This study aimed to develop a Japanese version of the Fresno test.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Methods</h3>\u0000 \u0000 <p>First, the original version of the Fresno test was translated into Japanese following the standard instrument translation procedure. Subsequently, family medicine residents and certified physicians took the Japanese version of the Fresno test to assess its validity and reliability.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Results</h3>\u0000 \u0000 <p>Translation of the Fresno test into Japanese was completed successfully. A total of 39 participants took the test to verify reliability and validity. Family medicine specialists scored higher than majors (129.7 ± 36.0 vs. 100.6 ± 45.1, <i>p</i> = 0.036), which indicated high construct validity. Intra-class correlation coefficient and Kappa value of all questions were above 0.7, which suggested sufficient reliability. Item difficulty and floor/ceiling effect also fulfilled pre-determined criteria.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Conclusions</h3>\u0000 \u0000 <p>This study successfully developed a Japanese version of the Fresno test. This study validated these scales and explored factors that could improve their predictive accuracy among Japanese home health care patients.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Methods</h3>\u0000 \u0000 <p>This was a single-center retrospective cohort study. The primary outcome of unplanned ED visits was used to assess the validity of the DIVERT scale, CARS, and EARLI. Additionally, we examined whether the addition of patient age and receipt of advance care planning as variables on these assessments could enhance their precision.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Results</h3>\u0000 \u0000 <p>Altogether, 40 (17.8%) had at least one ED visit during the 6 months study period. In these patients, the DIVERT scale, CARS, and EARLI of the patients with ≥1 ED visit was superior compared with no ED visit (both <i>p</i> &lt; 0.05). The area under the curve (AUC) of the DIVERT scale, CARS, and EARLI were 0.62, 0.59, and 0.60, respectively. Adding patient age and receipt of advance care planning improved the AUC in all three scales.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Conclusions</h3>\u0000 \u0000 <p>Our findings suggest that these assessment scales could be applicable to home health care patients in Japan. Older adults often experience multimorbidity, cognitive impairments, and physical frailties, increasing healthcare utilization and costs. Traditional medical approaches that focus on organ-specific diagnoses are insufficient for addressing these multifaceted needs. The Age-Friendly Health Systems (AFHS) framework, introduced in the U.S., offers a patient-centered, value-based approach to geriatric care, encompassing the \"4Ms\": Mentation, Mobility, Medication, and What Matters. These pillars prioritize cognitive health, physical function, appropriate medication use, and patient values. However, Japan has yet to implement this system widely. The integration of AFHS, with an additional focus on \"Multicomplexity\" (5Ms), aligns well with the core competencies of General and Family Medicine. This specialty is crucial in leading interdisciplinary efforts to enhance geriatric care, addressing fragmentation and variability in healthcare delivery. To successfully implement AFHS in Japan, general medicine physicians must be trained in managing complex conditions and coordinating care across specialties. This shift toward holistic, patientcentered care is essential for improving outcomes for older adults and reducing healthcare costs. A comprehensive search was conducted across multiple electronic databases to identify relevant studies published up to 27 February 2023. A total of 17 articles met the inclusion criteria. The global prevalence of anxiety symptoms among older adults was 28%. Subgroup analyses indicated the continent, and the assessment tools significantly affect the overall prevalence. The associated factors identified encompassed sociodemographic, health-related, and psychosocial domains, with the female gender, physical comorbidities, and low social support and social isolation being the most reported associated factors for anxiety in each domain. This is because generalist disciplines including primary care have their own body of knowledge and research areas.&lt;span&gt;&lt;sup&gt;1&lt;/sup&gt;&lt;/span&gt; In North America, for example, the North American Primary Care Research Group (NAPCRG), which specializes in research activities, holds annual meetings, presents high-quality interdisciplinary research, provides research education for primary care providers, and offers various forms of support.&lt;span&gt;&lt;sup&gt;2&lt;/sup&gt;&lt;/span&gt; Conversely, in Japan, although the strengthening of primary care has been attracting attention as an important issue in the health care system debate, there has been a paucity of research in this field from an international perspective.&lt;span&gt;&lt;sup&gt;3&lt;/sup&gt;&lt;/span&gt; This is due to a lack of research infrastructure, training of researchers, and networks for research collaboration.&lt;/p&gt;&lt;p&gt;Recognizing the need to strengthen research activities in this area in Japan, Primary Care Research (PCR) Connect was established in 2019, with reference to the work of the NAPCRG. It is a collaboration between the Japan Primary Care Association, the American College of Physicians Japan Chapter, and the Society for Clinical Epidemiology. The three societies are drawing on their respective strengths, including clinical expertise and research methodology, to raise the level of research in primary care (general practice, family medicine and general internal medicine) in Japan to international levels. The term “Connect” in PCR Connect is derived from the concept of “connecting” the three societies involved in primary care research and “connecting” researchers in this field.&lt;/p&gt;&lt;p&gt;The annual meeting, which is at the heart of PCR Connect's activities, aims to increase the quality and quantity of research in the field and to promote networking between primary care providers and researchers. This is achieved by providing a place for friendly competition and growth among primary care researchers. A total of five PCR Connect annual meetings have been held to date. The second and subsequent annual meetings are held online and can be attended from anywhere in the country. The annual meeting includes invited lectures by overseas researchers, a variety of research seminars including quantitative, qualitative, and mixed research, symposia providing the latest national and international findings on important research topics in primary care, “Research Protocol Evolution” where the research leadership provides feedback on young researchers' protocol. Table 1 shows the representative programs of the PCR Connect annual meeting.&lt;/p&gt;&lt;p&gt;The first annual meeting had 90 participants, but the number has grown over the years to more than 200, with a notable increase in the number of young people, including residents and students. However, the perspectives of medical trainees under the pre-2022 MCC revision were under-examined. Here, we investigated what these trainees felt they had learned about general medicine (GM)-related topics.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Methods</h3>\u0000 \u0000 <p>We performed a nationwide cross-sectional study using an anonymous online questionnaire, which was developed with reference to the 2022 revised MCC. The questionnaire consisted of 14 items. For all items, we asked, “Did you learn enough during medical school?” Respondents were asked to respond on a 5-point Likert scale (from 1 = strongly disagree to 5 = strongly agree).</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Results</h3>\u0000 \u0000 <p>Three hundred and eighty-six participants (response rate 55.4%) were included in the analysis. For the item “Behavioral science,” the number of participants who chose “3 = neither agree nor disagree” was highest, at 171 (44.3%) and with an average of 3.28, indicating that this item was perceived as insufficiently studied. Approximately half of the participants chose “4 = agree” for all items other than “Behavioral science.”</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Conclusions</h3>\u0000 \u0000 <p>The study suggested that behavioral science may be underlearned among medical trainees of the pre-2022 MCC generation. Medical educators in Japan should formulate curricula in accordance with the 2022 revision MCC and improve curricula regarding behavioral science. Anti-oppressive practice (AOP) provides a framework that challenges structural inequities and illuminates the lives of both patients and professionals as well as the historical and sociopolitical contexts in which power imbalances and oppression have developed.&lt;span&gt;&lt;sup&gt;1&lt;/sup&gt;&lt;/span&gt; Drawing on Freire's concept of critical consciousness,&lt;span&gt;&lt;sup&gt;2&lt;/sup&gt;&lt;/span&gt; AOP encourages professionals to reflect on their own authority and how these dynamics may unintentionally perpetuate oppression.&lt;span&gt;&lt;sup&gt;3&lt;/sup&gt;&lt;/span&gt; Expanding the understanding of AOP could significantly enhance health professions education,&lt;span&gt;&lt;sup&gt;1&lt;/sup&gt;&lt;/span&gt; as evidenced by the Interprofessional Education Collaborative's Core Competencies for Collaborative Practice.&lt;span&gt;&lt;sup&gt;4&lt;/sup&gt;&lt;/span&gt; Despite this potential, AOP remains underrepresented in healthcare research and education.&lt;span&gt;&lt;sup&gt;5&lt;/sup&gt;&lt;/span&gt; Given the critical role that primary care professionals play in advancing health equity and addressing the bio-psycho-social dimensions of diverse patient populations, we advocate for the integration of AOP into primary care practice.&lt;/p&gt;&lt;p&gt;At the 15th Japan Primary Care Association (JPCA) Annual Meeting in June 2024, the JPCA SDH Committee hosted a symposium on AOP, followed by a workshop with approximately 40 participants with different professional backgrounds at the 21st JPCA Fall Education Seminar in September 2024. The goal of the online workshop was to foster dialogue about integrating AOP into daily practice. In the workshop, we began by outlining the concept of oppression and the structures that perpetuate it. Our main messages are listed in Table 1. Participants then shared their personal encounters with oppression in clinical settings.&lt;/p&gt;&lt;p&gt;Subsequently, two cases were reported, including one from a marginalized region in Japan, where a staff member made a meaningful connection with a resistant patient by listening attentively to his/her narrative, moving beyond the conventional dynamic of charity-based care, or the structure of the staff giving charity and the patient receiving it. Another case examined health care for foreign residents.
