Management for diabetic ketoacidosis is well documented, which involves the administration of short-acting insulin, fluids, and electrolyte correction, with some studies describing a probable relationship between diabetic ketoacidosis and coronavirus disease 2019, resulting in extreme hyperglycemia.</div></div><div><h3>Patient Presentation</h3><div>This case review details the emergency nursing management of a 34-year-old female who presented in metabolic extremis from probable diabetic ketoacidosis. Serum blood analysis results revealed a blood glucose of 2394 mg/dL, a ketone level of 45.32 mg/dL, a potassium level of 6.1 mmol/L, unmeasurable hypothermia, coronavirus disease 2019 positivity, and progressive torsades de pointes. Management strategies focused on airway management, suitable tonicity and osmolarity correction, and rectification of electrolyte derangements.</div></div><div><h3>Conclusion</h3><div>The patient was transferred to a tertiary care hospital and discharged home with no physiological deficits. Measuring emergency nurses' subjective well-being is necessary to identify their perceived well-being and areas for improvement. However, none of the existing instruments are specifically designed to measure emergency nurses' subjective well-being, making measurement of it less representative and accurate than with an appropriate instrument. Given that the emergency department has unique characteristics different from other units, the development of the Emergency Nurses' Subjective Well-Being at Workplace Instrument is needed. This protocol outlines the stages of development and testing to obtain a valid and reliable instrument.</p><p><strong>Methods: </strong>The exploratory sequential mixed-methods design will be used in instrument development, which consists of 3 stages. The first stage will involve qualitative research exploring emergency nurses' experiences and perspectives on workplace well-being and a literature review to identify relevant concepts and theoretical frameworks. The second stage will focus on generating the instrument's items, and the final stage will involve psychometric testing to assess the instrument's content validity, face validity, construct validity, internal consistency, and test-retest reliability. Thus, the current research examined the association between moral injury and the quality of work life of the nurses.</div></div><div><h3>Methods</h3><div>In this descriptive-correlational study done in 2022, 168 nurses working in the emergency department of the referral hospitals of Mazandaran University of Medical Sciences participated via census sampling. The demographic questionnaire, moral injury questionnaire (Moral Injury Symptom Scale-Healthcare Professionals version), and Brooks’ Quality of Nursing Work Life scale were used to collect the data, which were analyzed by SPSS 25 and used descriptive and inferential statistics<span> with Spearman’s rank correlation coefficient and Mann-Whitney and Kruskal-Wallis tests.</span></div></div><div><h3>Results</h3><div>The mean moral injury and the mean quality of nursing work life resulted as 9.58 ± 46.69 and 129.92 ± 15.76, respectively. Moral injury displayed a significant and indirect relationship with the quality of nursing work life (r = −0.433, P&lt;0.05). In addition, moral injury was meaningfully associated with age, work record, work shift, job rank, and the number of patients (<em>P</em> = .05). The quality of work life was also significantly associated with gender, education, and marital status (<em>P</em>&lt;.05).</div></div><div><h3>Discussion</h3><div>In the current study, a significant relationship was discovered between moral injury and the quality of work life of nurses during the coronavirus disease 2019 pandemic. Bladder rupture resulting from the insertion of a urinary catheter is exceedingly uncommon. Here, we report a case of bladder rupture after urinary catheter insertion in a patient with bladder cancer.</p><p><strong>Case presentation: </strong>An 80-year-old woman presented to the emergency department with fever, flank pain, and difficulty urinating. Her medical history included bladder cancer and neurogenic bladder. A urinary catheter had been placed owing to recent worsening of bladder function and had been maintained for 10 days. It was removed at an outpatient urology clinic the day before her emergency department visit to prevent a urinary tract infection. However, due to urinary retention, the urinary catheter was reinserted in the emergency department. After insertion, she complained of acute abdominal pain. Computed tomography revealed an intraperitoneal bladder rupture, with the urinary catheter positioned extraluminally through the bladder dome. The catheter was repositioned under ultrasound guidance, and conservative management with broad-spectrum antibiotics was initiated. This project aimed to enhance emergency staff knowledge and reduce the door-to-consult times for patients with left ventricular assist devices presenting to the emergency department by implementing a bundled care approach.</p><p><strong>Methods: </strong>To better support patients with left ventricular assist devices, the project team collaborated with emergency clinical staff to implement a comprehensive care bundle. This included a clear pathway for patients with left ventricular assist devices upon arrival to the emergency department, targeted education for clinical staff (with pre- and post-education survey), and a transportation checklist for admission cases. Data were collected and analyzed using SAS Statistics to summarize the results and improve patient outcomes.</p><p><strong>Results: </strong>Patients with left ventricular assist devices experienced significant improvements in care after the implementation of this care bundle. After education, patients benefited from increased staff confidence, better identification of alarms, and improved access to educational resources. Interventions initiated in emergency departments have the potential to prevent suicidal behavior. However, the scope and nature of available interventions remain unclear.</p><p><strong>Methods: </strong>A scoping review was conducted to explore the extent and nature of the literature on suicide prevention interventions in emergency departments. A search strategy based on the population, concept, and context framework was applied to 6 databases.</p><p><strong>Results: </strong>A total of 10,415 studies were retrieved, and 16 met eligibility criteria after full-text screening. Interventions were categorized into organizational interventions, safety planning interventions, psychological interventions, and psychotherapeutic interventions. Organizational interventions (n = 6) and safety planning interventions (n = 5) were most commonly reported, followed by psychological (n = 3) and psychotherapeutic interventions (n = 2). Organizational and safety planning interventions demonstrated positive outcomes, including enhanced follow-up, care coordination, and reduced readmissions for suicidal behaviors. Psychological and psychotherapeutic interventions showed mixed results.</p><p><strong>Discussion: </strong>This review highlights the range of emergency department-based suicide prevention interventions and their potential to reduce suicidal behaviors and improve patient outcomes. Organizational coordination and safety planning interventions emerged as feasible and effective strategies. Educational efforts to enhance emergency department staff knowledge and attitudes may help to facilitate the implementation of the identified interventions. Traditional emergency frameworks, primarily developed within Western medical contexts, often require adaptation to accommodate populations whose cultural beliefs shape their engagement with triage systems, medical interventions, and health care communication during critical situations. This paper examines how emergency nurses can uphold cultural sensitivity while delivering time-critical care, illustrated through 3 real-world case scenarios. We explore practical strategies for rapid cultural assessment, effective communication systems, and adaptive care practices that can be implemented in high-stress situations. This study aimed to examine the effects of professional quality of life constructs on caring ability in emergency nurses.</p><p><strong>Methods: </strong>A sample of emergency nurses (n = 238) in the United States participated. Using a convergent mixed method approach with quantitative priority, bivariate associations were explored, and a model of caring ability was obtained via multiple regression analysis incorporating the independent variables to answer the research questions. A final regression model of caring ability was obtained using the independent variables in conjunction with adjustment for significant demographic characteristics. Using a qualitative descriptive design, the qualitative data were subjected to content analysis. Themes were derived and qualitative data was quantitized for a better understanding of the phenomena under study.</p><p><strong>Results: </strong>Most participants reported low levels of compassion satisfaction, secondary traumatic stress, burnout, and caring ability. A multiple regression analysis identified compassion satisfaction as the strongest predictor for caring ability in emergency nurses. Content analysis revealed 3 themes: (1) I Love My Job But…; (2) Quantity Versus Quality: I Want What is Best, But I Cannot Provide it; and (3) The Abuse: Make it Stop. The results from the qualitative strand in this study confirmed the quantitative findings. Given that compassion satisfaction had the highest predictive value for caring ability, this variable was quantitized from the qualitative strand with 30% of participants endorsing feeling compassion satisfaction in their qualitative information.</p><p><strong>Discussion: </strong>This novel study concludes that promoting the nurse's purpose in their work, meaning and joy in their work, and feelings of pleasure in their work has the potential to augment their caring ability resulting in a greater likelihood of quality outcomes for patients in the emergency department.</p>","PeriodicalId":51082,"journal":{"name":"Journal of Emergency Nursing","volume":" ","pages":""},"PeriodicalIF":1.8,"publicationDate":"2025-06-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144210181","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
