So far, there is no empirical data on the association of gratification crisis and social support through the partner in physician's profession. <b>Objectives:</b> This study evaluates the effects of social support, in terms of distribution of house work and amount of working time within a partnership, on burnout and gratification crisis of residents. <b>Material and Methods:</b> Data acquisition was carried out within the multi-centric and prospective \"KarMed\" study in Germany at the end of the postgraduate training in 2016 (N = 433). The ERI and MBI scales were used. <b>Results:</b> Results yielded significant effects of distribution of house work and the amount of working time on gratification crisis and burnout. <b>Conclusions:</b> The satisfaction with the distribution of house work as well as working time in a partnership plays an important role for the wellbeing of residents. {"title":"[Training in psychotherapy for medical doctors - implementation in practice].","authors":"Kurt Fritzsche,&nbsp;Claas Lahmann,&nbsp;Anna Bächle,&nbsp;Rainer Leonhart","doi":"10.13109/zptm.2022.68.oa2","DOIUrl":"https://doi.org/10.13109/zptm.2022.68.oa2","url":null,"abstract":"<p><p><i>Mental Health Screening of Syrian Refugees in Germany: The Refugee Health Screener</i> <b>Objectives:</b> The purpose of the present study was to examine (1) the impact of training in psychotherapy on professional activities and personal life and (2) the frequency of psychotherapy services in primary care and specialty practices. <b>Method:</b> General practitioners and specialists in outpatient medical practices from Baden-Württemberg (n = 289) with completed additional training in psychotherapy were asked to fill out a questionnaire about the current state of implementation in their medical practice. Data were analyzed using descriptive and correlative statistical methods. <b>Results:</b> The response rate was 53.7 % (n = 150). Physicians reported very positive effects on professional activity, job and life satisfaction, and personal growth. They provide an average of 9.1 hours (SD 6.4) of psychotherapy per week. They provide low-frequency psychotherapy for longer periods per quarter to 21.3 (SD 55.6) patients with primarily chronic physical illnesses. <b>Conclusions:</b> Additional training in psychotherapy for physicians seems to be proving successful in its implementation in medical practice. Most physicians regularly provide psychotherapy on a small but significant scale. The aim of this study is to determine specific characteristics of these innovative office-based practices. <b>Methods:</b> In a qualitative cross-sectional study, twelve board-certified specialists in psychosomatic medicine, practicing office-based psychosomatic medicine were surveyed by means of semi-structured telephone-interviews. After transcription and coding in MAXQDA 2020, a content analysis was conducted. <b>Results:</b> The innovative practice model was characterized by high numbers of initial contacts in walk-in or pre-planned consultation hours. Besides regular psychotherapy long-term and low-threshold treatment was offered. Physician assistants organised the workflow and administrative tasks. For settlement the EBM-number psychosomatic interview or short-term treatment was used. Focusses were set on somatopsychic and socio-medical treatment and consultant support. Psychotherapy often was realised in form of group sessions by integrating behavioural and psychodynamic psychotherapy. <b>Discussion:</b> The innovative psychosomatic practice model with focus on acute care shows an independent profile. A treatment based on medical understanding of complex somatic disease processes can be provided to somatopsychic patients. So far, however, it is largely unclear which processes underlie the treatment successes. In the partial evaluations of the Hersfeld catamnesis study presented here, recourse is made to the construct of mentalizing ability, which has become very important in recent psychotherapy research. <b>Methods:</b> The mentalization ability of a large sample (N = 559) was assessed with the help of the Mentalization Questionnaire (MZQ), and psychological and somatic complaints were assessed with HEALTH-PSB scale from HEALTH-49 at three points in time, namely at admission, at discharge and in a 6-month catamnesis. <b>Results:</b> Both the total score of the MZQ and all subscores show significant reductions in mentalization deficits in the small effect size range, the HEALTH-PSB in the high effect size range and the catamnesis in the medium effect size range. Despite this widespread practice, the actual impact and potential outcome have been studied relatively scarcely and are still open to debate. The present study therefore examines whether the therapeutic relationships in the group vary depending on the group composition and whether this has an effect on therapy outcome. <b>Methods:</b> 66 patients (N = 55 women) were treated in our multimodal, day clinic groupbased treatment program and filled out symptom questionnaires (ETI, BDI-II, PHQ-15, Questionnaire on social support) at admission and discharge and the Group Questionnaire (GQ; including subscales positive bonding, positive working and negative relationships) at the end of each of the eight weeks of treatment. Patients took part in five women-only and six mixed-gender groups and underwent therapy in a closed group format, so that they began and ended therapy within the same group. All patients were diagnosed with trauma-related disorders, which included PTSD, cPTSD, depression, somatoform disorders and further mental diseases. Outcome data were examined using functional regression analysis and linear mixed models. <b>Results:</b> In women-only groups, positive bonding and positive working relationship were significantly higher than in mixed-gender groups. Negative relationship ratings were significantly lower in women-only groups, especially in the middle phase of treatment. However, no effect of an exclusively female group composition on treatment outcome was found, except for the higher increase of perceived social support for patients treated in women-only groups. <b>Conclusions:</b> Group relationships were considered more pleasant in women-only groups than in mixed-gender groups, but this had no effect on the symptom-based therapy outcome. It refers to whether inspected treatments differ from one another. In the history of psychoanalysis, the differentiation between psychodynamic and analytic psychotherapy was often discussed but seldom inspected empirically. In this study, we propose a set of items which should in theory offer the possibility to differentiate between psychodynamic and analytic psychotherapy on session level. We inspect these items using therapists' self-reports concerning N = 295 cases of different psychodynamic and analytic therapies. Results of an exploratory factor analysis and subsequent inspections of the psychodynamic and analytic items in different forms of therapies strengthen the usability of these items for differentiating between psychodynamic and analytic therapy. However, further studies using different perspectives (e. g., observer ratings) are essential.
