The 2022 World Falls Guidelines (WGs) provide a validated algorithm to stratify fall risk (low, -intermediate, or high) and guide tailored prevention strategies. However, large-scale implementation of the WGs faces major barriers, including limited healthcare resources. A prescreening self-administered tool could optimize resource use by identifying individuals at potential moderate or high risk who warrant HCP assessment, while empowering older adults in their own care. To describe the development process of a self-administered questionnaire (APP-Chutes®) designed to pre-screen adults aged 65 and over for moderate or high fall risk, as defined by the WGs algorithm, with the aim of subsequent digitalization as a mobile application. A multidisciplinary expert group - including WGs contributors, geriatricians, a physiotherapist, a patient's representative, and methodologists - met over three days across six months. Inspired by the MASK-air® mHealth app model, the group defined the required qualities of the questionnaire, established an item development methodology, and designed a two-stage validation strategy (development and validation studies). For each WGs algorithm parameter, the group selected the most appropriate self-question(s) format - binary or visual analogue scale (VAS) - prioritizing simplicity, caregiver-assisted completion, and future digitalization. The group developed a self-questionnaire comprising binary questions and VAS items replacing WGs physical tests (walking speed, Timed Up and Go, grip strength). Key innovations include splitting ambiguous questions, applying VAS for subjective parameters (unsteadiness, fear of falling, fatigue, physical activity), and defining VAS thresholds to align with WGs binary outcomes. A two-stage prospective validation methodology, using weighted Gwet coefficient and ROC analyses, was defined. APP-Chutes® represents a novel, patient-centered pre-screening tool designed to facilitate the large-scale roll-out of the World Falls Guidelines. Validation studies will determine its accuracy and support its digitalization as a mobile application.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 2","pages":"167-176"},"PeriodicalIF":0.3,"publicationDate":"2026-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148400493","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Prospective data from individuals aged 60 years and older without loss of independence who were included in the ICOPE Corsica program between July 2022 and November 2024 were analysed. Limited mobility was identified using the chair rise test, followed by an in-depth assessment including the Short Physical Performance Battery (SPPB). The characteristics of older adults with limited mobility were compared with those of older adults assessed for impairment in other intrinsic capacities using Student's t-test, chi-square tests, and multivariate analysis. Among the 1,184 older adults included (70% women; mean age: 74 years), 124 (10%) had limited mobility, of whom 70 underwent an in-depth assessment. Among these participants, 31% had already experienced a fall and 87% had poor physical performance according to the SPPB. Compared with the other older adults assessed (n = 246), those with limited mobility were more likely to live in rural areas and to present three or more impaired intrinsic capacities (p < 0.05). These associations were confirmed by multivariate analysis. These results suggest that limited mobility, as defined in step 1 of the ICOPE program, particularly affects older adults living in rural areas and is frequently associated with severe mobility impairment as well as with other alerts indicating established frailty.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 2","pages":"158-166"},"PeriodicalIF":0.3,"publicationDate":"2026-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148400502","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study aimed to explore physicians' responses to suspected cases of intrafamilial elder abuse. A qualitative study informed by grounded theory was conducted, based on ten semi-structured interviews with physicians from various specialties. Physicians evaluated suspected abuse situations and implemented medico-social interventions while preserving the therapeutic alliance. Legal authorities were involved when there was an immediate risk to the older person or when alternative measures had failed. Physicians sought collegial discussion, although the relevant stakeholders were not always clearly identified. These conditions are associated with a high risk of both short- and long-term complications in the elderly population, including hospitalisation, decompensation of comorbidities and loss of functional independence. Some of these infections are preventable through measures such as vaccination, which nevertheless remains underused in France. This insufficient prevention contributes to an increased incidence of bacterial infections in a population already disproportionately affected, and consequently to the widespread prescription of antibiotics. Such overexposure is one of the drivers of antimicrobial resistance, a major public health challenge that is expected to worsen in the coming years. Here again, several strategies are available, including the appropriate use of antibiotics and, once more, vaccination. Adapting our practices accordingly is essential, both in the interest of patients and for the protection of public health.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 1","pages":"9-14"},"PeriodicalIF":0.3,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147871248","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The aim of this study is to assess in-hospital mortality risk factors in geriatric patients. This is a retrospective multicenter study. We included all patients hospitalized in geriatric wards with a positive nasopharyngeal rt-PCR for RSV during the winter seasons between November 2018 and April 2023 from three Parisian hospitals. We used a chi-square test for the ordinary variables and a Wilcoxon test for the quantitative variables to define the characteristics associated with in-hospital death. 58 patients were included, median age 89 years (72-98 years), 42 (72%) women, mean daily living activities 3.9, 10 (17%) living in nursing homes. They had several comorbidities: malnutrition (81%), neurocognitive disorder (74%), chronic heart (43%) or kidney (40%) failure. Mortality was higher in patients with dyspnea (90,0% versus 35,4% ; p = 0,003), need for oxygen supplementation (80,0% vs 35,4% ; p = 0,014), acute respiratory distress (40,0% vs 8,3% ; p = 0,024), acute heart failure (70,0% vs 27,1% ; p = 0,023), delirium (70,0% vs 22,9% ; p = 0,007), viral pneumonia (70,0% vs 39,6% ; p = 0,095), thromboembolic disease (20,0% vs 2,1% ; p = 0,074). The median hospitalization duration was 22 days (4-186 d). Although our cohort was of small size, compared with the literature, we found a higher rate of in-hospital mortality (17%) and a longer duration of hospitalization in our frail geriatric patients. The low hospitalization rate for RSV is probably explained by a lack of screening for RSV in covid epidemic. A preventive strategy against RSV seems highly relevant in this population.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 1","pages":"102-110"},"PeriodicalIF":0.3,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147871263","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The lack of social support has been independently associated with the progression of cognitive symptoms and the worsening of behavioral and psychological symptoms of dementia (BPSD) in residents with major neurocognitive disorders (MND). However, few studies have examined the evolution of these symptoms in relation to movement restrictions and isolation, independent of Covid-19 infection. This study aims to assess the impact of lockdown measures on the severity of BPSD in NH residents with MND. A retrospective, descriptive, and analytical multicentric study was conducted in 15 NH in the Indre-et-Loire region, France. A total of 251 residents diagnosed with MND by their primary care physician or a specialist were included. The severity of BPSD was assessed using the Neuropsychiatric Inventory (NPI), and scores were compared before and after the lockdown in March 2020. Of the 251 residents included, a total of 240 NPI measurements were performed before the lockdown, involving 222 residents, and 95 NPI measurements were conducted after the lockdown, involving 84 residents. The average NPI score after the lockdown (39.3) was higher than the average score before the lockdown (37.7), although this difference was not statistically significant (p = 0.3). The observed increase in NPI scores following the lockdown suggests a trend towards worsening BPSD related to the lockdown, although this trend did not reach statistical significance. Further studies with a more robust methodology and larger sample size are needed to confirm these findings and to better understand the impact of isolation measures on this vulnerable population.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 1","pages":"111-117"},"PeriodicalIF":0.3,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147871260","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This historical review of late-onset visual hallucinations (LVH)-that is, hallucinations occurring exclusively in elderly individuals (as opposed to those seen in psychotic disorders or delirium, which may occur at any age)-also provides an opportunity to discuss Lewy body dementia (LBD), a condition that remained poorly recognized until the late 20th century and in which hallucinations constitute a core clinical feature. Although FTLD is less frequent than Alzheimer's disease (AD) in older adults, recent epidemiological studies indicate that its incidence increases with age, peaking around the seventh decade, thereby challenging the traditional view of FTLD as a disorder predominantly affecting younger individuals. In older patients, diagnostic accuracy is reduced by the frequent absence of typical early-onset features. Late-onset FTLD often presents with less characteristic clinical phenotypes, including attenuated behavioural symptoms, misleading memory impairment suggestive of AD, and subtle motor or language manifestations, all of which contribute to delayed recognition. From a neuropathological perspective, late-onset FTLD displays distinct features compared with earlier-onset forms, including less pronounced frontotemporal atrophy, a higher frequency of mixed pathologies, and the presence of age-related lesions, which may influence clinical expression. In clinical practice, the identification of FTLD in older adults relies on a multimodal approach integrating detailed clinical and neuropsychological evaluation with structural or functional neuroimaging. In the absence of FTLD-specific biomarkers, cerebrospinal fluid biomarkers of AD are frequently used to support the diagnostic process, primarily to identify or exclude concomitant AD pathology, although their interpretation requires caution in this age group. Genetic analyses may also contribute to the diagnostic assessment, particularly in presentations compatible with the behavioural variant of FTLD, including late-onset cases. Overall, integrating clinical, neuropsychological, imaging, biomarker, and genetic data is essential to improve the recognition of late-onset FTLD and optimize patient management in this population.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 1","pages":"42-58"},"PeriodicalIF":0.3,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147871256","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Their collaboration may facilitate its identification. This study aims to assess healthcare professionals' awareness of frailty in older adults and its screening, and to describe the organization of interprofessional collaboration. A qualitative study was conducted using semi-structured group interviews and an approach inspired by ongoing thematic analysis. Sixteen healthcare professionals (physicians, nurses, and physiotherapists) from a community-based outpatient health center in France participated on a voluntary basis. Healthcare professionals were aware of frailty in older adults and described its multidimensional nature as well as the challenges associated with its screening. However, they did not report systematic or standardized identification practices. Collaboration, fostered by group practice, appears essential for frailty screening in outpatient care. It places the general practitioner at the center of care, supported by allied health professionals. Differences in practices and in relationships with patients increase the diversity of perspectives, thereby facilitating a more comprehensive identification of frailty. Current practices, combined with limited knowledge of screening tools, do not support the systematic detection of frailty. However, healthcare professionals show interest in such screening when it aims to improve quality of life and prevent loss of independence in their patients. The Memory Binding Test (MBT) was designed for this purpose, targeting episodic memory binding through structured, semantically cued encoding, thereby reducing executive demands. Although a French adaptation of the MBT has been validated, no version has yet been developed for the Quebec French population, despite known linguistic and cultural differences that may affect verbal memory performance. This study aimed (1) to validate a Quebec French adaptation of the MBT derived from the French version developed in France, and (2) to assess its convergent validity with the Rey Auditory Verbal Learning Test (RAVLT), a widely used but more executive-dependent episodic memory task. Materials and methods. Fifty-two participants aged 55 years and older, all professionals or executives, completed both tests in a remote cognitive assessment setting. Moderate to strong correlations were observed between MBT free recall and RAVLT recall scores, increasing across RAVLT learning trials. However, no significant correlation was found for recognition scores, likely due to methodological differences and ceiling effects in the RAVLT. Paired comparisons showed significantly higher free recall performance in the MBT than in RAVLT trial 1, but lower than after five trials, reflecting the MBT's structured encoding advantage versus the cumulative learning effect in the RAVLT. Recognition scores were significantly lower in the MBT, consistent with its higher cognitive demands (i.e., source memory). Overall, these findings support the convergent validity and clinical relevance of the Quebec MBT, particularly for detecting subtle episodic memory deficits in aging. Its structure makes it less reliant on executive strategies, positioning it as a complementary tool to traditional memory tests such as the RAVLT for the early detection of AD-related memory decline in cognitively high-functioning individuals.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 1","pages":"90-101"},"PeriodicalIF":0.3,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147871297","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
