Its early detection and supporting measures improve prognosis of these patients.</p><p><strong>Aims: </strong>To know the incidence of SIRS among patients who come to ER, their frequency and distribution factors and the clinical evolution at 3 and 30 days.</p><p><strong>Patients and methods: </strong>Observational prospective simple-blind study. During 24 hours, SIRS was detected by observant doctors with an independent registry. Their management was observed. Patients from Obstetrics and Traumatology were not observed. Follow-up was done using telephonic and informatical techniques at 3 and 30 day. A descriptive analysis was done.</p><p><strong>Results: </strong>There were 163 patients attended in ER; 25 of them with SIRS (15.3%), 16 were male (65%) and 9 female (35%). By ages 8 were under 30, 4 were between 30 and 60 and 13 were over. The respiratory rate was not measured in 12 of the 25 patients with SIRS (48%). An infectious etiology (sepsis) was found in 19 of those 25 (76%) patients. The most frequent criterion of sepsis was tachycardia, followed by leukocyte disorders. Support of volume and antimicrobial therapy were only started at once in 21 and 42% respectively on patients with sepsis. 15 of 25 were admitted (2 in ICU). After 3 days, 11 of 25 remained at hospital and after 30 days 2.</p><p><strong>Conclusions: </strong>SIRS is a prevalent situation in ER with a high percentage of admissions. Most of SIRS were of infectious origin (sepsis). To determine the pattern of use of the spirometry (SPIR) in comparison with the use of the echocardiography (ECHO) in heart failure (HF).</p><p><strong>Methods: </strong>We included those patients that during one year they were discharge with the primary diagnosis of \"COPD exacerbation\" or \"Heart Failure\" (codes: ICM-9-CM: 491.21 y 428). From the generated list, history charts were reviewed and original SPIR and ECHO were pooled out from the pulmonary function test and non-invasive cardiac unit departments.</p><p><strong>Results: </strong>Among 408 patients with COPD, SPIR was done in 110 (27%) and among 328 patients with HF, ECHO was prescribed in 250 (76%) (p < 0,001). COPD group was younger (71.8 +/- 9.8 vs. 74.9 +/- 10.6, p < 0,001), but no other differences were found with HF group. Among those patients who had SPIR, age was lower (69.5 +/- 9.1 vs. 72.5 +/- 9.9, p=0,005) and there was a high % of in-patients carried by pulmonology versus internal medicine (30 vs. 13%, p < 0,002) comparing with the group of patients who had no SPIR.</p><p><strong>Conclusions: </strong>We found a small percentage of COPD patients who had spirometry during hospitalization due to COPD exacerbation. It can be functional and non-functional. The studies of image don't clarify to us if procedence becomes of parathyroid or it has another origin, and the diagnosis is made with the analysis come from the liquid obtained in the puncture aspiration, where high PTH's concentrations are seen. Surgery is the treatment for functional cysts and the puncture aspiration for non- functional, the fact that they use to get solved with only one but sometimes is necessary to repeated it, even more than one time, keeping for the surgery when recurrent. We presented a clinical case of recurrent parathyroid cyst and his natural evolution, due to patient rejects surgery, being necessary repeated punctures.
