As well as, update and identify advances in Spain to improve care for grief. Method: Observational, descriptive, prospective and comparative study. Made from data from the EAPC bereavement group relating to Spain, collected through a questionnaire consisting of 54 questions in 6 sections: background; support; personal; access, levels of care; community aspects; resources and care model. Distributed by online mail through the EAPC to 56 national PC associations. Comparisons between Europe and Spain were made using Pearson's χ2 test. Results: Bereavement care is offered in 83 % of the 314 responding services in Europe and in 73 % of 56 services in Spain, with similar data to previous Spanish studies. Most services correspond to hospital units. The type of support or specialized intervention depends on the level of care in both groups. In Europe, 27 % have National Grief Guides in their country, in Spain 61 % of the services, which facilitates action. “Formal risk instruments” are used by only a quarter of the services. Conclusions: This study presents an advance and a future challenge in bereavement care in Europe and Spain. Data has shown that bereavement care is not part of all services. The number of responses to the questionnaires has not increased nor have the services in Spain increased. Both in Europe and in Spain, it is necessary to continue promoting, formalizing and increasing services with attention to grief, as an essential part of palliative care services.","PeriodicalId":49831,"journal":{"name":"Medicina Paliativa","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"68138785","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objective: To explore the need for palliative care in hospitalized HIV/AIDS patients by health providers. Method: An observational, cross-sectional study was carried out at the “Pedro Kourí” Institute of Tropical Medicine between March and April 2016, with patients infected by the human immunodeficiency virus in the AIDS stage. The exclusion criteria were the willingness of the patient not to be part of the study and those cases that died 48 hours after admission. The NECPAL questionnaire was applied to the attending physicians to identify patients with palliative care needs. Results: Of the 123 patients studied, 87.8 % were men, under 45 years of age (66.7 %), with more than 10 years of HIV diagnosis (33.3 %). 65.0 % had CD4 + < 200 cells/ml and 87.0 % had detectable CV. The NECPAL instrument yielded 75 patients (61.0 %) with positive identification. The wasting syndrome (p > 0.006, OR 3.9 [95% CI 1.4-10.4]) and the presence of anemia (p > 0.001, OR 4.7 [95 % CI 1.9 -11.8]) were the variables that were related to the positive identification of NECPAL. Conclusions: The NECPAL instrument was useful in detecting palliative needs by medical assistance personnel who care for HIV/AIDS patients, and who do not have training in palliative care.","PeriodicalId":49831,"journal":{"name":"Medicina Paliativa","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"68139776","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: Narrative review using the Embase, CINAHL, and PubMed databases until November 2022. Results: 2,666 articles were retrieved and screened by title and abstract with included studies read in full for data extraction. Five main themes of the different approaches were identified in relation to access and use of palliative care: a) symptom management, b) adequacy and quality of care, c) appropriate palliative care, d) advance care planning and e) place of death. Older people, men, from a minority ethnic group, of low socioeconomic status and residing in rural areas are less likely to access to palliative care to relieve pain and suffering produced from cancer disease and at end of life. Conclusions: Palliative care is considered an essential component to provide comprehensive care throughout the cancer disease continuum. However, despite the existence of scientific evidence that recommends the integration of palliative care from the diagnosis of the disease, age, sex, ethnicity, socioeconomic level and residence create inequity in access and use of palliative care in patients with advanced cancer, producing groups of greater structural vulnerability.","PeriodicalId":49831,"journal":{"name":"Medicina Paliativa","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"68140816","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
