Early diagnosis and treatment may avoid acute heart failure fatalities. Sympathetic crashing reduces afterload, shifting fluid into the pulmonary circulation and causing pulmonary edema. The usual strategy of progressively increasing vasodilator dosage has increased preload. Treating SCAPE with large doses of nitroglycerin and non-invasive breathing is helpful. High doses of nitroglycerin boost afterload and eliminate ICU admissions. In this study, a standard protocol for the treatment of patients with sympathetic crashing acute pulmonary edema is assessed for its effectiveness. Methods: An observational study was conducted prospectively at Bhima Bhoi Medical College and Hospital in Balangir, Odisha, India over the course of a year. The patients presenting with SCAPE symptoms were treated according to standard protocol. The outcomes were subjected to statistical analysis to derive a correlation between treatment and response. Results: The mean initial bolus given to all the patients was 836 µg. The total cumulative dose of nitroglycerin used in each patient was 36 mg. The symptoms of most of the patients resolved within the first 6 hours of treatment. Conclusions: The standard protocol developed at the institute, which included a high dose of nitroglycerine along with non-invasive ventilation, was efficient in treating acute heart failure due to sympathetic crashing and acute pulmonary edema. The purpose of this study was to examine the link between dengue fever severity and liver function test. Methods: This prospective observational study was conducted in the Department of General Medicine, Madhesh Institute of health sciences, provincial hospital, Janakpurdham in which we included dengue positive patients (aged 18 years or more) based on NS1 antigen or high titer on IgM/IgG testing from July 2023 till August 2023. We excluded patients with diseases like malaria, cirrhosis of liver, enteric fever, viral hepatitis or any other disease or taking any medication which can derange LFT. Results: We included 96 patients fulfilling the study criteria. Of these, 71% had DF, 22% had DHF and 7% had DSS. Among liver enzymes, mean AST of the patients was significantly higher in DSS group of patients (775.19±39.65 U/l), as compared to those in the DF and DHF group of patients, p value <0.01. Similarly, mean ALT of the patients was significantly higher in DSS group of patients (387.8±18.6 U/l), as compared to those in the DF and DHF group of patients, p value<0.01. On the contrary, mean alkaline phosphatase levels were similar between the three patient groups. Conclusions: Based on the results our study, we conclude that raised AST and ALT levels were significantly associated with severity of DSS and DHF. Sometimes a very small stone may require a surgical removal and sometimes a large stone might pass spontaneously. We report a case of a 43 years old male patient who presented to our department with complaint of right flank pain of 1-week duration. This patient was diagnosed with right proximal ureteric calculus (11×6 mm) at L3 level. He was planned for ureterorenoscopy lithotripsy (URSL)/push back percutaneous nephrolithotomy (PCNL). Patient needed time for getting financially prepared for the surgery. Meanwhile we prescribed him an alpha blocker once daily at bedtime, oral analgesia SOS and advised to take adequate hydration. On 10th day of first presentation, to our surprise, the patient came with a stone in his hand, which he has passed spontaneously, and was confirmed by a radiograph. So, the present study is conducted to evaluate the immediate effect of neurodynamic slider and tensioner technique on hamstring muscle flexibility and balance in healthy individuals. Method: Total 42 healthy individuals both male and female age between 18-26 with AKE <70 degree participated in the study. 42 individuals were randomly assigned in two groups. Group A received neurodynamic slider technique followed by group B received tensioner technique. AKE angle was immediately examined before and after the intervention to evaluate hamstring flexibility. Results: Results is concluded that both slider and tensioner technique are equally effective to improve hamstring muscle flexibility (p<0.05) and balance in healthy individuals within the group but. As the findings of between the group shows that both the techniques are equally effective not superior on one another (p>0.05). We presented a case of a 53-year-old female patient who presented to the emergency department with clinical characteristics of abdominal wall abscess with no significant history of its appearance. Abscess drainage plus biopsy of muscle tissue in the anterior region of the abdominal wall was performed, histopathology report compatible with desmoid tumor, radical surgical treatment was decided in a second intervention. Desmoid tumor is rare, since its diagnosis and treatment require a multidisciplinary approach; Active surveillance is currently the treatment of choice for patients who have DD in noncritical locations, and at least 1 to 2 years of active surveillance is now recommended because of its likelihood of spontaneous regression. It has caused almost seven million deaths since its onset. SARS-CoV-2 commonly enters cells by binding to angiotensin-converting enzyme 2 (ACE2) molecules on the surface of cells in the human body. SARS-CoV-2 infection, although mild in many patients, has the potential to cause dysfunction of many organ systems in the body. The body response to the internalisation of the virus in the epithelial cells of the lungs can lead to alveolar epithelial inflammation, commonly referred to as the exudative phase of acute respiratory distress syndrome (ARDS). Cardiac symptoms shown by patients infected with SARS-CoV-2 include chest tightness/pain and palpitations. These features can be because of newly developed or worsening ischaemic heart disease and arrythmias, respectively. SARS-CoV-2 infection is known to cause a clinical condition known as COVID-19-associated nephropathy (COVAN), a disease quite similar to HIV-associated nephropathy (HIVAN). Like HIVAN, COVAN is relatively more common in people of African descent and is associated with the APOL1 variant gene. Researchers have not identified unique morphological changes that could be used to identify the infection in tissues. This study aimed to characterise the phytochemical profile, nutritional composition, and acute oral toxicity of the n-hexane extract of Terminalia catappa nuts. Methods: Gas chromatography-mass spectrometry (GC-MS) analysis was conducted to determine the phytochemical profile of the n-hexane extract of Terminalia catappa nuts. Proximate analysis was performed to evaluate the nutritional composition. Acute oral toxicity testing was carried out in Wistar rats as per OECD Guideline 425 to assess safety. Results: The phytochemical analysis identified 19 bioactive compounds in the Terminalia catappa nut extract. The major components were cis-vaccenic acid (24.493%), propyleneglycol monoleate (23.783%), and mitotane (14.186%). Proximate analysis revealed the nuts to be rich sources of fat (56.71%) and protein (26.30%). The median lethal dose (LD50) of the n-hexane extract in rats was greater than 5000 mg/kg, indicating a high margin of safety. Conclusions: This work provides valuable insights into the nutritional potential and safety profile of Terminalia catappa nuts. Although the correlation between sonographic features of breast cancer and immunohistochemistry markers expression is found, it is not still very clear; thus, this study aimed to investigate the ultrasound features of breast cancer and analyze the correlation between them. Methods: This was a prospective study, in which patients with breast lumps were sonologically categorized as BI-RADS 4 and above. Percutaneous biopsy was done. Histopathology and immunohistochemistry markers were correlated with ultrasound findings. Results: ER (+), PR (+) tumors were associated with irregular shape. ER (+), PR (+), and Her2 (+) tumors were associated with indistinct margin. TNBC tumor was associated with microlobulated margin. TNBC cases had more posterior acoustic enhancement. Conclusions: Young female with ultrasound features of oval/round shape, micro-lobulated margin, abrupt tumor interface, showing posterior acoustic enhancement, absence of microcalcification was significantly associated with TNBC. Tumor with an irregular shape, indistinct margin, hyperechoic halo, no change in posterior acoustic feature, and presence of microcalcification were significantly associated with ER (+) cancers. Tumor with irregular shape, indistinct margin, and no change in the posterior acoustic feature was significantly associated with PR (+) cancers. Tumour with indistinct margin, and hyperechoic halo is significantly associated with Her2 (+) cancers. Known being controversial for its etiology and pathogenesis, diagnosis of acute appendicitis has been challenging despite the development of various clinical and laboratory studies. Inflammatory markers of hematological origin-total leucocyte count (TLC), platelet indices viz. mean platelet count (MPV), plateletcrit (PCT), and platelet distribution width (PDW), have long been preferred as rudimentary diagnostic parameters for acute appendicitis despite being contentious. Methods: An analytical and cross-sectional study among 100 individuals. Statistical evaluation of TLC, MPV, PCT and PDW of 21 appendicitis cases and 70 healthy individuals were complied. Results: Compared to the control group, cases showed significantly higher values of TLC and MPV while the PCT and PDW were normally distributed and showed no significant statistics. The sensitivity of TLC and MPV was determined to be 95.2%, 71.4%, and specificity 100%, 73.8% respectively.
