Ces affections sont secondaires à une atteinte dysplasique des différents feuillets embryonnaires, et particulièrement du neuroectoderme, expliquant le caractère multi-systémique de ces affections ainsi que leur polymorphisme clinico-radiologique. Étant les plus constantes et souvent révélatrices de ces affections, les lésions cutanées constituent un marqueur pour la plupart d’entre elles. L’atteinte du système nerveux central est également fréquente, vu la communauté d’origine génétique entre la peau et ce dernier. Elle constitue un élément capital pour le pronostic. À travers ce travail, les auteurs rappellent les caractéristiques cliniques et neuroradiologiques des affections les plus courantes (telles que la neurofibromatose type 1, la sclérose tubéreuse de Bourneville, la maladie de Sturge-Weber et la maladie de Von Hippel-Lindau) et attirent l’attention sur la place de l’imagerie, et particulièrement l’imagerie par résonance magnétique (IRM), dans le diagnostic précoce, le pronostic ainsi que le suivi évolutif de ces affections.</p></div><div><p>The neurocutaneous syndromes or phacomatoses are a group of genetic disorders that produce a variety of developmental abnormalities due to dysplastic lesions of the different germ layers, particularly the neuro-ectoderm, explaining the multi-systemic nature of these diseases and their clinical and radiological polymorphism. Being the most constant and often indicative of these conditions, skin lesions are a marker for most of them. The central nervous system is also frequently affected, which is a crucial element for prognosis. Jiddane","doi":"10.1016/j.frad.2015.10.006","DOIUrl":"10.1016/j.frad.2015.10.006","url":null,"abstract":"<div><h3>Objectif</h3><p>Savoir repérer par imagerie le noyau sous-thalamique, qui correspond à une structure anatomique encéphalique profonde de très petite taille (20–30<!--> <!-->mm<sup>3</sup>), dont la stimulation dans les formes avancées de la maladie de Parkinson est une méthode thérapeutique récente qui a fait ses preuves.</p></div><div><h3>Technique</h3><p>La technique repose d’abord sur la réalisation d’une IRM encéphalique selon un protocole très précis. L’IRM sera couplée dans un deuxième temps aux données de la TDM encéphalique spiralée.</p></div><div><h3>Résultats</h3><p>Pour localiser le noyau sous-thalamique, il est nécessaire de définir au préalable la ligne bicommissurale commissure antérieure-commissure postérieure (CA-CP), et de localiser les noyaux rouges et la substancia nigra. Le noyau sous-thalamique est situé immédiatement en dehors de la partie antérieure du noyau rouge et en dedans de la capsule interne. Il est en hyposignal T2, et se trouve à 5<!--> <!-->mm en bas, 1–2<!--> <!-->mm en arrière et 9–12<!--> <!-->mm en dehors du point médio-commissural.</p></div><div><h3>Conclusion</h3><p>L’efficacité du traitement par stimulation du noyau sous-thalamique dépend de la précision du repérage de ce dernier, d’où la nécessité d’utiliser les séquences adaptées et les repères appropriés.</p></div><div><h3>Objective</h3><p>Describe imaging localization of the subthalamic nucleus, which corresponds to a very small (20–30<!--> <!-->mm<sup>3</sup>) and deep encephalic structure, that, according to a recent and effective therapeutic method, can be stimulated in advanced cases of Parkinson's disease.</p></div><div><h3>Technique</h3><p>The technique is based on brain MRI with a very precise protocol. A secondary brain CT may also be useful.</p></div><div><h3>Results</h3><p>To localize the subthalamic nucleus, the bi-commissural anterior commissure-posterior commissure (AC-PC) line has to be define together with the localization of the red nucleus and the substancia nigra. The subthalamic nucleus is situated immediately lateral to the anterior edge of the red nucleus and medial to internal capsule.
