Methods: Descriptive observational study. Clinical and laboratory alterations were analyzed, as well as the classification of the related profile of a cohort of Latin American pregnant women with thrombotic events during gestation and up to the 120-day puerperium. Antiphospholipid syndrome antibodies, coagulation protein C, S, antithrombin III, mutations against factor V Leiden, mutations in methylenetetrahydrofolate reductase, hyperhomocysteinemia, prothrombin mutation, and elevation of factors VIII, IX, and XI were performed. Results: The mean age was 24,5 + 7,6 years, of which 9 patients (10,3%) had a history of thromboembolism, 23 patients (26,4%) had had at least one fetal loss. Elevated antiphosphatidylserine antibodies were found in 23 patients (26,4%), elevated antibodies against beta2-glycoprotein in 20 patients (22,9%), positive lupus anticoagulant in 16 patients (18,3%), elevated factor VIII in 13 patients (14,94%), elevated factor IX in 15 patients (17,2%), elevated factor XI in 12 patients (13,7%), prothrombin mutation in 7 patients (8,07%) and the others to a lesser extent. Conclusions: The results found here point to the high prevalence rate of underdiagnosed thrombophilic disorders in pregnant women. There is still a lack of strong evidence to analyze this relationship with worse results during pregnancy. Methods: This is an observational, descriptive, correlational study. The sample consisted of 90 women (90 fetuses) with a single pregnancy between 34 and 40 weeks of gestation. Results: From the doppler variables analyzed, it was evidenced that the fetal pulmonary artery pressure decreased significantly (p < 0.001) with advancing gestational age. It was determined that there is a significant inverse association between fetal pulmonary artery pressure and fetal maturity (p < 0.001), such that the lower the fetal pulmonary artery pressure value, the higher the percentage of fetal maturity. A fetal pulmonary artery pressure cut-off point located at 55.9 mm Hg jointly determined the highest sensitivity and specificity f or t he d iagnosis o f f etal l ung maturity, i ts s ensitivity being 93.75% and specificity being 80.77%. With a positive predictive value of 92.31% and a negative predictive value of 84%. Conclusion: The estimation of fetal pulmonary artery pressure obtained by doppler ultrasound is useful as an indicator of fetal maturity. Ardila, Mireya González Blanco","doi":"10.51288/00840105","DOIUrl":"https://doi.org/10.51288/00840105","url":null,"abstract":"Objective: To compare the evolution of healing in patients in whom amnion with antibiotic coverage was applied, with another group in which it was not applied, after cervical conization performed in cervical pathology consultations of the San Cristobal Central Hospital, between February and May 2023. Methods: Experimental, randomized, longitudinal comparison. It included 66 patients, randomly divided into two groups. Group 1: in which amnion was applied (n = 36/54.5 %); Group 2: no treatment (n = 30/45.5%). The researcher prepared the amnions of placentas obtained by cesarean section or vaginal delivery, in the operating room. Informed consent was obtained from both donors and recipients. Results: On day 14, none of the patients in the group 1 had epithelialization. In the group 2, one patient (2.7%) had 50% epithelialization and 35 (94.6%) had reached 100% (p < 0.001). Epithelialization was total in 13.3% of the first group and 94.6% of the second group (Odds Ratio of 227 (95% CI: 24.0 to 2157.0; p < 0.001). There were no cases of cervical infection. Five patients (16.7%) in the group 1 and one in the group 2 (2.8%) had ectopias (p = 0.051). There was no association between complete or partial epithelialization and clinical characteristics (p ˂ 0.05). Conclusion: Amnion with antibiotic coverage is associated with better healing outcomes in conized patients. Its effectiveness as a medical therapy was proven when placed on the surgical wound. And determine their relationship with maternal complications. Methods: Analytical, retrospective study. Full-term pregnant women, older than 18 years and with single live fetuses, with legible clinical history and prenatal care card with first and last check-up, treated at a public hospital in Peru between January - June 2022, were included. Patients with mental problems and those with fetuses with malformations, were excluded. The variables measured were: sociodemographic characteristics, obstetric, gestational weight gain, and maternal complications. Excess gestational weight was defined using pre-pregnancy body mass index, difference in weight between the first and last prenatal check-up, and Institute of Medicine guidelines. A descriptive analysis was made and to recognize the relationship between excessive gestational weight and maternal results, the X2 test was used. The National University of San Agustín Institutional Research Ethics Committee granted approval of the study. Results: We included 1021 pregnant women in the study. Of these, 49.0% had excessive gestational weight. 43.0% of overweight patients had maternal complications. A relationship was found between excessive weight gain in pregnancy and gestational diabetes. Conclusions: Patients with excessive weight gain in pregnancy were more likely to have gestational diabetes. We present the case of a 70-year-old patient who came to the clinic for recurrent and diffuse abdominal pain. On physical examination, a large mass was palpated that occupied the entire abdominal cavity, up to the xiphoid appendix, rounded, retentive, with defined edges, non- painful, non-movable. Presence of edema in lower limbs. During the surgical act, a myomatous uterus and a giant left ovarian cyst measuring approximately 40 x3 0 cm and weighing 13.6 kg were identified. Total abdominal hysterectomy and bilateral salpingo-oophorectomy were performed. The biopsy confirmed the diagnosis of benign ovarian mucinous cystadenoma and uterine leiomyomatosis. The postoperative period progressed favorably. The incidence of giant ovarian cystadenoma is relatively unknown due to the lack of a standardized concept and currently available advanced imaging modalities. Methods: Analytical cross-sectional study, done in 217 obstetrics students from the Universidad Nacional Mayor de San Marcos.The chi-squared test was applied, in addition to the raw and adjusted prevalence ratio (PR) Results: The prevalence of the use of dual protection was 41.5%. In the bivariate analysis, the factors associated with it included: age from 25 to 29 years old (raw prevalence ratio: 1.57), Lima as a place of origin (raw prevalence ratio: 2.07), previous pregnancy (raw prevalence ratio: 1.91), previous abortion (raw prevalence ratio: 1.92), antecedent of STI in the couple (raw prevalence ratio: 1.81), knowledge about dual protection (raw prevalence ratio: 3,16), and orientation in family planning (raw prevalence ratio: 1.76). In the multivariate analysis, knowledge about contraceptive protection maintained its significant association (adjusted prevalence ratio: 2.44). Conclusions: Knowledge about contraceptive protection is the only reproductive factor that was associated with a higher probability of using this method. Psychotic denial of pregnancy is typically accompanied by a history of mental illness. Negative effects include psychological distress, unsupported births outside of hospital, and a potential rise in mother and newborn morbi-mortality. It is crucial to include a personal and/or family history of psychopathology as part of the anamnesis because this condition should be treated by a psychiatrist. Regarding the management of this disorder during pregnancy or puerperium, there are no specific recommendations. To provide comprehensive care, a multidisciplinary approach is typically required. A case of psychotic denial of pregnancy is presented. There are two histological types: adult and juvenile. Juvenile granulosa cell tumors account for 5%, with only 3% occurring in people over 30 years of age. Given the suspicion, the definitive intraoperative diagnosis is complex given its rarity and its easy confusion with other ovarian neoplasms. The cystic pattern with immature granulosa cells, with frequent mitosis, the absence of Call-Exner bodies and the immunocystic study confirm this. Its low prevalence makes it difficult to diagnose. The stage of the disease is the most important prognostic factor, and complete resection is essential. The role of complementary therapy is not well established, and the available studies include only a minimal number of cases, which do not differentiate between adult women. Adequate follow-up for the early detection of a possible late recurrence is a clinical challenge. A case of failure in contraception with a copper intrauterine devices is presented after 5 years of use in a 26-year-old pregnant woman, who went to the emergency room with a pregnancy of 38 weeks and 4 days in the expulsive period; a healthy term newborn was obtained. The pathological (prolonged) delivery period due to the presence of copper intrauterine devices in ovular membranes.
