From a global health perspective, in which human health is closely linked to surrounding fauna, flora, and the broader environment, environmental exposures are key determinants of healthy ageing. We conducted a narrative review (2000-2025) based on PubMed, Google Scholar, and institutional reports (World Health Organization, IPCC, Inserm). The review focused on adults aged ≥ 60 years and the health impacts associated with soil, water, and air pollution, as well as climate change. A total of 44 sources were ultimately included. Findings indicate that soil and water contamination from pesticides are linked to increased risks of Parkinson's disease, neurocognitive decline, and cancer. Air pollution, including fine particulate matter and carbon monoxide, raises the incidence of chronic obstructive pulmonary disease, lung cancer, and pneumonia. Climate change contributes to cardiovascular and respiratory decompensations, infectious diseases, and undernutrition, further exacerbated by extreme weather events such as droughts, floods, and wildfires. These adverse outcomes are amplified by intrinsic factors (age, comorbidities, socioeconomic status) and extrinsic factors (housing, infrastructure, healthcare system). Protecting older adults from environmental threats should be recognized as a new cornerstone of geriatric care, requiring structural policies in urban planning and the integration of environmental health into healthcare professionals training.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 1","pages":"15-29"},"PeriodicalIF":0.3,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147871292","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Moreover, certain medications may exacerbate these disorders. The aim of this study was to assess the prevalence of such medications in patients aged 65 years or over with swallowing disorders hospitalized in a post-stroke unit. An observational study was conducted in a geriatric hospital among patients aged 65 years or over admitted for stroke or transient ischemic attack (TIA). A literature review was first carried out to identify potentially iatrogenic medications. The primary outcome was the prevalence of the use of these medications within 14 days following the assessment of swallowing function. Among the 284 patients included, 113 (39.8%) had swallowing disorders, of whom 110 (97.3%) were taking one or more medications likely to worsen the condition (mean 4.7 ± 2.3 medications). The three most commonly identified medications were amlodipine (9.6%), oxazepam (9.3%), and aspirin (8.7%). The main mechanism involved was dry mouth. These medications are frequently prescribed for the management of risk factors and sequelae following stroke or TIA, highlighting the importance of a multidisciplinary evaluation of their benefit-risk balance.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 1","pages":"80-89"},"PeriodicalIF":0.3,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147871280","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

An 86-year-old patient presented with coma, hypothermia, and sweating. The differential diagnoses of hypothermia were ruled out and a diagnosis of a variant of Shapiro syndrome was made. Brain imaging revealed small corpus callosums. Treatment was symptomatic, with external warming and management of complications. The integration of advanced practice nurses (APNs) is a possible approach that remains underexplored in this setting. Objective. To explore healthcare professionals' perceptions of the APN role in nursing homes and to identify tasks that could potentially be shared with coordinating physicians. A multicenter descriptive study was conducted in the Val-d'Oise department (France) between November 2024 and February 2025. Three self-administered questionnaires were distributed to coordinating physicians, coordinating nurses, and nursing home directors. The analysis combined descriptive and comparative approaches, including a cross-analysis of perceived delegation and reported practices. A total of 131 professionals from 75 facilities participated. Several tasks were identified as potentially shareable, including geriatric assessment, care coordination, staff training, and risk management. Medication prescribing was largely perceived as non-delegable. Differences in perception were observed between professional groups, with coordinating nurses generally more supportive of expanded APN roles than physicians, consistent with observed differences in practice patterns. This study shows a gradient in the acceptability of task sharing with APNs, depending on the type of task. It identifies clinical and organizational areas where roles could evolve. Experience from other countries suggests that structured task-sharing models can improve care coordination and access to care. These findings support the need for pilot studies to assess the integration of APNs in French nursing homes.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 1","pages":"71-79"},"PeriodicalIF":0.3,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147871277","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Its diagnosis remains challenging due to its nonspecific and often subtle clinical presentation, typically including confusion, cognitive decline, gait disturbances, or somnolence, which may mimic other geriatric syndromes. Neuroimaging is essential, with non-contrast CT as the first-line modality, complemented by MRI in selected cases to better characterize complex or atypical lesions. Surgical evacuation via burr-hole trepanation remains the cornerstone of management in symptomatic patients, particularly when associated with significant mass effect. The addition of subdural drainage and intraoperative irrigation with warm saline (37 °C) has been shown to reduce postoperative recurrence. Recently, middle meningeal artery (MMA) embolization has emerged as a promising minimally invasive technique aimed at occluding the vascular supply of the neomembrane responsible for persistent microbleeding. Recent randomized controlled trials have demonstrated the efficacy and safety of MMA embolization in reducing recurrence rates, either as a primary or adjunctive therapy, particularly in high-risk or inoperable patients. Beyond the procedure itself, patient outcomes largely depend on a multidisciplinary approach encompassing neurosurgery, interventional neuroradiology, anesthesiology, geriatrics, and rehabilitation. Optimal perioperative management includes correction of coagulation disorders, monitoring of fluid balance, and individualized postoperative surveillance. The integration of novel endovascular strategies with traditional surgical treatment opens new avenues for personalized care and improved prognosis in this vulnerable population. Further research is warranted to refine patient selection criteria for embolization, evaluate long-term outcomes, and determine its place in treatment algorithms.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 1","pages":"30-41"},"PeriodicalIF":0.3,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147871258","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Nevertheless, the level of complete vaccination (including all recommended boosters) is not optimal, even in the most vulnerable population, i.e. older adults living in nursing homes (NH) and long-term care units (LTCU). In this context, the understanding of the patients' reluctance to vaccination seems particularly important and our objective was to understand the acceptance or refusal mechanisms of the Covid-19 vaccine among people living in NH and LTCU. National French cross-sectional study conducted between June and September 202 including 101 health care professionals working in NH or LTCU and represents 10,152 residents living in the respondents' establishments. The Vaccovid-Senior cohort included 323 residents (89 ± 6.5 years, 69% women, 89.2% living in NH); 179 agreed to vaccination and 144 were not vaccinated. In multivariate analysis, the only modifiable characteristics associated with the acceptance was a regular influenza vaccination (OR = 29.43; 95CI [12.11-71.53]; p < 0.001), and with refusal was receiving information from Internet and social media (OR = 0.34; 95CI [0.13-0.88]; p = 0.026). Remarkably, the interaction analysis revealed that the residents who were usually vaccinated against influenza and able to understand the information given about Covid-19 were 17,5 times more likely to accept the Covid-19 vaccination (RR = 17,5; 95CI [4,5-68,0]) compared to residents with polypharmacy but without regular influenza vaccination. Vaccine acceptance can be improved by repeated efforts to listen to and explain the vaccine to the older population, their families, and the care teams. Isolating these situations of refusal allowed us to foresee avenues of improvement such as the communication of information on the vaccine, the nudge strategy and training to give information adapted to the level of understanding.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"23 4","pages":"417-426"},"PeriodicalIF":0.3,"publicationDate":"2025-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"146109651","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This article reports the results of a qualitative study, for which data from 19 individual interviews with nurses were collected on their experience in caring for patients over the age of 65 who were assessed in psychiatric emergency departments. A semi-structured interview guide was used during the recorded interviews. Inductive thematic analysis identified nine themes of interest: positive aspects of care, difficulties, most memorable consultations, care of suicidal patients, cognitive disorders identified by nurses, isolation and restraint measures, exchanges with caregivers, cases of abuse, and areas for improvement suggested by nurses. The results of this study suggest that nurses perceive specific training in geriatric psychiatry, combined with the development of protocols for the care of elderly patients, as levers for improving their care. Thus, a protocol for the nursing care of elderly suicidal patients in psychiatric emergency departments has been developed.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"23 4","pages":"493-502"},"PeriodicalIF":0.3,"publicationDate":"2025-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"146109673","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

While much attention has been given to the psychopathological manifestations frequently observed post-stroke (such as mood disorders, agitation, and aggression), there is a risk that the potential role of the psychological dynamics may be overshadowed by a more direct symptomatic reading of the cerebral lesion. Nevertheless, stroke represents a significant existential disruption, a somatic and psychological experience whose sequelae and resulting psychological suffering profoundly impact quality of life. The aim of this article is to outline certain consequences of stroke on the psychological organization and the ways in which the individual experiences and appropriates this event. It will be shown that, alongside functional and cognitive impairments, there are multiple losses of individual and social supports that further destabilize the subject, thereby heightening vulnerability. With a commitment to supporting the psychological life of these individuals, we have implemented a therapeutic intervention in our clinical practice through drawing mediation, aimed at facilitating communication and fostering new forms of expression. Using a single-case study approach, we present the therapeutic process applied to a brain-injured nursing home resident exhibiting behavioral disorders, and its impact on the emergence of a psychological dynamic conducive to emotional relief.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"23 4","pages":"503-511"},"PeriodicalIF":0.3,"publicationDate":"2025-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"146109655","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Its application may be influenced or worsened by structural factors, such as staffing shortages. This study aimed to explore the relationship between caregiver burden and the use of physical restraints, with particular attention to whether the caregiver was the initiator or the applier of the restraint. Fifteen caregivers assessed their perceived burden in relation to 34 residents across two different care facilities. Data collected included Neuropsychiatric Inventory (NPI) scores, Iso Resource Group (GIR) scores, Mini-Mental State Examination (MMSE) scores, the number of restraints per resident, and the duration of restraint use. Caregiver burden was significantly higher in the presence of physical restraint (β = 14.68; p = 0.005). In contrast, greater caregiver experience (β = - 0.20; p = 0.039) and higher MMSE scores (β = - 0.175; p = 0.005) were associated with reduced burden. Caregivers who physically applied the restraints reported significantly greater burden (F = 4.60; p = 0.039). Interestingly, the highest burden was observed in the facility with the lowest restraint-to-resident ratio. These findings suggest that both the use of physical restraints and being the individual who applies them contribute to an increased caregiver burden. However, the relationship between these variables appears to vary by facility and restraint duration, indicating that structural factors may influence the use of restraints and their impact on staff.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"23 4","pages":"475-484"},"PeriodicalIF":0.3,"publicationDate":"2025-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"146109647","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
