DBIs can also be known as idiopathic osteosclerosis, bone whorl, focal periapical osteopetrosis, bone scar and enostosis. We found a paucity of literature for management and reporting of this condition in children. For this reason, the authors describe sixteen cases of children and adolescents with dense bony islands and suggest a pathway for management.</p><p><strong>Case series: </strong>Cases presented to the RNENT and Eastman Dental Hospital or private practice, either as chance findings or for diagnosis and treatment planning of undiagnosed radiopaque areas. The individuals were aged between 10 and 17 years; 6 boys and 10 girls. All radiographic reports described DBIs. Diagnoses were confirmed by a Dental and Maxillofacial Radiology Consultant and advised no intervention. In some cases, monitoring was advised. Caution in orthodontic tooth movement was advised for five patients.</p><p><strong>Conclusion: </strong>DBIs are common findings that seldom require treatment; however, caution should be exercised when undertaking orthodontic movement in the area of a DBI due to a potential risk of root resorption. This study evaluated the mean EPT threshold values and appropriate electrode placement sites for primary molar teeth based on their physiological root resorption levels.</p><p><strong>Methods: </strong>This study was conducted with 100 primary second molars in 100 children ages 3-12 (mean age: 7.88 years). Teeth were divided into 2 groups according to physiological root resorption levels. EPT response thresholds were recorded at the mesiobuccal cusp, occlusal third, middle third and cervical third of the crown. Data were analyzed according to resorption level using one-way analysis of variance and Duncan tests, with the level of significance set at 0.05.</p><p><strong>Results: </strong>Mean threshold values at all placement sites were higher in the group with the higher level of physiological root resorption. Structured questionnaires identified socioeconomic condition, medical/dental history, behavior and dietary habits. Data were analyzed using Chi-square tests (p < 0.05).</p><p><strong>Results: </strong>Of the enamel congenital defects, DF was the most prevalent (6.1%, n = 40), followed by DMH (5.6%, n = 37). Of the acquired defects, attrition was the most prevalent (36.9%, n = 242), followed by erosion (2.4%, n = 16). The caries experience was similar between the affected children (29,5%; n = 94) and total sample (30.6%; n = 201). The etiological factors related to congenital defects were: not exclusively fed breast milk (p = 0.003) and jaundice at birth (p < 0.001); the association with acquired defects was found with vomiting frequent episodes (p = 0.037).</p><p><strong>Conclusion: </strong>The general prevalence of enamel defects in primary dentition in Araraquara was 48.6%. Caries experience was outlined by adapting ICDAS<sub>0-6</sub> criteria to the d/D component of the WHO dmf/DMF index configuration. Percentages (%) of caries experience-free children, of children with initial caries (ICDAS<sub>1-2</sub>), and the mean d<sub>1-2</sub>t/D<sub>1-2</sub>T, d<sub>3-6</sub>mft/D<sub>3-6</sub>MFT and d<sub>3-6</sub>mfs/D<sub>3-6</sub>MFS indices were calculated. The probability of presenting with d<sub>1-2</sub>t/D<sub>1-2</sub>T ≥ 1 was assessed by binary logistic regression analysis, whereas negative binomial regression models examined the effect of socio-demographic parameters on d<sub>3-6</sub>mfs/D<sub>3-6</sub>MFS indices (level of significance: p ≤ 0.05).</p><p><strong>Results: </strong>60.1%, 48.1%, and 34.7% of the 5, 12, and 15-year-old children, respectively, had no caries experience at the defect level (d<sub>3-6</sub>mft/D<sub>3-6</sub>MFT = 0). Initial lesions (ICDAS<sub>1-2</sub>) were detected in 17.7%, 19.3% and 17.4% of the 5, 12 and 15-year-olds, accordingly. Mean d<sub>1-2</sub>t/D<sub>1-2</sub>T was 0.93, 1.70, and 2.51, whereas mean d<sub>3-6</sub>mft/D<sub>3-6</sub>MFT was 1.48, 1.61, and 2.46 for the 5, 12, and 15-year-olds, respectively. Children with higher educated parents and 15-year-old urban residents exhibited significantly less caries experience at the defect level. Initial caries lesions presented a significantly higher probability of being detected in urban-residing 5- and 15-year-olds, while no consistent trend could be identified for parental education level. Forty-four primary molars with deep active caries, whose removal resulted in pulp exposures, were treated with direct pulp capping and were allocated randomly into two groups: Group A, DPC with 3Mix-MP; and group B, DPC with Calcium Hydroxide (CH). All teeth were restored with composite resin restorations. The exposures were classified according to their diameter, location and number of the filling surfaces. Clinical follow-ups were performed at intervals of 3 months for 1 year, and radiographical follow-ups were done after 6-12 months. Data were assessed using Chi-squared test (χ2) at a 0.05 level of significance and binary logistic regression was performed to determine the effect of the characteristics of the carious lesion on success rates.</p><p><strong>Results: </strong>The success rate of the technique was 54.5% in group A and 77.3% in group B, but the difference was not statistically significant. The pulp exposure diameter, its location and the number of the filling surfaces did not affect the success rate significantly.</p><p><strong>Conclusion: </strong>(a) Direct pulp capping in primary teeth achieved comparable success rates with no significant difference between 3Mix-MP and Calcium hydroxide. (b) The exposure location, its diameter and the extent of the restoration had no significant impact on the success rates of direct pulp capping in primary teeth.</p><p><strong>Trial registration: </strong>ISRCTN, ISRCTN13525994. {"title":"Caries arrest effectiveness of silver diamine fluoride compared to alternative restorative technique: randomized clinical trial.","authors":"H M Abdellatif,&nbsp;A M Ali,&nbsp;S I Baghdady,&nbsp;M A ElKateb","doi":"10.1007/s40368-020-00592-0","DOIUrl":"https://doi.org/10.1007/s40368-020-00592-0","url":null,"abstract":"<p><strong>Objective: </strong>This RCT study assessed and compared the effect of a biannual application of 38% silver diamine fluoride (SDF) with alternative restorative technique (ART) on arresting caries in primary dentition.</p><p><strong>Methods: </strong>The RCT was conducted over a period of 12 months and included 79 healthy children (237 primary teeth), aged 3-8 years, selected from the University Pediatric Dentistry outpatient clinics, with at least one primary asymptomatic tooth with active dentinal occlusal/labial lesions. Children were randomly assigned to SDF application or ART treatment. Visual and tactile examinations were used after 6 and 12 months to assess the activity of the carious lesions using the ICDAS II.</p><p><strong>Results: </strong>At 6- and 12-month-evaluation periods, no statistically significant differences in lesions arrest were found between SDF and ART groups (P = 1.000). Considering the frequency of arrested caries between the test groups at 6 and 12 months, no significant differences were observed between anterior and posterior lesions, as well as between the maxillary and mandibular lesions. When comparing the working time for the two treatments, the median time for those treated with SDF was 3.3 min, compared to 14.4 min for ART and was statistically significant (P < 0.0001).</p><p><strong>Conclusion: </strong>Both SDF and ART are, indeed, effective for arresting caries. However, the chair-time management required for SDF is significantly shorter compared to ART. The study used a narrative perspective and thematic analysis to analyze data.</p><p><strong>Results: </strong>The findings from children and their parents (N = 25) indicated DTGA is disconcerting from both views. Parental guilt and the desire of both parents and children to not have to go through the experience again fueled at least short-term compliance with brushing, flossing, and changes in dietary habits.</p><p><strong>Conclusion: </strong>The children participants provided valuable information to augment that gathered from parents. As the study revealed that the DTGA is psychologically and emotionally troubling for both children and their caregivers, it is imperative to explore ways to ease the GA experience. Specimens were divided into three solution groups (n = 20) and immersed in two iron syrups (Ferrum and Ferro Sanol B) and distilled water. Two subgroups, sealed (BisCover ( +)) and unsealed (BisCover (-)), were established for each group (n = 10). Color changes between baseline and measurements at 4, 8, 24, 48, and 72 h were calculated with CIEDE2000 (ΔE<sub>00</sub>) metrics. Data were analyzed with 4-factor mixed-design ANOVA.</p><p><strong>Results: </strong>The first null hypothesis of this study that the use of surface sealant would not mitigate the restorative materials' susceptibility to staining was rejected: significant differences were found between BisCover ( +) and BisCover (-) groups in ΔE00 values for all restorative materials tested in Ferro Sanol B (p < 0.001) and Ferrum (p = 0.002) solutions. The ΔE<sub>00</sub> values in the Ferro Sanol B/BisCover ( +) groups were significantly lower than in Ferrum/BisCover ( +) groups (p = 0.002), the second null hypothesis that different forms of iron syrups would not impact the staining resistance of restorative materials was rejected. ΔE<sub>00</sub> values were different for each restorative materials tested, the third null hypothesis that the type of restorative material would not affect staining resistance was rejected.</p><p><strong>Conclusions: </strong>The application of surface sealant significantly improved the color stability of restorative materials. The content of iron syrups was also an important factor affecting color change. This study aimed to investigate the prevalence of dental caries and attendance to dental care in Finnish heart-operated CHD patients born in 1997-1999.</p><p><strong>Methods: </strong>The cohort of children born in 1997-1999 was selected using a national register on all heart-operated children in Finland. Gender, general health problems, diagnosis, type of the heart defect (shunting, stenotic and complex defects), and number of operations were available and included in the analyses. Dental records from primary health care were collected from municipalities with their permission. The data comprised of the number of dental examinations and data on caries status (dt, DT, dmft, DMFT) at the age of 7 (grade 1), 11 (grade 5) and 15 (grade 8) years and at the most recent examination. The control group consisted of dental data on patients born in 1997-1999 provided by the City of Oulu, Finland (n = 3356).</p><p><strong>Results: </strong>Oral patient records of 215/570 children were obtained. The difference between the defect types was statistically significant both for DT (p = 0.046) and DMFT (p = 0.009) at the age of 15 (grade 8). The prevalence of caries did not differ between the study population and the controls. High present and past caries experiences were not associated with higher number of visits to oral health care, especially to oral hygienist, or with oral health promotion. A systematic review and meta-analysis.","authors":"Andressa da Silva Arduim,&nbsp;Débora Plotnik Gonçalves,&nbsp;Luciano Casagrande,&nbsp;Tathiane Larissa Lenzi","doi":"10.1007/s40368-021-00615-4","DOIUrl":"https://doi.org/10.1007/s40368-021-00615-4","url":null,"abstract":"<p><strong>Purpose: </strong>To evaluate the quality of root canal filling of primary teeth using lentulospiral in comparison with other instruments.</p><p><strong>Methods: </strong>Electronic databases (PubMed/MEDLINE, Scopus, TRIP, LILACS, and CENTRAL Cochrane) were searched up to Jan 2021. Clinical trials that compared the quality of root canal filling of endodontically treated primary teeth using lentulospiral with other instruments were included. Two reviewers independently selected the studies, extracted data, and assessed the risk of bias. Two outcomes were considered: inadequate root canal filling (under or overfilling) and presence of voids. Conventional meta-analyses were performed using a fixed-effects model. Statistical analyses were performed using RevMan5.3 at a significance level of 5%.</p><p><strong>Results: </strong>Of the 68 potentially relevant studies, eight were selected for full-text analysis, and three were included in the systematic review. The use of syringes resulted in a lower risk of presence of voids in the root canal filling compared to the use of lentulospiral (RR: 0.62 95% CI 0.45; 0.85). However, there was no difference between lentulospiral and bi-directional spiral (RR:1.17 95% CI: 0.90; 1.51). There was no significant difference between lentulospiral and syringes (RR: 1.37 95% CI 1.00; 1.87) considering the length of the root canal filling. The use of bi-directional spiral had a higher risk of inadequate root canal filling compared to the use of lentulospiral (RR: 1.75 95% CI: 1.12; 2.74). Two studies were at \"high\", and one study at \"unclear\" risk of bias in the key domains.</p><p><strong>Conclusions: </strong>There is insufficient scientific evidence showing the superiority of using lentulospiral for the root canal filling in endodontically treated primary teeth.
