During the annual US workshop at the GRAPPA 2024 annual meeting in Seattle, Washington, participants reviewed preliminary analyses from the Diagnostic Ultrasound Enthesitis Tool (DUET) study and discussed the initial results of a DUET substudy aimed at defining enthesitis at the single enthesis level. Participants also previewed the ongoing systematic review of articular and periarticular manifestations of PsA, with particular attention to smaller joints. Axial symptoms affect a significant portion of individuals with PsA. The Axial Involvement in Psoriatic Arthritis (AXIS) study, a multinational, multicenter cross-sectional study conducted under the umbrella of the Group for Research on Psoriasis and Psoriatic Arthritis (GRAPPA) and Assessment of SpondyloArthritis international Society (ASAS), aimed to establish a unified definition and classification criteria for axial involvement in PsA. The study enrolled 409 patients from 41 centers across 19 countries. Comprehensive clinical and imaging assessments were conducted for each patient including radiographs and magnetic resonance imaging of the sacroiliac joints and spine. Imaging data were reviewed by both local investigators and a central imaging committee. Final judgment on the presence or absence of axial involvement was made by investigators following evaluation of all data by the investigators and the central expert imaging team. Axial involvement was assessed to be present in 112/409 patients (27.4%). Data analysis is ongoing and the full study results are expected shortly. We assessed health-related quality life (HRQOL) in adults with SAPHO-CNO and performed a review of EQ-5D outcomes amongst similar chronic rheumatic and inflammatory diseases.METHODSWe enrolled patients in the first US-based SAPHO-CNO prospective registry and assessed their HRQOL using the EQ-5D index and EQ-5D VAS. A focused scoping review was performed of EQ-5D related outcomes amongst disease cohorts with phenotypic similarities to SAPHO-CNO.RESULTSIn the 138 SAPHO-CNO participants, mean age (standard deviation) was 37.7 (15.0) years with a mean disease duration of 6.4 (6.2) years. All subjects had musculoskeletal involvement (44% having spine involvement) and 48% had skin involvement. Mean EQ-5D-5L index value was 0.75 (0.16) and mean EQ-VAS was 63.0 (21.0). Most patients were impacted by pain/discomfort (86%), difficulty in carrying out usual activities (65%) and anxiety/depression (54%); over one-third reported problems in all five EQ-5D domains. These SAPHO-CNO adults had similarly reduced EQ-5D and VAS values, and similar high proportion of perceived problems, as other inflammatory disease cohorts.CONCLUSIONThe HRQOL among adults with SAPHO-CNO is low, with more than half of patients experiencing an impact in perceived pain, performing usual activities, and feelings of anxiety or depression. Exposures were: 1) receipt of an additional dose of monovalent COVID-19 vaccine prior to January 1, 2022, with follow-up to August 31, 2022; and (2) receipt of bivalent COVID-19 vaccine between September 1, 2022 to August 31, 2023.RESULTSAmong 201,165 patients with SARDs, 126,756 (63%) had received one monovalent booster as of January 1, 2022. During 94,842 person-years of follow-up, the COVID-19 hospitalization rate was 15.6 per 1000 person-years among those who had received a monovalent booster versus 20.1 per 1000 person-years among those who had not, with an adjusted VE of 38% (95% confidence interval [CI] 31% - 44%) and a number needed to vaccinate of 267 (CI 230-325). Among 246,991 patients with SARDs with 233,622 person-years of follow-up in the bivalent study period, the COVID-19 hospitalization rate was 7.9 per 1000 person-years for the 87,906 (36%) patients who received the bivalent vaccine, versus 10.2 per 1000 person-years for the patients who did not. The adjusted VE of the bivalent vaccine was 32% (CI 24% - 39%) with a number needed to vaccinate of 617 (CI 500-838).CONCLUSIONCOVID-19 booster vaccinations provided significant protection against severe COVID-19 in persons with autoimmune disease. However, the development of multiple regional guidelines often leads to methodological overlap, variability in implementation, and resource redundancy. This Viewpoint compares recent recommendations from ACR, GRAPPA, EULAR, PANLAR exploring their similarities and differences in methodology, structure, and stakeholder composition. We discuss the challenges in maintaining updated recommendations in a rapidly evolving therapeutic landscape and propose a collaborative framework centered on core global recommendations with regional adaptations. Emerging tools, such as living systematic reviews and artificial intelligence, may support more efficient updates, but stakeholder engagement remains essential.
