This study aimed to determine the incidence of bacterial pneumonia and identify its associated risk factors in a nursing home population. This retrospective observational study was conducted over a one-year period in a nursing home affiliated with a university hospital in France. The incidence of bacterial pneumonia was determined, and potential risk factors were evaluated using descriptive, univariable, and multivariable statistical analyses. A total of 392 residents were included (mean age, 84.5 years; 41.4% men and 58.6% women). Seventy-three residents (18.6%) experienced at least one episode of bacterial pneumonia during the study period. Heart failure was independently associated with an increased risk of bacterial pneumonia (odds ratio [OR] 3.22, p = 0.002), as were swallowing disorders (OR 2.11, p = 0.011). Conversely, neurocognitive disorders and a history of chronic alcohol use were associated with a lower risk of bacterial pneumonia (OR 0.22, p = 0.0002, and OR 0.27, p = 0.007, respectively). Heart failure and swallowing disorders were identified as independent risk factors for bacterial pneumonia among nursing home residents. The inverse associations observed with neurocognitive disorders and chronic alcohol use should be interpreted cautiously because they may reflect residual confounding or diagnostic bias. Although pneumococcal vaccination was not associated with a lower incidence of bacterial pneumonia in this cohort, previous studies have consistently demonstrated its effectiveness in reducing disease severity.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 3","pages":"0"},"PeriodicalIF":0.3,"publicationDate":"2026-08-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148724189","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Neuropathologically, LATE is characterized by the abnormal accumulation of TDP-43 protein in limbic structures, particularly the hippocampus and parahippocampal regions. Clinically, LATE presents as a slowly progressive, isolated mesiotemporal amnestic syndrome, typically affecting individuals aged over 75 years. Brain MRI usually reveals marked hippocampal atrophy, while FDG-PET may demonstrate medial temporal hypometabolism. A diagnosis of probable LATE requires the exclusion of underlying amyloid pathology, although concomitant Alzheimer's disease pathology is common in older adults. To date, no symptomatic or disease-modifying pharmacological treatment has demonstrated efficacy in LATE. However, its clinical course appears to differ from that of typical Alzheimer's disease, with potentially slower progression and longer preservation of functional independence. LATE therefore represents a common and likely underrecognized cause of memory impairment in older adults, and its identification has important implications for diagnosis, prognosis, and therapeutic decision-making.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 3","pages":"0"},"PeriodicalIF":0.3,"publicationDate":"2026-07-21","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148552058","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In Alzheimer's disease (AD), autobiographical memory progressively deteriorates, disrupting personal narratives and self-perception. This article provides an integrative review of current knowledge on autobiographical memory and its impairment in AD. Neuroanatomical correlates are discussed, highlighting the key role of the hippocampus and its interactions with the frontal and parietal cortices and the amygdala. Preserved cognitive resources and current supportive strategies, including multisensory stimulation, narrative reconstruction, and immersive approaches, are also examined. The role of family caregivers is emphasized, as mediators of memories, supporters of identity continuity, and contributors to shared memory processes. Future perspectives include the development of digital tools and integrative methodologies aimed at bridging experimental rigor and the lived experience of patients. Such approaches are essential for advancing our understanding of autobiographical memory decline in AD and for developing tailored interventions that support identity, autonomy, and psychosocial well-being.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 2","pages":"207-219"},"PeriodicalIF":0.3,"publicationDate":"2026-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148400439","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

When carefully assessed, cognitive neurofunctional disorders appear to be relatively common in memory clinics, accounting for up to 20% of consultations. The widespread adoption of the term \"functional\" over the past decade reflects the non-organic nature of these disorders and is generally well understood by patients. This terminology has progressively replaced earlier terms such as \"psychogenic\" and the older concept of hysteria. Functional cognitive complaints are often unusual or incongruent and display distinctive clinical features that may appear symbolic, disproportionate, or inconsistent with anatomical, physiological, or theoretical models. Cognitive symptoms are frequently excessive in relation to the clinical context, neurological examination, biological findings, or psychometric results. Symptom onset may occur after a delay following a precipitating somatic or psychological event. Marked subjective complaints or exaggerated impairment during testing may contrast with preserved autonomy in everyday life. Loss of automatic procedures, such as writing or signing one's name, is frequently observed. The pathophysiology of these disorders remains poorly understood. However, amplification of emotional processes, elevated stress hormones, and heightened attentional focus may, under certain conditions, inhibit specific neural networks. Such dysfunction may be demonstrated by hypometabolism on functional neuroimaging studies. Dissociative amnesia, previously referred to as \"functional amnesia\" in the 1990s, also appears to involve emotional and stress-related mechanisms. Clinically, it is characterized by altered consciousness, particularly disturbances of identity, together with disproportionate retrograde amnesia of variable extent, which explains the current preference for the term \"dissociative.\" Careful listening and clinical attention are essential for improving the identification of these disorders. Large-scale neuroscientific studies are urgently needed to better understand their pathophysiology and to develop therapeutic trials, including non-pharmacological approaches. Achieving this goal will require improved clinical characterization of these conditions.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 2","pages":"230-239"},"PeriodicalIF":0.3,"publicationDate":"2026-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148400470","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The aim of this study was to provide a French adaptation of the STOPPCog criteria to facilitate their use by French healthcare professionals. The cross-cultural adaptation into French was conducted by two experts, confirmed through a forward-backward translation process, and finalized based on feedback from ten French reviewers (geriatricians and clinical pharmacists). Validation was performed by assessing inter-rater agreement using the intraclass correlation coefficient (ICC) after application of the STOPPCog criteria to five standardized clinical vignettes. The 23 criteria of the STOPPCog tool, organized into six sections, were retained with the same classification as in the original English version, while being adapted to French clinical practice. Back-translation confirmed preservation of the original clinical meaning, and good agreement among the ten reviewers was observed for the identification of PIMs (ICC = 0.791). The French adaptation of the STOPPCog criteria represents an important step toward facilitating their implementation in France. Adjustments based on recent French guidelines and the literature strengthen its relevance in the national context. As a secondary objective, we investigated differences according to educational level in pharmaceutical and speech-language assessments and recommendations. A total of 110 patients were included. Polypharmacy was identified in 55% of patients, and at least one pharmaceutical intervention was recommended in 75%. A high prevalence of language disorders was observed, leading to referral for speech therapy in 61% of cases. These situations may require close collaboration between nephrologists and geriatricians. However, in France, little is known about the current state of such collaborations or the training needs expressed by geriatricians regarding nephrological care for older adults. To identify nephrogeriatric collaborations and geriatricians' wishes for training in this field, a questionnaire was developed by the Action Group in Nephrology of Older Persons (Granpa) of the Société française de gériatrie et gérontologie (SFGG) and was disseminated through its constitutive societies between March and May 2025. Among 252 geriatricians, 89% reported having access to a nephrologist and 37% identified someone to refer to for nephrogeriatric questions. Geriatricians reported referring to nephrologists more frequently than the reverse. The most frequent need of geriatricians was the discussion of care plan for stages 4/5 CKD without prior follow-up. The most frequent reason for referral to geriatricians by nephrologists was neurocognitive disorders, delirium and then shared decision about dialysis. Geriatricians with previous advanced training in nephrology were more frequently involved in shared decision-making process about dialysis. Training in nephrology was desired by 75% of geriatricians, in particular on metabolic, bone and cardiovascular complications of CKD (60%), non-stage 5 CKD (46%) and drug management in renal failure (38%). Among the earliest manifestations of these disorders, subjective cognitive decline (SCD) warrants particular attention, as it may represent an early indicator of future neurocognitive impairment. To address this issue, the CogniScan questionnaire was developed. This study aimed to examine its psychometric properties and to investigate factors associated with SCD, including anxiety and depressive symptoms, personality traits, and interoception. CogniScan, a questionnaire assessing five cognitive domains (memory, attention, executive functions, language, and spatiotemporal orientation), was administered to and standardized on 61 healthy participants (mean age = 63.68 ± 6.60 years; 44 women, 17 men). Questionnaires assessing potential risk factors (anxiety, depression, personality traits, and interoception), together with a neuropsychological assessment of objective cognitive performance (memory, attention, executive functions, and language), were administered to 83 participants (mean age = 64.00 ± 6.65 years; 60 women, 23 men). Psychometric analyses generally supported the validity of the instrument. Attention-related SCD was positively correlated with anxiety and depressive symptoms (p ≤ 0.05). Neuroticism was positively associated with SCD, whereas conscientiousness was negatively associated with SCD (p ≤ 0.05). Interoception was negatively correlated with SCD (p ≤ 0.05). In addition, SCD was negatively correlated with performance on several attention and executive function tasks (p ≤ 0.05). CogniScan is a brief and easy-to-administer instrument that provides an accurate assessment of SCD. Anxiety and depressive symptoms, lower interoceptive abilities, and higher levels of neuroticism appear to be associated with greater SCD. These preliminary findings require confirmation in larger studies to determine whether these factors precede or coexist with SCD and to assess their potential relevance for preventive strategies.
","PeriodicalId":520632,"journal":{"name":"Geriatrie et psychologie neuropsychiatrie du vieillissement","volume":"24 2","pages":"187-197"},"PeriodicalIF":0.3,"publicationDate":"2026-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148400421","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
