The primary outcome of interest in this study was cardiac surgery-associated AKI. By analyzing time-series nasopharyngeal temperature (Tnp) monitored intraoperatively with multiple categories, we obtained indicators reflecting the duration and depth of hypothermia. We used univariate and multivariate logistic regression analyses to identify perioperative factors associated with the outcome and construct a prediction model, which was evaluated in terms of discrimination and calibration.</p><p><strong>Results: </strong>Mild hypothermia (32-34 °C) was independently associated with a reduced risk of AKI compared to other temperature categories. The proportion of the duration with temperature at 32-34 °C during CPB (CPB_32-34 °C_DurProp) demonstrated the greatest predictive value for AKI compared to other temperature-related characteristics. We found age, preoperative creatinine, history of hypertension, intraoperative blood loss, intraoperative transfusion of allogeneic blood, and the use of left ventricular assist device (LVAD) were risk factors for the development of AKI. Its diagnosis is challenging under general anesthesia due to nonspecific signs and limited patient communication. However, few cases of perioperative MINOCA have been reported to date, and clinical awareness remains limited.</p><p><strong>Case presentation: </strong>A 56-year-old living-related kidney donor with no history of cardiovascular disease was scheduled to undergo nephrectomy under general anesthesia. The induction of anesthesia was uneventful. Electrocardiography (ECG) monitoring demonstratedF the presence of ST-segment elevation approximately 90 min after the start of the surgery. The heart rate (HR) was increased to 90-100 bpm, and despite the administration of a large dose of catecholamines, it remained difficult to maintain the systolic arterial blood pressure (ABP) over 90 mmHg. The patient was transferred to the intensive care unit (ICU) at the end of the surgery. Standard 12-lead ECG revealed aST-segment depression in v1-v4. Moreover, the cardiac troponin I (cTnI) and N-terminal pro-brain natriuretic peptide (NT-proBNP) levels had increased to critical values. Emergency coronary angiography revealed 40 % stenosis in the opening and proximal regions of the left anterior descending (LAD) artery; no stenosis was observed in the other branches of the coronary arteries. After ruling out perioperative anaphylaxis, hypovolemia, and pulmonary embolism, the patient was diagnosed with MINOCA intraoperatively. The patient was extubated 24 h postoperatively under stable hemodynamic conditions. The cTnI and NT-proBNP levels were also restored to the normal range nine days after the operation.</p><p><strong>Conclusions: </strong>Diagnosis of MINOCA indicates that an ischemic mechanism is responsible for the myocyte injury; however, its causes are complex and occult. In 2024, there is a growing focus on patient-centered approaches aimed at improving overall outcomes. This includes the adoption of Enhanced Recovery After Surgery (ERAS) protocols to promote faster recovery and enhance patient satisfaction, multimodal pain management strategies to minimize opioid use while improving patient comfort, and conservative blood transfusion practices to mitigate risk and enhance the quality of patient care. The expansion of anesthesia practice beyond traditional operating rooms offers greater flexibility and efficacy in managing patients during diagnostic and therapeutic interventions. Technological innovations are also transforming the field. The role of pulmonary artery catheterization in perioperative cardiopulmonary function monitoring and management for high-risk patients remains a subject of debate. Concurrently, continuous assessment of cardiac and pulmonary function, coupled with the development of clinically relevant cardiac biomarkers, more specific medications, and enhanced monitoring devices, contributes to better management and perioperative outcomes. Furthermore, machine learning (ML) algorithms, artificial intelligence (AI) and virtual reality (VR) are playing an increasingly crucial role in training and improving clinical decision-making in complex cases. These innovations have the potential to further revolutionize cardiac anesthesia in the coming years.</p>","PeriodicalId":519929,"journal":{"name":"Journal of anesthesia and translational medicine","volume":"4 2","pages":"33-41"},"PeriodicalIF":0.0,"publicationDate":"2025-04-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13507988/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148835928","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The patient underwent orotracheal intubation under total intravenous anesthesia and non-depolarizing muscle relaxants. After intubation, anesthetic maintenance was achieved through the administration of sevoflurane and intravenous agents. During the initial stage, there was a gradual increase in the patient's heart rate and end-tidal carbon dioxide pressure (PetCO2). Five hours after anesthesia, sevoflurane administration was ceased, resulting in effective control of the condition. However, a subsequent episode of elevated blood pressure led to the re-administration of sevoflurane, causing a sharp rise in heart rate, PetCO2 levels, and body temperature. The patient's oropharynx temperature peaked at 39.4 ℃, axillary temperature at 45.1 ℃, PetCO2 level at 69 mmHg, and heart rate at 115 bpm. Due to no dantrolene, we used physical cooling through rapid infusion of ice-cold saline, which initiated a downward trend in core temperature, PetCO2 levels, and heart rate within 1 h. Throughout the anesthesia process, no significant acidosis or hyperkalemia was observed. After completion of the surgery, the patient was admitted to the ICU with a stabilized oropharynx temperature of 37.8 ℃. Subsequently, the patient experienced febrile symptoms in the following days but was discharged safely on the third postoperative day. Genetic testing revealed homozygous mutations in the RYR1 gene, both in the patient and his son, confirming the occurrence of malignant hyperthermia during the operation.</p><p><strong>Conclusions: </strong>This case report details the diagnosis and management of a 48-year-old male patient with a homozygous RYR1 gene mutation who developed delayed malignant hyperthermia (MH) following sevoflurane anesthesia.
