{"title":"[Characteristics of Guillain-Barré syndrome in the healthy area III of Aragon Country].","authors":"G Piñol-Ripoll, P Larrodé Pellicer, M Garcés-Redondo, I de la Puerta González-Miró, C Iñiguez Martínez","doi":"10.4321/s0212-71992008000300002","DOIUrl":"https://doi.org/10.4321/s0212-71992008000300002","url":null,"abstract":"Introduction: Guillain-Barré Syndrome is an acute immune-mediated inflammatory polyneuropathy characterized by flaccid paresia with arreflexia, changeable sensitive disorder and albuminocytologic dissociation in the cerebrospinal fluid (CSF).Patients and methods: We conducted a retrospective study of 30 GBS patients diagnosed in Hospital Clínico de Zaragoza between 1999 and 2005. Annual incidence, seasonal distribution, preceding acute infection; clinical, electrophysiological and electrocardiographic (ECG) data and evolution were all evaluated.Results: The incidence was 1.56/100000 inhabitants/year. We observed an upward tendency during winter months. The rates of incidence were higher in men (66.7%) and increased with age in both sexes. Main clinical presentation form was paraparesia of lower limbs followed on cranial nerve palsy. Immunoglobulin therapy was received by 62.5% of patients. The rate of death was 10%. Thirteen per cent of patients had dysautonomia. Electrocardiographic abnormalities were present in 37% of patients.Conclusions: GBS incidence in Aragon Country is similar to that found in other studies. An increase with age and an upward tendency during the winter months was observed. Mean age: 85.8 y (48% men, 52% women). 19% FINE-3, 49% FINE-4, 32% FINE-5. Known etiology: 25% (Pneumococcal 19%, H. Influenzae and other Gram (-) 6%. 75% treated by Internists, 22% treated by Pneumologists. Standard Guidelines followed up by 30,5% a variant 60% (Equal by Internists or Pneumologists). Time door-1st antibiotic dose 6.6 hours. Global Mortality 16.7%. Women died at 87.4 y, men at 84.5 y (p = 0.035). Mortality FINE 3-4-5: 4.5, 12.4, 30% respectively. Mortality treated before 4 hours: 34.6%, after 4 hours: 11.5% (p = 0.01). Many more FINE 5 cases in Int. Medicine than Pneumology. Mortality by Internists 22%. Mortality by Pneumologists 3% (p = 0.001).
