{"title":"Anastomotic Leak in Colorectal Surgery: Predictive Factors and Survival.","authors":"Swetha Prabhakaran, Sowmya Prabhakaran, Wei Mou Lim, Glen Guerra, Alexander G Heriot, Joseph C Kong","doi":"10.5604/01.3001.0016.1602","DOIUrl":"https://doi.org/10.5604/01.3001.0016.1602","url":null,"abstract":"<p><p>&lt;br&gt;&lt;b&gt;Introduction:&lt;/b&gt; Anastomotic leak (AL) is a serious complication following colorectal surgery.&lt;/br&gt; &lt;br&gt;&lt;b&gt;Aim:&lt;/b&gt; The aim of this study was to identify factors associated with the development of AL and to analyze its impact on survival.&lt;/br&gt; &lt;br&gt;&lt;b&gt;Materials and methods:&lt;/b&gt; All consecutive adult colorectal cancer resections performed between 2007 and 2020 with curative intent and anastomosis formation were included from a prospectively maintained database. The primary outcome measure was the rate of AL. The secondary outcome measure was 5-year overall survival (OS).&lt;/br&gt; &lt;br&gt;&lt;b&gt;Results:&lt;/b&gt; There were 6837 eligible patients. The rate of AL was 2.2% and 4.0% in patients with colon and rectal cancer, respectively. AL was a significant independent predictor of reduced 5-year OS in patients who underwent curative surgery for rectal cancer (odds ratio 2.293, p = 0.009). Emergency surgery (p = 0.015), surgery at a public hospital (p = 0.002), and an open surgical approach (p = 0.021) were all associated with a significantly higher risk of AL in patients with colon cancer, with higher rates of AL noted in left colectomies as compared to right hemicolectomies (4.4% &lt;i&gt;vs.&lt;/i&gt; 1.3%, p &lt; 0.001). In rectal cancer patients, AL was associated with neoadjuvant chemoradiotherapy (p = 0.038) and male gender (p = 0.002). {"title":"Dupuytren's disease - what's new: a review.","authors":"Andrzej Żyluk","doi":"10.5604/01.3001.0016.0058","DOIUrl":"10.5604/01.3001.0016.0058","url":null,"abstract":"<p><p>&lt;b&gt;&lt;br&gt;Introduction:&lt;/b&gt; Dupuytren's disease is a common fibrotic disorder of the palmar aponeurosis characterized by the formation of nodules and cords, as well as development of progressive flexion deformities in the digits, leading to functional impairment. Surgical excision of the affected aponeurosis remains the most common treatment. Quite a few new information appeared about epidemiology, pathogenesis and particularly treatment of the disorder.&lt;/br&gt; &lt;b&gt;&lt;br&gt;Aim:&lt;/b&gt; The aim of this study is an updated review of scientific data in this topic.&lt;/br&gt; &lt;b&gt;&lt;br&gt;Results:&lt;/b&gt; Results of epidemiologic studies showed that Dupuytren's disease is not so uncommon in Asian and African population as it was earlier believed. An important role of genetic factors on development of the disease in a proportion of patients was demonstrated, however, it did not translate neither to the treatment nor to the prognosis. The most changes concerned the management of Dupuytren's disease. A positive effect of steroids injections into the nodules and cords for inhibition of the disease in early stages was shown. In the advanced stages, a standard technique of partial fasciectomy was partly replaced by mini-invasive methods such as needle fasciotomy and collagenase Clostirdium hystolyticum injections. Colonoscopy procedure, which is applied in the early diagnosis and treatment of this disease, has a very important role. In this study, we aimed to examine the effect of the COVİD-19 pandemic period on our colonoscopic procedures. &lt;/br&gt;&lt;/br&gt; &lt;b&gt; Material and methods:&lt;/b&gt; In this observational study, the data of the patients who underwent colonoscopy in our General Surgery Endoscopy Unit, between March 11, 2019 and March 12, 2021 were scanned retrospectively. Patients under 18 years of age were excluded. The patients were divided into 2 groups. 1. Group patients between 11 March 2019-11 March 2020; patients in the pre-COVID-19 period, Group 2, on March 12, 2020-March 12, 2021; Grouped as the COVID-19 era. &lt;/br&gt;&lt;/br&gt; &lt;b&gt;Results:&lt;/b&gt; Data of 8285 patients were analyzed. A total of 4889 patients in Group 1 and 3396 patients in Group 2 were included in the study. The detection of precancerous polyps between the groups was found to be significantly higher in group 1 (p &lt; 0.05) (4.3% vs 2.1). Similarly, the presence of precancerous polyps over the age of 65 was found to be significantly higher in the pre-covid group. In group 1, no significant difference was found in the evaluation of cancer patients according to gender (p &gt; 0.05) (F/M: 1.2%/1.6%). In group 2, cancer patients were found to be significantly higher in males. &lt;/br&gt;&lt;/br&gt; &lt;b&gt;Conclusions:&lt;/b&gt; The COVİD-19 pandemic has had negative effects in many areas, as well as on our colonoscopic procedures. Surgical treatment depends on the form of the disease and surgeon&apos;s preferences regarding the method. The treatment process does not consider preoperative treatment that would affect the structural components of the skin involved in the pathological process. The results of surgical treatment remain unsatisfactory, leading to a significant number of recurrences and long-term healing of the postoperative wound. &lt;/br&gt;&lt;/br&gt; &lt;b&gt; Materials and methods:&lt;/b&gt; Morphological examination of the pieces of skin with altered sacrococcygeal tissues after radical surgical treatment of 46 patients with pilonidal cyst of the sacrococcygeal region was performed, and 46 patients with acne inversa of the intergluteal cleft, groin and scalp were selected. The sex distribution of patients with pilonidal cysts was as follows: 43 (91.3%) male patients, 3 (6.5%) female patients. Among 46 patients with acne inversa and dissecting cellulitis, the distribution was as follows: 32 (69.6%) male patients and 14 (30.4%) female patients. Patients with pilonidal cysts of the sacrococcygeal region underwent surgical treatment according to the developed method of economical median resection using sutures with internal fixation. &lt;/br&gt; &lt;/br&gt; &lt;b&gt; Results:&lt;/b&gt; Considering and comparing the morphological picture observed in the pathomorphological examination of histological specimens in patients with pilonidal cysts, acne inversa, dissecting cellulitis, the similarity of changes in most patients is noteworthy. \"Acne inversa\" was first described in 1839 by Velpeau, who originally called the disease \"hydradenitis suppurativa\", believing that inflammatory changes occur in the sweat glands [1-3]. In 1854, surgeon Verneuil described this disease, and later it was named after him. We observed chronic proliferative inflammation in different layers of the dermis and subcutaneous tissue (the main focus is in the subcutaneous tissue, but closely related to the dermis and epidermis). &lt;/br&gt;&lt;/br&gt; &lt;b&gt;Discussions:&lt;/b&gt; Pilonidal cyst of the sacrococcygeal region, especially without hair in the cavity of the cyst, can be considered as a type of acne inversa with appropriate location. The gold standard treatment is lateral internal sphincterotomy (LIS), and this procedure carries a risk of incontinence. &lt;/br&gt;&lt;/br&gt; &lt;b&gt; Aim:&lt;/b&gt; The aim of this study is to determine the long-term risk of fecal incontinence after LIS. &lt;/br&gt;&lt;/br&gt; &lt;b&gt; Method:&lt;/b&gt; All patients who had LIS for chronic anal fissure between the years 2004-2010 were interviewed by phone and assessed for sphincter function (incontinence) using Wexner fecal incontinence score (WIS). &lt;/br&gt;&lt;/br&gt; &lt;b&gt;Results:&lt;/b&gt; Fifty-nine patients (34 females, 57.6%) with a mean follow-up duration of 10.6 years (range 8-15 years) were interviewed. Twelve patients (20.3%) had a WIS score of one or more. The majority of the patients noticed a change in sphincter function years after the operation. There was no association between vaginal delivery and the WIS score. &lt;/br&gt;&lt;/br&gt; &lt;b&gt;Conclusion:&lt;/b&gt; The long-term risk of abnormal sphincter function after LIS appears to be higher than expected, especially in the presence of multiple vaginal deliveries or systemic diseases such as diabetes mellitus. In such a scenario, unexpected, sudden complications can happen during surgery. While better diagnostic and advanced techniques in surgery, have minimised surgical errors to a great extent, with the risk of post-operative death being as low as 3.6% now, still when complications due occur, the surgeon faces a huge backlash not only from the patient relatives, but from his peers, the hospital management, the social and print media. The surgeon also fears violent retribution, not just consumer issues but a threat of arrest and legal battles. All these make a surgeon the \"second victim\" in the event of a post-operative complication, leading to changes in practice behaviour, emotional turmoil and even serious mental issues like depression and suicide. In August 2021, two independent reviewers reviewed a number of articles with the aim of finding studies on the management of acute cholecystitis during the COVID-19 pandemic. Articles were searched in the Cochrane, Embassies, and Medline libraries. Using the Stata statistical software 14, the estimated pooled rates were calculated. Funnel plot and I2 indices were applied for evaluating the heterogeneity between the studies.&lt;/br&gt; &lt;br&gt;&lt;b&gt;Results:&lt;/b&gt; An overall of 8 studies consisting of 654 patients suspected for AC were included. The prevalence of COVID-19 among our included patients was 82% (95% CI: 79-84%, I2: 99.2%). Regarding the type of management, 35% (95% CI: 26-45%, I2: 46.9%) of patients undergone cholecystectomy, 47% (95% CI: 43-51%, I2: 54.4%) were managed by non-surgical methods, and 19% (95% CI: 14-23%, I2: 68.1%) of patients were treated by percutaneous cholecystostomy. The prevalence of grade 2 and 3 among our patients was 44 and 15%, respectively.&lt;/br&gt; &lt;br&gt;&lt;b&gt;Conclusions:&lt;/b&gt; Considering the fact that due to the current pandemic, the number of patients referring with higher grades is assumed to be increased, early cholecystectomy remains the best management option for AC patients. However, LC seems not to be the most favorable option since it is associated with a relatively higher risk of contamination with COVID-19. In general surgery practice, incarcerated / strangulated inguinal hernias take a prominent place among emergency surgeries. In 1% of these patients, the hernia contents retreat spontaneously into the abdomen until the hernia sac is opened. It is strongly recommended that these bowel segments be evaluated for possible intestinal necrosis.&lt;/br&gt;&lt;/br&gt; &lt;b&gt;Results:&lt;/b&gt; Patients who underwent emergency surgery and hernioscopy in the Sakarya Training and Research Hospital General Surgery Service due to incarcerated or strangulated inguinal hernia between March 2020 and October 2020 were included in the study. Hernioscopy procedure was performed using the single-port and glove-port methods. Visits to telemedicine have increased to provide access and maintain continuity of care. Internet access has almost become a necessity, and new technologies allow for the easy flow of data from patient to doctor and vice versa. Doctors of all specialties were forced to adapt to the pandemic and emerging conditions. The provision of surgical services and the learning ability of surgeons are particularly disrupted, and the pediatric surgical community is not exempt. During the pandemic, telemedicine proved to be a viable and safe technique for providing health services.
