{"title":"Experiences of migrant parents of children with special health and welfare needs in Nordic countries: a scoping review.","authors":"Archlove Takunda Tanyanyiwa, Victor Chimhutu, Henning Pettersen","doi":"10.1177/14034948241277057","DOIUrl":"https://doi.org/10.1177/14034948241277057","url":null,"abstract":"<p><strong>Study aim: </strong>The objective of this scoping review was to explore and summarise emerging themes in existing studies on personal experiences of migrant parents of children with special health and welfare needs and their interaction with health and welfare services in the Nordic countries.</p><p><strong>Methods: </strong>A comprehensive and systematic search for relevant articles in electronic databases was conducted in PubMed, PsycINFO and Web of Science between 2 April 2023 and 31 May 2023. The scoping review followed identified guidelines in conducting scoping reviews. Out of the initial 1836 study results, 62 studies were read and assessed as full text and a total of 13 studies met the inclusion criteria.</p><p><strong>Results: </strong>Using thematic analysis, three key thematic categories were identified: (a) Initial responses to having a child with health and welfare challenges; (b) encountering the Nordic health and welfare services; (c) implications on social inclusion. The challenges to participation faced by migrants threatened their wellbeing, impacted their relationships and influenced their coping strategies and opportunities for social inclusion.</p><p><strong>Conclusions: </strong><b>The Nordic countries have accessible, affordable and well-equipped infrastructure for health and welfare services compared with the home countries of most migrants. They answered an anonymous questionnaire with the Cornell-Radimer hunger scale. Questions addressed hunger, concerns about food access, financial difficulties purchasing food, impact on children, patient demographics, children, and use of medications for chronic disease. We present logistic regression models with odds ratios (ORs) with 95% confidence intervals to examine associations between food insecurity and patient characteristics.</p><p><strong>Results: </strong>Among 2571 invited patients, 81.2% (<i>n</i>=2089) participated in the study. Of the participants, 40.1% were considered food insecure. The questions in the Cornell-Radimer hunger scale indicated that most had a mild degree of food insecurity. Food insecurity ranged from 28.9% among those >70 years old to 68.0% among those <30 years old. Food insecurity was associated with age <30 years, being migrant, inversely associated with higher educational levels (OR 0.60, 0.41-0.87) or having own children (OR 0.24, 0.18-0.31). Food insecurity was higher among participants using medications for chronic disease (OR 1.33, 1.05-1.68).</p><p><strong>Conclusions: </strong>\u0000 <b>This study underscores the presence of food insecurity in high-income countries like Norway, particularly among specific groups such as young adults, migrants and patients with chronic diseases. This scoping review aimed to identify frameworks, components, and operationalizations of social sustainability described in peer-reviewed articles and systematize the results from a health promotion perspective.</p><p><strong>Methods: </strong>Six databases were searched for relevant articles following the JBI methodology and PRISMA guidelines. Articles were included if they provided a unique framework for social sustainability, that is, a conceptual model outlining the essential components of this concept. Information concerning the components of social sustainability and their operationalizations was analyzed through a six-step narrative synthesis. The final step involved categorizing the operationalizations of social sustainability into distinct levels by employing a socio-ecological model as an analytical tool.</p><p><strong>Results: </strong>This review identified 22 articles presenting a social sustainability framework. The frameworks covered 11 components, of which social equity, safety, and neighborhood quality were the most frequent, while only five included health and well-being. The frameworks commonly provided practical interpretations of the concept with limited theoretical considerations. Furthermore, the identified operationalizations revealed a diverse understanding of social sustainability, encompassing all levels of the socio-ecological model applied.</p><p><strong>Conclusions: </strong>\u0000 <b>Health promotion research can offer theoretical and empirical insights that enhance the understanding of social sustainability, especially how societal, physical, and social determinants of health and well-being interact to create socially sustainable places. Sociodemographic risk factors can impact the workload of Child Health Services and affect the possibilities of providing the national child healthcare programme.</p><p><strong>Aims: </strong>This study aims to present the sociodemographic characteristics of families who are registered within the Child Health Services, as defined by the Child adjusted Care Need Index.</p><p><strong>Methods: </strong>By collecting personal identification numbers from children six years or younger registered at a child healthcare centre, and combining this with their caregiver's sociodemographic background, this study was able to create a sociodemographic index for each child healthcare centre in Sweden.</p><p><strong>Results: </strong>The study included 687,543 children and 1,335,540 caregivers from 981 child healthcare centres in Sweden. Approximately 21% of all children in the study population had a caregiver born in Southern or Eastern Europe outside the European Union, or in Africa, Asia, or South America, 7% had single parents, 17% had at least one unemployed caregiver, and 9% had at least one caregiver who had not completed high school. The average input values and the average index values varied widely both between and within the regions.</p><p><strong>Conclusions: </strong>\u0000 <b>This study displays a large variation in sociodemographics for child healthcare centres both within and between regions. They should be accessible, affordable, safe, equitable and culturally acceptable. Translating these general principles into Norwegian-specific dietary recommendations is essential, as foods beneficial for health tend to also be environmentally sustainable. Following the dietary recommendations is an important step towards sustainability.</p><p><strong>Aim: </strong>To identify challenges and potential solutions for transitioning towards more sustainable diets in Norway.</p><p><strong>Methods: </strong>We used scientific articles, reports, policy documents, and statistics on Norwegian food production and consumption to discuss a sustainable diet in a Norwegian context.</p><p><strong>Results and discussion: </strong>There is a large gap between dietary guidelines and actual consumption. More than 60% of the calories in the Norwegian diet are based on imported foods and feed. Changing people's diet is identified as central in transforming the food system to become more sustainable, as is prioritizing the use of local resources. Good animal health and welfare are also fundamental premises for a sustainable food system.</p><p><strong>Conclusions: </strong><i>Transitioning to a more sustainable diet requires comprehensive efforts at multiple levels. There is considerable room for action to increase the use of Norwegian resources in a sustainable and responsible way. Potential strategies include reducing meat intake in favour of plant-based foods and fish, consuming more local products, decreasing food waste and supporting agricultural practices that promote environmental and social sustainability. The objective of this study was to investigate the association between socioeconomic status and other risk factors with snus use and smoking among people in their late teens.</p><p><strong>Methods: </strong>Based on data from the longitudinal MyLife school survey, collected in five yearly waves between 2017 and 2021 (<i>N</i> = 1830, age 16.8-20.6 years in 2021, 64% girls), logistic regressions were performed with monthly snus use and smoking as dependents, and parents' education, psychological characteristics and earlier smoking or snus use as independent variables.</p><p><strong>Results: </strong>High socioeconomic status (parents' education) was associated with less smoking and snus use also after controlling for psychological characteristics and earlier smoking and snus use. Higher age increased the risk of snus use and reduced the risk of smoking.</p><p><strong>Conclusions: </strong>\u0000 <b>Youth snus use and smoking are associated with lower socioeconomic background. Denmark swiftly imposed strict governmental restrictions, while Sweden favoured a more gradual, voluntary approach. This study aims to analyse the disparities between the two nations in mental health indicators (depressive symptoms, sleep problems and loneliness), daily activities (shopping, going for a walk, visiting family and meeting other people) and medical care after the Covid-19 outbreak.Methods:Data from adults aged ⩾50 years in the Survey of Health, Ageing and Retirement in Europe (SHARE) wave 8 (2019/2020) and the two SHARE Covid-19 surveys (summer 2020/2021) were utilised. Multilevel logistic regression models assessed longitudinal and cross-sectional changes in Sweden and Denmark.Results:Both countries witnessed reduced risk of depressive symptoms and sleep problems during summer 2020, albeit with a more significant decline in depressive symptoms observed in Denmark: 17.8% (95% confidence interval (CI) 15.0–20.6) vs. 12.5% (95% CI 9.0–15.9). Swedish respondents were more likely to reduce their daily activities in the summer of 2020 (shopping: odds ratio (OR)=0.45, 95% CI 0.36–0.55; visit family: OR=0.76, 95% CI 0.60–0.97) and less likely to have medical appointments postponed (OR=1.83, 95% CI 1.46–2.28).Conclusions:Minimal differences were observed between Sweden and Denmark in mental health and daily activities during the pandemic. Persson","doi":"10.1177/14034948241272949","DOIUrl":"https://doi.org/10.1177/14034948241272949","url":null,"abstract":"Aims:This study aimed to investigate body function and daily life activities 18 months after Covid-19 infection, depending on the initial severity of disease and according to sex.Methods:All 11,955 individuals on sick leave due to Covid-19 during the first wave of the pandemic in Sweden were invited to answer a questionnaire regarding experiencing negative changes in body function and daily life activities approximately 18 months after the start of sick leave. The analysis of data included descriptive statistics, group comparisons and multivariable binary logistic regressions (two groups).Results:Of 5464 responders (45.7%), 4676 (85.6%) reported experiencing at least one problem with body function, and the reported prevalence of problems with daily life activities was 46%. The most frequently reported problems were fatigue (66.3%), cognition, sleep and movement. In general, women and those initially hospitalised reported more problems. In the regression analyses, problems with body function could partly explain whether individuals experienced problems with daily life activities. However, only fatigue and movement significantly contributed throughout all groups ( p&lt;0.001). Furthermore, the odds ratios for fatigue were larger in regressions for women than for men.Conclusions:In this nationwide study, more than 8 out of 10 individuals experienced problems with body function 18 months after being on sick leave due to Covid-19, with women and those initially hospitalised reporting more problems. Problems with body function, such as fatigue, could partly explain problems with daily life activities. Maternal birth country is known to influence the risk of placental dysfunction, but little is known about the possible mechanisms of this relation.Aims:(a) To estimate the proportion of non-registered prenatal ultrasound examinations; (b) to examine associations between non-registered ultrasound examinations and adverse perinatal outcomes, by migrant-related factors, in women giving birth in Norway.Methods:Individually linked data from the Medical Birth Registry of Norway and Statistics Norway, 1999–2016, comprising 999,760 singleton pregnancies to immigrants ( n=196,220) and non-immigrants ( n=803,540). Crude and adjusted odds ratios (aORs) with 95% confidence intervals (CIs) were estimated using logistic regression with robust standard error estimations, adjusted for year of childbirth, maternal age, parity, maternal smoking, educational level and Norwegian health region at birth.Results:Compared with non-immigrants, immigrant women had a higher proportion of non-registered ultrasound examinations (2.3% vs. 4.3%; aOR 2.0 (95% CI 1.9, 2.0)). Compared with women with ultrasound examination, the aOR for perinatal mortality for women with non-registered ultrasound was 2.27 (95% CI 1.85, 2.79) for immigrants and 3.61 (3.21, 4.07) for non-immigrants. Non-registered ultrasound examination was also associated with placental abruption (aOR 1.32 (1.08, 1.63)) for non-immigrant women, but it was not associated with preeclampsia.Compared with non-immigrants, immigrant women have a higher proportion of non-registered data on prenatal ultrasound examinations. Both immigrants and non-immigrants with non-registered ultrasound examinations have an increased aOR of perinatal mortality. Logistic regression was used to estimate the adjusted predicted probability of daily smoking and former smoking for persons in a formalised relationship (married or cohabitant) and persons not in a formalised relationship.</p><p><strong>Results: </strong>Since the 1970s, the decline in daily smoking has been present for men regardless of relationship status, but with more smoking among men not in a relationship from the 2000s onward. For women not in a relationship, daily smoking increased among those not in a relationship from the mid-1980s, surpassing the smoking rate among men in a relationship. From the 1990s, daily smoking was higher in women not in a relationship compared with women in a relationship. In the past decade (2013-2022), the adjusted odds ratio of daily smoking was 0.49 for people in a relationship compared with those not in a relationship.</p><p><strong>Conclusions: </strong>\u0000 <b>Downward trends in daily smoking have been substantial the past 50 years, but differences by relationship status persist.
