{"title":"[Personality functioning and physical health].","authors":"Carsten Spitzer, Philipp Göbel, Tobias Wilfer, Robert Mestel","doi":"10.13109/zptm.2022.68.3.238","DOIUrl":"https://doi.org/10.13109/zptm.2022.68.3.238","url":null,"abstract":"<p><p><b>Objectives:</b> Considering direct and indirect evidence of associations between deficits in personality functioning and high somatic morbidity, our exploratory study determined whether and, if so, which physical illnesses may be associated with impairments in personality functioning. <b>Method:</b> A large sample of diagnostically heterogeneous psychotherapy inpatients (N = 5401) was assessed with the short version of the Structural Questionnaire of the Operationalized Psychodynamic Diagnosis (OPD-SQS) and the Work Ability Index (WAI). In addition, discharge diagnoses were recorded. For a subsample (N = 2582), levels of personality functioning were assessed by their therapists according to the OPD system. <b>Results:</b> While consistent associations were found between personality functioning and neurological, urogenital, dermatological, and musculoskeletal disorders, findings on possible associations with other disorders revealed inconsistencies. The number of diseases as surrogate marker for disease burden was significantly associated with several indicators of personality functioning. <b>Conclusions:</b> Our results suggest associations between impairments in personality functioning and poor physical health. Further, it examined the question how publicly available information about eating disorders may contribute to psychoeducation. <b>Methods:</b> Thirteen relatives of patients with anorexia nervosa and bulimia nervosa were interviewed based on a semistructured interview guide. The interview transcripts were interpreted by using a structuring qualitative content analysis. <b>Results:</b> Relatives showed a high need for information throughout the entire course of illness. More information about the disorder itself and its potential treatment were requested. The needs were addressed to public services and to family doctors as well as paediatricians. <b>Conclusions:</b> Relatives as well as family doctors and paediatricians play an important role in the detection and support of the treatment of eating disorders. The description of support needs and specific topics brought to supportive conversations will be used to add to the body of knowledge about stressors and resources. <b>Methods:</b> A total of 109 telephone conversations with 69 quarantined Corona patients were documented by psychotherapists and physicians at Heidelberg University Hospital from November 2020 to April 2021. Subsequently, clinical documentations were analyzed according to a qualitative content analysis. <b>Results:</b> Most physical complaints related to cardio-respiratory symptoms (29 %), previous illnesses (24 %), and exhaustion or fatigue (16 %). On the psychological level, patients reported mainly anxiety (31 %) and depressive symptoms (16 %). On a social level, patients described stress related to family (56 %), work (20 %), and time in quarantine (16 %). Social support, individual coping strategies, a positive prognosis on the course of the corona disease, psychotherapy, and satisfactory medical care were mentioned as relieving factors. Therapeutic interventions aimed at stabilization and consisted of psychoeducation, relaxation techniques, and general counseling. <b>Conclusions:</b> The study shows that physical complaints, psychological symptoms, and social factors are brought into telephone support conversations. A previously neglected occupational group is medical technical assistants (MTA). The aim is therefore to identify stress factors among MTA in Germany during the pandemic. <b>Methods:</b> A cross-sectional online survey of medical staff was conducted in spring 2020 (N = 8088). <b>Results:</b> N = 1483 records of MTA were analyzed. Retrospectively, the stress increased under the pandemic, and 60.1 % of MTA suffered from work stress (ERI). Staff shortages and extra work were associated with an increase in work stress. Problems of work-life balance and contact with contaminated material/infected persons favored stressful experiences. <b>Conclusions:</b> Some working conditions in the pandemic pose a potential health risk to MTA. Stoller (1979), particularly his assumption of a transformation of aggressive impulses into sexual deviant fantasies due to traumatic experiences. <b>Methods:</b> In the present study different aspects of the theory were examined by using a sample (N = 954) of individuals convicted of sexually motivated offenses who had been clinically and forensically assessed between 2002 and 2018 at the Federal Evaluation Centre for Violent and Sexual Offenders (FECVSO) in the Austrian Prison System.
