Driven by lower costs, aesthetic surgeries abroad are also gaining popularity. However, there is a lack of official data on acute complications, their management, and the resulting healthcare costs.We conducted a retrospective study including all patients who required inpatient treatment for acute postoperative complications following aesthetic surgery in Germany or abroad (January 2018 to March 2024). We analysed patient characteristics, the type of aesthetic procedure, the country in which the aesthetic surgery was performed, complications, revision procedures, treatment costs, and the presence of follow-up cost insurance.Fifty patients were included in the study. The aesthetic procedures were primarily performed in Germany (44%), Turkey (32%), and the Czech Republic (16%). The most common complications were infections (54%) and wound dehiscence (34%). Re-operation was performed in 94% of patients and 8% of patients required intensive care treatment due to life-threatening complications. The average cost of inpatient treatment was EUR 6,192. Only 28% of patients had a follow-up cost insurance.Aesthetic surgery can lead to serious postoperative complications that frequently necessitate re-operation and result in high treatment costs. Training is pursued through specialist qualifications such as orthopaedics and trauma surgery, or plastic and aesthetic surgery, and it is supplemented by a two-year additional qualification (Zusatzbezeichnung, ZB) in hand surgery. Despite the significance of hand surgery, comprehensive studies on the subjective perception of training and potential areas for improvement in the German-speaking region are lacking. Given the increasing shift towards outpatient care, which relocates training procedures to the outpatient setting, this study aimed to assess the current training situation and to develop solutions for a more structured surgical education.This study sought to evaluate the quality of practical and theoretical training from the perspective of surgical trainees (Weiterbildungsassistenten, WBA), to identify weaknesses, and to develop targeted measures for optimization. A particular focus was placed on examining these factors with respect to the stage of training, both during specialist training and in the ZB hand surgery program.Between April and June 2024, an online survey was conducted among 117 WBAs in Germany. The survey included questions on demographics, training status, satisfaction, continuing education opportunities, surgical exposure, and subjective perceptions of training. Additionally, data were collected on trainees' wishes, challenges, and issues related to their education.Most participants (63% women, 42% aged 30-34) were enrolled in specialist training (58%) or the additional qualification (ZB) hand surgery program (42%). While satisfaction increased with each year of training, 51% rated their surgical exposure as insufficient. A total of 56% desired more surgical exposure, while 18% sought better individualized mentorship. Frequently reported challenges included staff shortages and inadequate support for continuing education.The findings suggest that targeted measures - such as increased surgical exposure, structured mentoring programs, and better financial support for continuing education - could significantly improve the perceived quality of training. The affected finger is placed in increased extension (relative motion extension splint, RMES) or flexion (relative motion flexion splint, RMFS) relative to the neighbouring fingers, allowing for active tendon gliding within a protected range of motion at the site of repair or injury. While originally introduced for the treatment of extensor tendon injuries, the concept of relative motion splinting is now being used for a growing number of indications. It is characterised by a compact splint design that enables early return to work with minimal restrictions in daily activities. Successful surgical treatment requires intensive rehabilitation. However, it is often difficult for patients to obtain timely appointments with a therapist. Several studies have already demonstrated the potential of home exercises. The aim of this study is to investigate the effectiveness of an additional hand therapy app compared to physiotherapy alone.This is a prospective, randomized, controlled, open-label study. A total of 112 patients aged 18 to 65 years (MV±SD: 36.9 years±15.5) with metacarpal and finger fractures as well as flexor and extensor tendon injuries participated. The app uses artificial intelligence (AI) and the smartphone's integrated camera to capture finger movements and determine the range of motion (ROM). Furthermore, the integrated AI automatically adjusts the type and intensity of treatment based on the patient's therapy progress and symptoms. The patients were divided into two groups. The intervention group (IG) received the app Novio Hand for 12 weeks after the immobilization phase in addition to hand therapy (18 units) as standard care (SoC). The control group (CG) received SoC alone. Improvement in ROM was measured at baseline and after 2, 6, and 12 weeks.Independent t-tests showed significantly greater ROM in the IG compared to the CG at 2 and 6 weeks (p=0.02). A significant trend was observed at 12 weeks. In the IG (50%), significantly more patients achieved the minimal clinically important difference (MCID) of 40 degrees compared to the CG (26%). At 6 weeks, the difference was also significant (IG: 79%, KG: 54%). In the IG, fractures showed an almost full range of motion on average after 6 weeks, whereas in the CG, significant movement deficits could still be quantified after 12 weeks.Therapy with the Novio Hand app, in addition to SoC can accelerate rehabilitation and improve functional results. Demographic data, fracture classification, suitability for surgery, preoperative pain scores, and waiting times were collected prospectively. Perioperative parameters, clinical outcomes (pain scores, Disabilities of the Arm, Shoulder and Hand [DASH] scores), complications, patient satisfaction (Visual Analogue Patient Satisfaction [VAPS] scale), and return to work were assessed.Patients in the WALANT group had significantly shorter waiting times for surgery and postoperative hospital stays than the IC-BP group. No significant difference was found in intraoperative blood loss. The WALANT group had lower pain scores at postoperative weeks 2, 6, and 12. DASH scores improved over time in both groups, with the WALANT group having a significantly lower mean score during the follow-up period. Patients in the WALANT group returned to work earlier, and patient satisfaction was significantly higher.Compared with IC-BP blockade, the WALANT technique offers several advantages in DRF surgery, including shorter waiting times, shorter postoperative hospital stays, lower pain scores, better functional outcomes, faster return to work, and higher patient satisfaction. The WALANT technique offers a safe and effective alternative that eliminates the need for general anaesthesia or tourniquet application. This technique is suitable for DRF surgery because it provides consistent results and does not require a high level of expertise.
