This study aimed to determine whether refitting symptomatic soft CL wearers in to orthokeratology could improve ocular symptoms and signs.

Methods: This was a prospective, 3-month, open-label study of symptomatic (Contact Lens Dry Eye Questionnaire [CLDEQ-8] ≥12) soft CL wearers who were between the ages of 18 and 45 years. All subjects were refit into orthokeratology CLs (Emerald, Euclid Systems). The following tests were completed: CL history, Standardized Patient Evaluation of Eye Dryness (SPEED) questionnaire, CLDEQ-8, CLDEQ-4, logarithm of the minimum angle of resolution visual acuity, pupil size, refractive error, slit-lamp biomicroscopy, noninvasive tear break-up time, tear meniscus height, phenol red thread, conjunctival staining, corneal aesthesiometry, and corneal topography.

Results: Twenty-nine of 40 qualifying subjects (age=24.43±4.62 years) completed the study. No significant differences were detected between completed and noncomplete subjects. Completed subjects had significantly better CLDEQ-8, CLDEQ-4, and SPEED scores at 3 months compared with baseline. Completed subjects had significantly better conjunctival staining scores and flatter keratometry values at 1 month compared with baseline.

Conclusions: Although not all symptomatic soft CL wearers were able to be refit into orthokeratology, subjects who were wearing orthokeratology at 3 months had a significant and clinically meaningful improvement in ocular symptoms. This work uses a simplified questionnaire to evaluate the changes in DED-related symptoms.

Methods: A new questionnaire based on a global rating of change scale was designed. The Change in Dry Eye Symptoms Questionnaire (CDES-Q) consists of 2 questions: CDES-Q1 asks for the change in symptoms (\"better,\" \"same,\" or \"worse\") relative to a determined previous time and CDES-Q2 quantifies this change (range: 0 to +100). To evaluate the CDES-Q, a prospective observational study was performed. At baseline (V1; day-0), DED-related symptoms were evaluated using the ocular surface disease index (OSDI). In the post-treatment visit (V2; day-90), OSDI, Symptoms Assessment Questionnaire in Dry Eye (SANDE) II, and CDES-Q were used. Also, clinical evaluations were performed in each visit.

Results: Thirty-six patients were included. At V2, OSDI, SANDE II, and CDES-Q showed a significant reduction in symptoms (-7.17±12.73, P=0.0021; -11.29±20.95, P=0.0035; -25.28±42.28, P=0.0011, respectively). The median number of D. folliculorum mites was 1 (interquartile range [IQR], 1-2) and that of D. brevis was 1 (IQR, 1-1). Although the authors highlight the fact that the main objective with Th PK is to reestablish the integrity of the globe, many times in an urgent setting, the excellent results leading them to conclude that this procedure achieves corneal clarity and improves vision in most patients have to be taken to caution, specially in some scenarios. There is a considerable shortage of corneal graft tissue worldwide, with an important percentage of world’s population having no access to corneal transplantation.2 In some countries, although this effort is constantly made, the access to corneal tissue is still difficult; the corneal tissue without an optical quality, due to a low endothelial cell count, or inadequate corneal excision technique, is sometimes stored to be used in ophthalmic emergencies. These issues generate lower chances of improvement in corneal clarity at follow-up. In addition to that, the presence of aggressive pathogenic organisms may lead to different complications as mentioned by the authors, including cataract, glaucoma, or graft failure.1 In a previous series of nontraumatic corneal perforations presented by us, the infectious etiology group had a worst visual acuity at presentation (median logMAR 3.0, interquartile range [IQR] of 1.91–4), and also, a worst visual acuity at final consultation (median 3.0, IQR 1.18 to 3), with most of the patients in this group being treated with Th PK.3 Another important issue not mentioned is the possibility of an infection recurrence, being important in all bacterial, viral, and fungal keratitis, deserving attention and continued treatment.3,4 Finally, I emphasize the value of this article in supporting the use of Th PK in the treatment of corneal perforations.","PeriodicalId":502139,"journal":{"name":"Eye & Contact Lens","volume":" ","pages":"212"},"PeriodicalIF":2.3,"publicationDate":"2019-05-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1097/ICL.0000000000000565","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"40448435","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
