This study analyzes 25 years of Eurotransplant data to evaluate trends, outcomes, and prognostic factors.

Methods: Retrospective analysis of all intestinal transplant candidates and recipients within Eurotransplant (2000-2024) was performed. Survival analysis used Kaplan-Meier curves and Cox regression models. Primary outcomes included transplantation rates, waitlist mortality, and post-transplant overall survival.

Results: Among 303 patients, 215 received transplants and 88 remained non-transplanted. Transplanted patients were younger (37.7 ± 16.0 vs 42.3 ± 16.3 years, p = 0.030). Isolated intestinal transplantation comprised 46.0 % of procedures, while combinations with other organs accounted for 54.0 %. Cumulative transplantation incidence reached 70 % at four years, with significantly higher rates for liver-inclusive procedures (95 % vs 85 %, p = 0.00041). Waitlist mortality reached 40 % at four years, with trends toward higher mortality in adults versus pediatric patients. Post-transplant overall survival showed better univariate outcomes for transplants without liver components (HR=0.61, p = 0.034), though this difference disappeared after multivariable adjustment. Transplant year was independently associated with worse survival (HR=1.062, p = 0.005), reflecting changing selection criteria as non-transplant management improved. Annual transplant volume declined from 17 procedures (2007-2010) to 4 in 2023, despite stable donor reporting.

Conclusions: Despite declining volumes, Eurotransplant ITx provides life-saving treatment with 70 % transplantation rates and outcomes comparable to international registries. During an annual follow-up, the patient reported difficulties with his central venous access. Computed tomography (CT) revealed pseudo-sclerotic lesions of the T1-T3 vertebrae, suggestive of metastatic disease. Further imaging showed severe stenosis of the superior vena cava related to chronic thrombosis of the central venous line. Endovascular dilatation of the stenosed vein led to complete resolution of the lesions. This phenomenon, known as \"vanishing bone metastases\" (VBM), results from contrast reflux into collateral venous pathways, and can mimic bone metastases. A 70 % small bowel resection was performed at 6 months of age (body weight, 5 kg), followed by an 85 % resection 4 months later. Untreated MMPs served as controls. Postoperative clinical conditions, weight, blood, and stool samples were monitored longitudinally. Nine months after the initial surgery, the animals were euthanized for organ collection and histological assessments.

Results: Both treated MMPs remained in good general condition and continued to gain weight postoperatively. Plasma citrulline levels decreased after the second surgery, reflecting a reduced intestinal epithelial mass. At 7 months postoperatively (13 months of age), fecal fat was graded as 2 + in both treated MMPs, whereas the 38-month-old control showed a grade of 0. Histologically, jejunal villi were shorter, and goblet cells were smaller and fewer in the treated MMPs. Colonic crypts were taller in the treated MMPs, with no signs of inflammation or fibrosis in any tissue. Histological examination of the liver revealed no changes in fatty acid levels or fibrosis.

Conclusions: The MMP SBS model demonstrated prolonged survival and maintained good health for nearly 9 months without PN, which is a significantly longer period than that reported in previous models. This model offers a practical and physiologically relevant platform for future translational and reverse translational studies on intestinal adaptation.
","PeriodicalId":520517,"journal":{"name":"Intestinal Failure (New York, N.Y.)","volume":"9 ","pages":"100344"},"PeriodicalIF":0.0,"publicationDate":"2025-12-19","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13365909/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148451305","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
