{"title":"The Decision-Making Experiences of Caregivers Regarding Feeding Tube Placement in Community-Dwelling Adults: A Descriptive Phenomenological Study.","authors":"Deltra C Muoki, Sandra Cesario, Peggy Landrum, Sabrenda Littles","doi":"10.1097/SGA.0000000000000526","DOIUrl":"https://doi.org/10.1097/SGA.0000000000000526","url":null,"abstract":"<p><p>Percutaneous endoscopic gastrostomy feeding tube placement is multifactorial and considered a lifesaving mechanism, which leads to a host of thoughts and feelings that affect the decision-making experience. As people live longer and the population ages, these decisions often involve the caregivers who have their own experience and therefore can result in caregiver burden and anxiety. A descriptive phenomenological study was conducted to describe and understand the caregiver's decision-making experience regarding percutaneous endoscopic gastrostomy feeding tube placement in community-dwelling adults. Edmund Husserl's philosophical underpinnings were utilized in conjunction with Colaizzi's (1978) method of data analysis to maintain the rigor of the study. Sixteen adult caregivers of patients from six rehabilitation and skilled nursing facilities were interviewed using a semistructured interview guide. The audio-recorded interviews were transcribed and thematic analysis was conducted. Coronavirus disease-2019 (COVID-19) is a viral infection that has become an international public health crisis. With this virus, the targeted organ is the lungs. However, extrapulmonary manifestations of the illness can also occur. In the early phases of the disease process, gastrointestinal symptoms have been observed and can be seen before respiratory symptoms become present (Lee, Huo, & Huang, 2020). Angiotensinconverting enzyme 2 (ACE2) is found in organs throughout the body including the heart, lungs, kidney, and liver (Yuki, Fujiogi, & Koutsogiannaki, 2020; Zaim, Chong, Sankaranarayanan, & Harky, 2020). ACE2 can also be found in the upper portion of the esophagus, ileum, and colon (Luo, Zhang, & Xu, 2020). The spiked proteins of coronavirus attack ACE2, which serves as a gateway that provides access to the interior of the human cell where it then replicates, kills the cell, and spreads (Zhang, Shi, & Wang, 2020). Reports of gastrointestinal complaints are common in primary care and account for 10% of the disease processes primary care providers treat (Gikas & Triantafillidis, 2014). In the gastroenterology setting, the complaint of diarrhea is also a common occurrence. Because COVID-19 is a novel virus, the usual presenting symptoms of respiratory and gastrointestinal symptoms could be mistaken for the typical etiology thus leading to misdiagnosis. Individuals who have COVID-19 are most contagious when they are asymptomatic and early in the disease process (He et al., 2020; Nebehay, 2020; Seay, 2020). {"title":"Pediatric and Maternal Anxiety at First Visit to a Turkish Gastroenterology Clinic Compared to Anxiety Prior to Pediatric Endoscopy Procedure.","authors":"Sebahat Cam","doi":"10.1097/SGA.0000000000000525","DOIUrl":"https://doi.org/10.1097/SGA.0000000000000525","url":null,"abstract":"<p><p>The aim of this study was the prospective comparison of pediatric and maternal anxiety at the first visit to a gastroenterology clinic with anxiety prior to a pediatric endoscopy procedure. A total of 101 children aged 4-18 years plus their mothers who were referred to an outpatient pediatric gastroenterology clinic were included in the first group. The second group consisted of 101 different children aged 4-18 years undergoing pediatric endoscopy. The Hospital Anxiety and Depression Scale (HADS) was administered to mothers. Visual analog scale scores were obtained from the children to measure anxiety. The demographic characteristics of the groups were similar. The mean HADS anxiety score was 3.83 in mothers of the first group. The same score was significantly higher as (6.96) in the mothers whose children underwent an endoscopy (p < .001). The comparison of the depression scores revealed significantly higher scores in the endoscopy group. Visual analog scale values of the children revealed remarkably higher anxiety scores in the endoscopy group. About 5% of mothers in the first group and almost half of the mothers in the endoscopy group developed remarkable anxiety (>8 HADS anxiety). The rate of significant depression (>8 HADS depression) was 2% and 17.82% in the first and second groups, respectively.
