Mid-term outcomes were assessed through retrospective analysis of 29 patients who returned for a routine follow-up more than 12 months after surgery. Outcomes included trapezial space ratio (TSR), patient-reported pain (VAS), Disabilities of the Arm, Shoulder, and Hand (DASH), Patient-Rated Wrist Evaluation (PRWE), range of motion (ROM), and return-to-work time.Both groups demonstrated significant improvements in pain and all functional outcomes, with no significant difference between the groups in DASH, PRWE, and VAS scores as well as in ROM limitation or return-to-work time. TSR reduction was observed in both groups, but there was no significant difference in subsidence between the two. Minor complications occurred in both groups.Both surgical techniques improved patient-reported outcomes. Despite growing expertise and well-established surgical techniques, postoperative complications remain a significant challenge. This study analyses postoperative complications and potential risk factors.In a prospective cohort study, data from 606 patients who had undergone reconstructive body contouring surgery were collected between 2015 and July 2024. Focusing on abdominal contouring procedures, we analysed complication rates and their influencing factors, including BMI, resection weight, and type of weight loss (bariatric surgery vs. lifestyle modification). Statistical analysis was performed using the Mann-Whitney U test, chi-square test, and ROC curve analysis.The overall rate of postoperative complications was 37.5%, with wound healing disorders being most common (21.5%). Patients who had undergone bariatric surgery experienced significantly more complications than those who had lost weight through lifestyle changes (p=0.029). A higher BMI at the time of surgery (cut-off: 31) and a disproportionately high resection weight relative to BMI (index>105) were significant predictors of postoperative complications (p<0.001).These findings underscore the importance of individualised preoperative planning that accounts for factors such as BMI and resection volume. These surgeries include breast augmentation or mastectomy and aim to align the physical appearance with gender identity. This review is based on the newly published German S2k guideline \"Surgical Measures for Gender Incongruence\" and offers a comprehensive overview of current breast surgery techniques and relevant literature.A literature analysis was conducted based on data from PubMed from 1973 to July 2024. The analysis included 122 studies on gender-affirming chest surgeries for individuals with GI. The core statements were extracted and summarized.Gender-affirming chest surgeries, including both breast augmentation and mastectomy, generally lead to significant improvements in the well-being of individuals. Breast augmentations often involve silicone implants or autologous fat grafting. Mastectomies for chest flattening aim to create a smooth, masculine-contoured or neutral chest while preserving nipple sensation. The choice of surgical technique largely depends on breast size, the degree of ptosis, and skin elasticity. Patient satisfaction with the surgical outcomes is consistently high, underscoring the importance of these procedures.This review provides a comprehensive overview of current surgical techniques for chest surgeries in individuals with gender incongruence. With the growing number of transgender and non-binary patients, clear surgical guidelines are increasingly important to meet the high demands and address complex anatomical challenges. The new German S2k guideline, developed by German experts from various participating disciplines, supports this need. According to the radiocarpal arthroscopic findings, TFCC lesions are classified into intra-disc lesions (Class 1), radial lesion (Class 2), peripheral disc lesions which indicate slight to moderate DRUJ instability (Class 3) and degenerative lesion (Class 4). The radioulnar ligament (RUL) lesions that indicate moderate to severe DRUJ instability were classified with staging system with DRUJ arthroscopic findings (Stage 1 to 5). Author's treatment algorism with wrist arthroscopic findings including DRUJ arthroscopy was demonstrated and along with this algorism, various arthroscopic and open techniques to treat TFCC injuries were selected and resulted in success. Precise diagnosis of the TFCC lesions helped to select an adequate treatment for each lesion.Dieser Artikel beschreibt die pathologischen Veränderungen des TFCC sowie deren Befunde in der Bildgebung und Arthroskopie. Am TFCC lassen sich mittels Arthroskopie bei Sicht von radiokarpal zentrale (Typ 1), radiale (Typ 2) sowie periphere Läsionen (Typ 3), die mit einer moderaten Instabilität des distalen Radioulnargelenkes (DRUG) einhergehen, und degenerative Läsionen (Typ 4) unterscheiden. Läsionen der radioulnaren Bänder, die mit einer moderaten bis ausgeprägten Instabilität des DRUG vergesellschaftet sind, werden anhand der Befunde bei der DRUG-Arthroskopie in fünf Stadien eingeteilt. Diesen unterschiedlichen Befunden angepasst wurde ein Behandlungsalgorithmus mit verschiedenen arthroskopischen und offenen Verfahren entwickelt, der sich als erfolgreich erwies. {"title":"[Effect of a single proximal interphalangeal Joint Fusion of the Index, Middle or Ring Finger on the Grip and Finger Force and Load Distribution in the Hand].","authors":"Polina Dimitrova,&nbsp;Angela Reger,&nbsp;Karl-Josef Prommersberger,&nbsp;Jörg van Schoonhoven,&nbsp;Marion Mühldorfer-Fodor","doi":"10.1055/a-1750-9586","DOIUrl":"https://doi.org/10.1055/a-1750-9586","url":null,"abstract":"<p><strong>Background: </strong>Due to the functional coupling of adjacent finger joints and the quadriga effect, arthrodesis of the proximal interphalangeal joint (PIPJ) can be assumed to lead to a different grip pattern resulting in altered force distribution of the hand.</p><p><strong>Patients and method: </strong>Ten patients with isolated arthrodesis of the PIPJ due to posttraumatic osteoarthritis (4×PIPJ II, 4×PIPJ III, 2×PIPJ IV) were assessed 59 (17-121) months postoperatively on average. The angle of arthrodesis was assessed by radiographs. Grip force and load distribution of both hands were measured by manugraphy using 3 differently sized cylinders. Grip force was separately assessed and compared for the whole hand as well as for each of the fingers and each phalanx.</p><p><strong>Results: </strong>Average total grip force of the affected hand compared to the uninjured opposite side was 74% (38-136%) for the small cylinder, 104% (68-180%) for the mid-sized cylinder and 110% (69%-240%) for the large cylinder. Arthrodesis of the PIPJ of the index finger led to a reduction of the grip force (91%) for the small cylinder, but increased grip force for the mid-sized (120%) and large cylinder (139%). Grip force was reduced for all cylinder sizes by arthrodesis of the PIPJ of the middle finger (56%, 88% and 91%). Arthrodesis of the PIPJ of the ring finger resulted in a grip force of 76%, 105% and 91%, respectively, for the different cylinder sizes.The finger force of the affected finger was reduced after arthrodesis of the PIPJ, with the exception of the index finger, which was stronger than the unaffected opposite finger when using the large cylinder. The force of the healthy fingers on the affected side was greater when compared with the same finger on the opposite side, which led to increased grip force for the mid-sized and the large cylinder of the affected hand. A reduction in load distribution was measured mostly for the middle phalanx but also for the distal phalanx of the operated-on finger.</p><p><strong>Conclusion: </strong>Arthrodesis of the PIPJ almost always led to force reduction in the middle and distal phalanx of the affected finger. However, the total grip force of the hand was compensated by a higher force of the adjacent healthy fingers. In many cases, total grip force was even higher on the affected side. Several factors have been identified that could affect the quality of microsurgical performance in training or surgical procedures.</p><p><strong>Objective: </strong>The objective of this study was to assess the impact of the selected factors - caffeine, alcohol and physical exercise - on a microsurgical task prior its performance.</p><p><strong>Methods: </strong>Ten students from the 5th and 6th years of medical studies who had completed the advanced microsurgical course performed a \"6-stitches test\" on a latex glove spanned over a cup prior to and after consumption of caffeine, alcohol and performing physical exercises. The times taken to complete the task at baseline and post-exposure were recorded.</p><p><strong>Results: </strong>The results of the study show a statistically significant positive effect of caffeine and a statistically significant negative effect of physical exercise on microsurgical performance when performed shortly before the task. In contrast, a wide spectrum of different postoperative treatment regimens can be found in the current literature. The results after USO with modern, angular stable implants with immobilization for 2 weeks are presented.</p><p><strong>Patients and methods: </strong>A retrospective database analysis identified 49 patients (31 women, 18 men, mean age 37.6 years) with a total of 51 USO over a 13-year period and were followed up for 73.5 (15.9-192.9). All USO were obliquely sawed and stabilized with palmar locking implants. The wrist was immobilized postoperatively in 30° extension in a dorsal forearm splint for 2 weeks.</p><p><strong>Results: </strong>All USO showed load-stable consolidation signs after an average of 7.0 (SD 1.9; 4.9-14.1) weeks. Wrist range of motion was significantly improved in extension/flexion from 107.6° (60-155) preoperatively to 123.7° (80-160) postoperatively and in ulnar/radial deviation from 55.1° (25-90) to 60.8° (30-90) (p<0.05). Pain level was significantly reduced from 3.2 (0-8) to 0.1 (0-2) at rest and from 7.3 (4-10) to 1.2 (0-9) under weight bearing (p<0.01). A total of 5 complications (9.8%) were noted.
