As this minimally invasive approach continues to gain popularity across various surgical disciplines, it is essential to address the associated challenges and complications that may arise. Aim of the Study: The aim of this study was to analyze the microbial distribution and antibiotic sensitivity patterns in laparoscopic port site infections. Methods: A prospective observational study was conducted on a cohort of 100 patients who underwent laparoscopic procedures at MH Samorita Medical College & Hospital, Hi Tech Surgicare Hospital & piles Centre, Savar Specialized Hospital, Lab Zone Hospital Savar, Dhaka, Bangladesh from Jan 2016 to Dec 2023. The study focused on identifying cases with culture-positive laparoscopic port site infections. Result: The Laparoscopic cholecystectomy being analyzed by Cholelithiasis, Acute cholecystitis, Ch. Cal. Cholecystitis, Empyema GB, and GB polyp. For Cholelithiasis, 33.3% of males and 40.5% of females had this condition. Regarding Acute cholecystitis, it was found in 20.0% of males and 16.7% of females. Majority 36.7% were 40 – 50 years in age group for male and 30.9% were in 40 – 50 years in age group for female respectively. Out of the total 100 patients in the study, 71% patients tested positive for port site infection. Carica papaya leaves juice is said to be increase platelet count and improve haematocrit. Objective: This study aimed to determine the effect of Carica papaya leaf juice on plasma leakage and platelet count in child patients with DF and DHF. Methods: This randomized controlled trial was conducted among 77 children with DF and DHF. Patients were purposively selected from the inpatient department of Dhaka Shishu hospital from July 2018 to December 2019. Patients were randomly allocated in group A and group B. Group A patients received standard treatment along with a bottle containing C. papaya leaves juice 10 ml single morning dose 15 minutes after breakfast for three consecutive days and group B patients received only standard management as per the National Guideline for Clinical Management of Dengue Syndrome, 2018. Three patients in group A and two patients in group B dropped out from the study. Results: There was no significant difference between the groups regarding age, sex, mean duration, type and severity of fever between the groups (p>.05). In group A, the mean platelet count increased from 105000.00/ mL to 152428.57/mL after 48 hours and it reached to 212228.57/mL at 72hrs. In group B, the mean platelet count initially decreased from 129485.71/ mL to 91114.28/mL after 48 hours and then it increased to 96142.85/mL at 72 hours. There was highly significant statistical difference between group A and group B at 48 hours and at 72 hours regarding mean platelet count as p<.001. Haematocrit values become significantly normal in group A then group B by 72 hours and p˂.05. There was highly significant statistical difference between group A and group B regarding mean hospital stay as p<.001. Additionally, the challenge arises when dealing with patients on antiplatelet or anticoagulant medications due to the associated risk of bleeding, hence the significance of RTUP-b. The objective of our study is to compare the functional outcomes and immediate postoperative complications between these two techniques and to identify predictive factors for complications. Methods: This is a prospective randomized study conducted between January 2022 and September 2022. The variables age, BMI, surgical indication, transrectal prostate volume, ASA classification (American Society of Anesthesiologists), previous pharmacological treatments, IPSS score, quality of life index (QLI), maximum flow rate (Qmax) and postoperative complications were collected. The Chi2 test and Student's t-test were used for statistical analysis. Results: Sixty-nine consecutive patients were included, 38 patients in the monopolar TURP group and 31 in the bipolar TURP group. Both groups were homogeneous in their baseline characteristics. The overall pre-surgical IPSS/QLI/Qmax score was 24,03/4,73/9,47. Functional results at 1st and 3rd month are shown in Table 1. For complications, see Table 2 and 3. These differences were not statistically significant (p = 0,427): the most frequent were hematuria (9,4%) and febrile urinary tract infections (9,4%). Only one patient presented hyponatremia due to glycocol reabsorption in the monopolar group. All complications were Clavien 1 or 2. We found no significant differences between the two groups with regard to complications or their predictive factors. Hypothyroid patients, particularly women of childbearing age, the elderly and/or cardiac patients, need to have a minimum level of knowledge of their pathology. The aim of our study was to assess the level of knowledge of a group of hypothyroid patients. The study involved 140 hypothyroid patients over a period of 06 months. The evaluation was carried out using a questionnaire, and the responses were recorded and analyzed using Excel software. 88.7% of patients knew that they had a hypofunctioning thyroid gland, with 84% identifying the cause of hypothyroidism. 85% knew how to take L-Thyroxine, 22% had an idea of the notion of drug interactions, 2% knew the signs of overdosage and only 3% knew the signs of underdosage. In the case of 52 patients of childbearing age, 92% are unaware that pre-conceptional balancing is compulsory, compared with 8% who are already aware of it. Our current survey reveals a significant lack of knowledge among hypothyroid patients, and has enabled us to re-educate this group of patients. In recent years, there has been a growing interest in identifying reliable biomarkers that can aid in the early detection, prognosis, and monitoring of breast cancer. High sensitivity C-reactive protein (hs-CRP), a sensitive marker of systemic inflammation, has gained attention for its potential association with various malignancies, including breast cancer. Objective: This study was designed to estimate the serum level of hs-CRP in different stages of breast cancer patients and determine its relation with the disease progression. Methods: This cross-sectional analytical study included a total of 60 women ranging in age from 30 to 65, and divided into two groups. Group A includes 30 diagnosed breast cancer patients, and group B includes 30 age-matched apparently healthy women according to the selection criteria. Serum hs-CRP (mg/L) was estimated for both groups. The mean and median values of the variable were compared between the groups by unpaired students't-tests and Mann-Whitney U tests. The level of significance was defined as p-value <0.05 at 95% confidence interval. Results: There was no significant difference in age and BMI between study groups. The median value of serum hs-CRP (mg/L) level was significantly (p<0.009) higher in breast cancer patients (3.57 mg/L) when compared to age-matched healthy women (1.56 mg/L). Serum hs-CRP concentrations of cases were increased significantly with the staging of breast cancer. Conclusion: This study revealed a significant relationship between hs-CRP levels with different stages of breast cancer. Despite the availability of diverse treatment modalities, transurethral resection of prostate (TURP) has conventionally served as a benchmark intervention. Recently, there has been a growing interest in bipolar energy techniques, spurred by their potential advantages in mitigating complications such as TUR syndrome and surgical bleeding. Objectives: The main aim of the study to evaluate the safety profile (including serum sodium and hemoglobin reductions, clot retention, TUR syndrome, catheterization duration, and hospital stay) and efficacy (resection rate, maximum flow rate increase, and International Prostate Symptom Score reduction) within the bipolar technique for the TURP. Methods & Materials: A prospective observational study was conducted at the Department of Urology, Square Hospitals Ltd, Dhaka, Bangladesh, spanning December 2020 to November 2021. The study employed comprehensive preoperative assessments, including bipolar transurethral resection of the prostate (B-TURP) using Olympus ESG-400 energy platform. Surgical parameters, postoperative monitoring, and statistical analysis were meticulously executed to evaluate outcomes and complications for symptomatic benign enlargement of prostate. Results: In a sample of 50 individuals undergoing bipolar transurethral resection of the prostate (B-TURP), the distribution of ages revealed 66.97±7.95 years as the bipolar system's average age, ranging from 53-82 years. The majority resided in urban areas (62%), with family income predominantly in the 25000-50000 bracket (42%). Indeed, patients with chronic kidney disease (CKD) are a population at risk of developing infections caused by antibiotic-resistant bacteria (ARBs), given their overexposure to healthcare facilities and the quality of their gut microbiota already damaged by CKD. It is a population with very high rates of colonization and ARB infection worldwide. The mechanisms deployed by these AROs to counteract the effect of antibiotics are multiple. This may include the production of antibiotic-inhibiting enzyme (ATB), waterproofing of the bacterial membrane, or modification of the antibiotic target. They include methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-resistant Enterococcus (VRE) species, and several multidrug-resistant Gram-negative organisms. The emergence and global spread of these ARBs is facilitated by ATB selection pressure, inter-agency transmission of resistance determinants, suboptimal infection control practices, and frequency of international travel, among other factors. Methods: This cross-sectional comparative study was conducted at the department of surgery with collaboration of department of Anaesthesiology in CMH for 6 months of period. Before commencement of the study formal ethical clearance was taken from the Ethical Committee of DGMS Army. Total 60 patients, scheduled for laparoscopic cholecystectomy under general anesthesia were included in this study and randomly divided into two groups (30 patients in each group). total 30 Patients, received both GA and ultrasound guided ESP block were assigned as group E and 30 Patients, received GA and opioid analgesia were in group N. Informed written consent was taken from each subject and ethical measures were followed strictly in whole study. Following data collection, data were analyzed by the SPSS 24. Results: In terms of age, sex, BMI and ASA grading, the two groups had identical demographic features (p>0.05 in all cases). There was no significant difference in pre-operative, post-operative, or recovery room heart rate (p>0.05) or mean arterial pressure (p>0.05). In the recovery room, group E has significantly less pain than group N (3.67±0.802 vs 4.3±0.877; p<0.05). A similar improvement was seen in group E at 6, 12 and 24 hours (p<0.05 in all follow ups). Post-operative analgesic consumption was similarly higher in group N at 1, 12, and 24 hours (p<0.05 in all follow up). Till mid-March 2022, around 460 million people already suffered from COVID-19, among them around 6 million people died and globally death rate was 2%. In Bangladesh till mid-March 2022, around 1.9 million people suffered from COVID-19 and around 29 thousand people had died. As pregnancy is a physiological immunosuppressive state, so pregnant women might be more susceptible to this infection. Studies on COVID-19 during pregnancy have showed, this infection occurred more during third trimester of pregnancy (about 64%). Reviewing several other studies on COVID-19 infection we found that, elevated serum ferritin level was associated with its severity; and ferritin could effectively discriminate severity and non-severity with sensitivity 96% and specificity 70%. We conducted this study to evaluate association of serum ferritin level with severity of COVID-19 infection during third trimester of pregnancy. Objectives: To evaluate association between rise of serum ferritin level and severity of COVID-19 infection during third trimester of pregnancy. Methods: This cross-sectional study was carried out in Department of Obstetrics and Gynaecology, COVID-19 Unit, BSMMU and DMCH. Sampling method was purposive. Total 77 cases were included in this study after matching eligibility criteria. After collecting intravenous blood, serum ferritin level was measured. Results: Among 77 cases of third trimester COVID-19, 19 cases (24.7%) were asymptomatic, 31 cases (40.3%) were mild, 19 cases (24.7%) were moderate and 8 cases (10.4%) were severe COVID-19. Non- severe COVID-19 group included 50 cases of asymptomatic & mild COVID-19; and their mean serum ferritin level was 51.48 ng/ml.
