Treatment options range from conservative management to surgery, but patients may not be able to tolerate symptoms with the former and the latter procedure may be associated with significant morbidity We describe the case of using a stent-assisted coil approach to treating a patient with jugular bulb diverticulum (JBD) and severe pulsatile tinnitus. We describe a 41-year-old woman who presented with disabling pulsatile tinnitus due to right JBD. Both surgical or endovascular approaches were possible, but we decided to treat the diverticulum with coil emboli-zation and stenting of the jugular bulb. Following the procedure, tinnitus presence and severity were self-reported or assessed using the tinnitus handicap inventory (THI) and tinnitus magnitude index (TMI). The total score of both THI and TMI improved in 6 months after surgery, and there was no complication. Kim","doi":"10.55911/jksbs.23.0004","DOIUrl":"https://doi.org/10.55911/jksbs.23.0004","url":null,"abstract":"A giant pituitary adenoma remains a major challenge for neurosurgeons, despite advances in en-doscopic endonasal surgery. These tumors can grow in different directions, which can affect the surrounding neurovascular structures. When the tumor grows towards the suprasellar region, it can affect important structures such as the optic nerves, anterior cerebral arteries, and hypothalamus. When it occupies the interpeduncular and prepontine cisterns, the oculomotor nerves or posterior communicating arteries may be at risk. Postoperative hemorrhage due to incomplete resection is also a concern and can lead to the need for revisional surgery or another surgical approach. Patients with giant pituitary adenomas typically have longer hospital stays due to the high rates of postoperative complications, such as cerebrospinal fluid leakage, diabetes insipidus, and infection. Gross total re-section rates are also lower for these types of tumors. Radiological find-ing on computed tomography (CT) or magnetic resonance image (MRI) may be confused with bony lesions of paranasal sinuses or invasive tumors of the skull base. A 43-year-old woman visited our hospital with headache, and an MRI scan performed at other hospital showed abnormal findings in the right sphenoid sinus. When the additional CT images were evaluated, it was diagnosed as an arrested pneumatization of the sphenoid sinus. A 46-year-old woman visited the neurologist with diplopia and had a brain MRI. Due to abnormal findings in the left sphenoid sinus, the patient was transferred to the otorhinolaryngology department. The left sphenoid sinus lesion was confirmed as an arrested pneumatization on CT and MRI. Both patients decided to follow-up without any special treatment. The annual incidence rate is reported as one case per 250,000- 500,000 population. Patients present with various non-specific symptoms including headache, fever, decreased consciousness and many other focal neurologic deficits, which often lead to difficult and delayed diagnosis. In the absence of proper management, prognosis is very poor with a mortality rate of about 70%. Magnetic resonance imaging (MRI) revealed a homogeneous enhancing mass on the left frontal lobe. Based on the impression of a convexity meningioma, surgical treatment was planned, and intravenous (IV) steroids were administered to alleviate brain edema. Additional brain MRI was performed just before surgery for intraoperative navigation, which showed a significant reduction in tumor size. Due to the good response to IV steroids, we suspected that the tumor might be a differ-ent lesion rather than a meningioma, and thus, we proceeded with surgery for histologic diagnosis. Intraoperative findings revealed a whitish-gray, rubbery tumor adhering to the dura and arachnoid membrane, which was removed, and intraoperative frozen biopsy reported lymphoma. Finally, the patient was diagnosed with diffuse large B-cell lymphoma.
