The objectives of the study are to study the risk indicators of terrorism for the population of a number of countries and the world population as a whole, as well as their dynamics in 2011–2020 in order to determine the average annual risk of being exposed to a terrorist attack, dying or being harmed to life and health. Materials and methods of research. Research materials – indicators of the open Global Terrorism Database (GTD), which was created, continues to be created and improved by employees of the National Consortium for the Study of Terrorism and Responses to Terrorism (START) of the University of Maryland (University of Maryland) – USA. Research results and their analysis: – according to GTD, on average over 10 years (2011–2020) 110 thousand TA were indexed in the world, in which 258.4 thousand people died, including 70 thousand terrorists (27.1%), and 284.2 thousand people were injured (injured), including 18 thousand terrorists (6.7%); – for the world population, the average annual individual risk was: the risk of being exposed to a terrorist attack – 1.49´10–6 or 1.41 [1.11; 1.89]´10–6 TA/(person ´ year); risk of death – 2.55 ´ 10–6 or 2.29 [1.94; 3.16] ´ 10–6 deaths/(person year); the risk of being hit (injured) – 3.63 ´ 10–6 or 3.32 [2.21; 5.18] ´ 10–6 injuries/(person ´ year); – all of the increased risks studied occurred in Afghanistan. In addition, there was an increased risk of being exposed to a terrorist attack in Ukraine. The purpose of the study is to provide a clinical description of injuries inflicted with the use of firearms of limited destruction (LDL) to residents of the Arctic zone of the Arkhangelsk region (Arctic zone). Materials and methods of research. A retrospective study was carried out using the type of documentary observation on the general population of victims of the use of OOOP in the Arctic zone in 2006–2022. Primary data collection was carried out by copying information from medical records. Research materials – 125 medical records of patients who received medical care in a day hospital (f. 003/u), issued for victims of POOP, as well as 123 Forensic Medical Examination Protocols compiled at the State Budgetary Institution of the Arkhangelsk Region “Bureau of Forensic Medical Examination”. The following variables were used for statistical analysis: gender, age, date of injury, circumstances of injury, presence of alcohol or drug intoxication, date of seeking medical help, method of delivery to a medical organization (HMO), severity of condition at the time of admission, location of injury, depth of the wound in relation to the body cavities, assessment of the severity of the wound on the VPH-P (OR) scale. Quantitative variables are presented as medians, qualitative variables as percentages. Research results and their analysis. Analysis of the research results showed: – injuries from OOOP are an urgent medical and social problem in the Arctic zone of the Arkhangelsk region. The purpose of the study is to use the example of Moscow to estimate the magnitude of health losses among the population in emergency situations (ES) that arise in a modern metropolis. Materials and methods of research. Research materials – regulatory and methodological documents defining the procedure for organizing and functioning of the Moscow Territorial Scientific and Practical Center for Disaster Medicine of the Moscow Department of Health (CEMP), as well as the procedure for providing medical care to emergency victims and their medical evacuation; reports from the emergency medical service (EMS) and the Center for Emergency Medicine on their activities to eliminate the health consequences of emergencies in 2018–2022; scientific works and publications devoted to current issues of medical evacuation of Moscow residents affected by emergencies. Research methods – statistical and analytical methods, direct observation method, logical and information modeling method. Research results and their analysis. Analysis of the research results showed: – in 2018–2022 in Moscow, for each emergency there were an average of 13.9 victims (in the Russian Federation as a whole – 22.9 victims); the risk of death among emergency victims in the metropolis averaged 20.9% (in the Russian Federation as a whole – 43.0%), which was due to competent routing, i.e. timely hospitalization of victims to medical organizations (HMOs), which provide comprehensive medical care; – medical and sanitary losses in emergencies in the metropolis are generally relatively stable. More pronounced differences are observed only in man-made emergencies; – the so-called “Covid” restrictions introduced in 2020 contributed to a decrease in the number of emergencies in general, but did not lead to a sharp decrease in the proportion of victims in emergencies and may have contributed to a two- to threefold increase in the number of victims in subsequent years – especially in social emergencies character; – in Moscow, in 35% of emergency situations, medical assistance was provided to victims at the emergency site; in 65% of cases, emergency victims were hospitalized; – approaches to medical evacuation and treatment outcomes for victims in emergency situations depend on the strength and duration of the impact of the damaging factors of the emergency, timely provision of first aid and medical aid, and the provision of the latter is inextricably linked with the speed and time of delivery of the patient to a specialized hospital, which has all the capabilities for provision of specialized, including high-tech, medical care. Materials and methods of research. The research materials are data from the annual reports of the operational department of the joint EMS and QMS service, as well as quarterly and annual reports of medical organizations (HMOs) in the region. The subject of the study is the routing of patients with suspected acute coronary syndrome (ACS) and acute cerebrovascular accident (ACVA) before and after the creation of a joint EMS and QMS service – both during work in everyday activities (2019) and during the new coronavirus pandemic Covid-19 infections (2020–2021), as well as the functioning of the joint EMS and QMS service in the context of a Special Military Operation (SVO). The research method is the method of four-field tables using the χ2 criterion. Research results and their analysis. In recent years, a protracted emergency of a biological and social nature has occurred on the territory of the Belgorod region, which required the consolidation of forces and means of the EMS and QMS services. The purpose of the study is to analyze the need to train medical workers with secondary medical education in order to ensure their readiness to provide medical care in conditions of mass admission of victims in emergency situations (emergency situations) to medical treatment organizations (HMOs). Materials and methods of research. The research materials are the results of a survey of nurses at a large multidisciplinary medical facility in the city of Orenburg regarding their subjective assessment of their own level of skills and the need for further improvement of labor functions performed when providing medical care to emergency victims in the emergency department of the medical facility. The survey was conducted anonymously on the Telegram platform. The survey participants included 72 medical workers with secondary medical education, of which 94.5% were women; 5.5% are men. Distribution of participants by professional experience: experience up to 5 years – 13.9%; from 5 to 10 years – 19.5; experience more than 10 years – 66.6%. Research methods. The obtained data were analyzed in the form Me [Q25;Q75], the calculations of which were carried out in Excel spreadsheets using the function “=QUARTILE.INC”. Statistical analysis of the obtained data was carried out by calculating relative values – intensive and extensive indicators. The level of statistical significance of differences between the study groups was determined using the Pearson χ2 test. Differences were considered statistically significant at p≤0.05. All calculations of statistical indicators were carried out in the Statistica 10.0 program. Research results and their analysis. The results of a survey of nurses at a large multidisciplinary medical facility showed the need for their additional training on the provision of medical care during the mass arrival of victims in emergencies. When developing training programs for nursing personnel, they should include training in triage skills and preparation for medical evacuation. The purposes of the study are to improve the quality of life of patients with congenital atresia or amputation of the external ear. Materials and research methods. In the experiment, two closely related individuals of minipig (domestic dwarf pigs) of the Sus salvanius breed were used as a living biological model. Two minipigs were surgically treated in the operating room under sterile conditions using standard surgical instruments. The surgical phase of the experiment consisted of three stages. All surgical stages were performed under multicomponent total intravenous (IV) anesthesia in the presence of an anesthesiologist. The operations were performed by two surgical teams of 2 surgeons each. The following steps were completed within 120 days: collection of ear cartilage from individuals for isolation and cultivation of chondrocytes; implantation and prefabrication of bioengineered endoprostheses into the muscle sheaths of the parietal region; collection of grafts for histological analysis. All three surgical steps on both minipigs were identical and were performed synchronously. Research results and their analysis. The objectives of the study are to perform a bibliometric analysis of scientific works of Russian authors on the organization of provision, delivery and training in first aid (FP), published in Russian over the past 20 years. Material and research methods. In September 2023, a search was made in the electronic library eLIBRARY.RU for publications of the type “journal articles” under the heading 76.00.00 “Medicine and Healthcare” containing the term “first aid” in the title, abstract or keywords (search query: ” first* help*”) and published in January 2003 – August 2023. In the process of screening the search results, Russian-language publications were selected that touched on the topic of PP, the first authors of which were affiliated with Russian institutions. The following were excluded: duplicates; publications not related to first aid; historical and journalistic materials; publications on veterinary medicine; works without specifying the affiliation of the authors; normative legal acts; articles in all languages except Russian. To save bibliographic records and create a catalog of publications, the bibliographic information management program Zotero (Corporation for Digital Scholarship, USA; version 6.0.26) was used. Research results and their analysis. The search identified 1459 bibliographic records. During the screening, 608 publications were excluded. Thus, the sample for analysis included 851 publications. During the study, a clear trend towards an increase in the annual number of publications on PP was revealed – linear regression: R2=0.898, p<0.001. A statistically significant increase in the annual number of publications by Russian authors in Russian devoted to first aid issues was noted. In the last five years, this figure ranged from 66 to 105 publications per year, compared with one to 26 publications per year during the first ten years of the period studied (2003–2012). The purpose of the study is to study and systematize the process of improvement and development in the USSR and the Russian Federation of one-year specialization (internship) of graduates of medical universities in the system of primary postgraduate education; identify problematic issues in their internship training. Materials and methods of research. Research materials – regulatory legal acts of the USSR and the Russian Federation; open scientific thematic sources characterizing the process of formation and development of internship in a historical aspect. Research methods – methods of comparative analysis and scientific generalization. Research results and their analysis. It was established that internship was the most successful form of practical training for medical specialists, and its completion was the main requirement for their employment. In the course of improving and reforming the system of higher medical education, internships were transferred from medical organizations (LMOs) to clinical departments of medical universities. The abolition of internship in 2017 had a negative impact on the quality of training of medical personnel, including for the Disaster Medicine Service (QMS) of the Russian Ministry of Health.
