasal cavities, in hypertrophy of palatine tonsils, in tumors of naso-pharyngeal cavity, in tuberculosis laryngis are described with full particulars.
","PeriodicalId":520738,"journal":{"name":"Otolaryngologia polska = The Polish otolaryngology","volume":" ","pages":"133-40"},"PeriodicalIF":0.6,"publicationDate":"2005-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"41008296","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The history of reconstructive surgery of the facial nerve is described. Tomasz Drobnik, surgeon from Poznań, on the turn of 19th century carried out many innovatory operations in the field of orthopedics, general surgery and otolaryngology. He was extremely talented practitioner and very much engaged in scientific research and social work, which he realized in a difficult time of Prussian occupation. For over hundred years he was regarded as a pioneer in treatment of the facial nerve injuries (by uniting it with the spinal accessory). The authors present some arguments which may deny the priority of Drobnik in this procedure. The authors conclude, that Drobnik's contribution to the development of surgery has not been estimated enough. When the obstruction of the nasopharynx causes recurrent infections of upper respiratory tract, chronic otitis media secretoria or sleep apnoea, then adenoidectomy with or without tonsillectomy is indicated. The purpose of the study was analysis of postoperative hemorrhage after adenoidectomy with or without tonsillectomy. The influent of frequent infections of upper respiratory tract, disorders of blood clotting, chronic diseases, seasons of the year, operation time and general anesthesia on postoperative hemorrhage was estimated. A group of 1184 children after adenoidectomy with or without tonsillectomy was studied. Postoperative hemorrhage occurred in 59 children (4.98%). Early bleeding was frequently occurred after adenotonsillectomy and late bleeding after adenoidectomy.
