{"title":"Methanol extract of Bauhinia purpurea leaf possesses anti-ulcer activity.","authors":"Z A Zakaria,&nbsp;E E Abdul Hisam,&nbsp;M Norhafizah,&nbsp;M S Rofiee,&nbsp;F Othman,&nbsp;A H Hasiah,&nbsp;M Vasudevan","doi":"10.1159/000336593","DOIUrl":"https://doi.org/10.1159/000336593","url":null,"abstract":"<p><strong>Objective: </strong>The aim of the present study was to determine the anti-ulcer activity of a methanol extract of Bauhinia purpurea leaf (MEBP).</p><p><strong>Materials and methods: </strong>MEBP was administered at doses of 100, 500 and 1,000 mg/kg and its effects on acute toxicity, absolute ethanol- and indomethacin-induced gastric ulceration, and pyloric ligation tests in rats were investigated.</p><p><strong>Results: </strong>At a dose of 5,000 mg/kg, MEBP did not cause any signs of toxicity in rats when given orally. Oral administration of MEBP exerted anti-ulcer activity (p < 0.05) in all models tested. However, a dose-dependent protection was observed only in the indomethacin-induced gastric ulceration model. Histological studies supported the observed anti-ulcer activity of MEBP. {"title":"Concomitant drug- and infection-induced antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis with multispecific ANCA.","authors":"Milica Kontic,&nbsp;Sanja Radovanovic,&nbsp;Milos Nikolic,&nbsp;Branka Bonaci-Nikolic","doi":"10.1159/000337944","DOIUrl":"https://doi.org/10.1159/000337944","url":null,"abstract":"<p><strong>Objective: </strong>To report the first case of concomitant drug- and infection-induced antineutrophil cytoplasmic antibodies (ANCA)-associated vasculitis (AAV) in a patient treated with propylthiouracil (PTU) and suffering from tuberculosis.</p><p><strong>Presentation and intervention: </strong>A 28-year-old woman with PTU-treated hyperthyroidism presented with fever, purpura, pulmonary cavitations and ANCA to myeloperoxidase, bactericidal/permeability-increasing protein (BPI), proteinase-3 and elastase. Skin histopathology confirmed vasculitis. However, sputum examination revealed Mycobacterium tuberculosis. Remission was achieved after PTU withdrawal and treatment with antituberculosis drugs.</p><p><strong>Conclusion: </strong>Our case confirmed that BPI-ANCA are elevated in active tuberculosis. {"title":"Can ephedrine pretreatment be effective in alleviating rocuronium injection pain?","authors":"Zeynep Nur Akcaboy,&nbsp;Erkan Yavuz Akcaboy,&nbsp;Ozlem Balkız Soyal,&nbsp;Guldane Turhan,&nbsp;Nermin Gogus","doi":"10.1159/000336582","DOIUrl":"https://doi.org/10.1159/000336582","url":null,"abstract":"<p><strong>Objective: </strong>The aim of this randomized, double-blind, placebo-controlled study was to evaluate the effect of ephedrine pretreatment on the intensity of rocuronium injection pain when rocuronium was applied by timing principle and also to compare this effect with lidocaine and placebo.</p><p><strong>Subjects and methods: </strong>120 American Society of Anesthesiology risk score I-II patients scheduled for elective surgery under general anesthesia were randomized into three groups to receive either 70 μg/kg ephedrine (ephedrine group, n = 40) or 0.5 mg/kg lidocaine (lidocaine group, n = 40) or 5 ml NaCl 0.9% (placebo group, n = 40) as pretreatment. Thirty seconds after pretreatment drugs, rocuronium 0.6 mg/kg was administered by the timing principle and rocuronium injection pain scores were recorded. Twenty seconds after rocuronium administration, anesthesia was induced with thiopental and the patient's trachea was intubated. Hemodynamic parameters and adverse effects were recorded.</p><p><strong>Results: </strong>The overall frequency of having pain was 82.5, 52.5 and 22.5% in placebo, ephedrine and lidocaine groups, respectively. The multistage random cluster technique was used to obtain the sample. Questionnaire-based interviews with questions relating to water pipe smoking and sociodemographic variables were used to collect the data. The multivariate logistic regression model was used to analyse the relationships between the dependent variable (water pipe smoking) and the independent variables (sociodemographic characteristics).</p><p><strong>Results: </strong>The prevalence of water pipe smoking was 6.3% among Iranian women. Water pipe smoking was more likely in the age range of 15-24 years [odds ratio (OR) = 4.8, 95% confidence interval (95% CI) = 2.7-8.6] compared to the age range of 35 years or older, in women with a university education (OR = 1.9, 95% CI = 1.3-3.0) compared to women with a high-school diploma or lower, and in women with an average crowding index score (OR = 4.7, 95% CI = 2.1-10.6) compared to women living in small families.</p><p><strong>Conclusion: </strong>The findings of this study show that Iranian women with a university education and the younger age group (15-24 years) were at most risk of water pipe smoking. This study consisted of 272 first-degree relatives and 201 non-first-degree relatives of index patients. The participants completed a questionnaire and underwent physical and ultrasonographic examinations. The risk factors examined included age, sex, body mass index (BMI), smoking status, alcohol consumption, pregnancy, fat content in dietary meat, history of gastrointestinal surgery, hypertension, hyperlipidemia, fatty liver, coronary heart disease and diabetes.</p><p><strong>Results: </strong>The prevalence of gallstones in first-degree and non-first-degree relatives of index patients was 38.2 and 10.9%, respectively. Age, pregnancy and BMI significantly differed between cases and controls (p < 0.05). Renal biopsy revealed minimal change disease with IgA deposition. Methods: The magnetic resonance imaging (MRI) scans performed within 3 months of the clinical diagnosis of 80 patients recently diagnosed with active MS were compared to those of 80 age- and gender-matched controls with headache but without any neurological deficits for the frequency and size of VR spaces. MRI was done with noncontrast axial and coronal T1W FSE, axial T2W FSE, axial T2W FLAIR and sagittal FLAIR sequences followed by postcontrast axial and coronal T1W sequences. The frequency of VR spaces in MS patients and controls at midbrain, lenticulostriate vessels and supraventricular levels was analyzed using a two-tailed McNemar test. Results: There was no difference in the frequency of VR spaces at the levels of the midbrain, lenticulostriate vessels and supraventricular white matter between MS patients and controls. In the supraventricular region, however, there were 91 dilated VR spaces in 26 (32%) of the MS patients while in the control group, there were 8 dilated VR spaces in 6 (7.5%) patients and the difference was statistically significant (p < 0.0001). Conclusion: The data showed that dilated VR spaces in the supraventricular region could potentially be used as a marker for MS and as a prognostic tool. The differences in frequency of genetic polymorphisms were found to be statistically insignificant between these two groups.</p><p><strong>Results: </strong>Logistic regression analysis after adjustment for age, sex, smoking and hypertension showed no difference. CT uses ionizing radiation (X-rays) which may damage DNA and increase the risk of carcinogenesis. This is especially pertinent in pediatric CT as children are more radiosensitive and have a longer life expectancy than adults. The purpose of this paper is to review and elucidate the potential harmful effects of ionizing radiation in terms of solid cancer induction from pediatric CT scanning. In the light of scientific and technical developments, we will also discuss the possible strategies and ongoing efforts to reduce CT radiation exposure in pediatric patients.
