Bone and joint TB are the most common types of extra-pulmonary TB, but can easily be misdiagnosed, since articular TB commonly mimics other joint diseases, including reactive and degenerative arthritis, and articular manifestations associated with systemic autoimmune diseases. Furthermore, patients with systemic auto-immune diseases are often more prone to mycobacterial infection. Scrub typhus, caused by Orientia tsutsugamushi, typically manifests with eschars that can be easily overlooked due to their painless nature. This case report describes a 35-year-old male gardener who presented with confusion, hallucinations, dry mouth, and dilated pupils after consuming parts of the plant. The patient was treated with rehydration, activated charcoal, and physostigmine, leading to rapid clinical improvement. This case highlights the importance of recognizing plant-based poisonings and the efficacy of physostigmine in managing anticholinergic syndrome. The primary composite outcome of mortality or bronchopulmonary dysplasia (BPD) at 36 weeks' post-menstrual age was 41% in LISA, 38% in INSURE and 77% in ETT groups (<i>p</i> < 0.01), the difference being mainly due to the high incidence of mortality in the latter. Such infections are uncommon but cause high mortality when the bacterium enters the bloodstream through an open wound. We present a case of <i>C. violaceum</i> infection isolated from a blood culture sample in a neonate identified at a tertiary care teaching hospital in North East Uttar Pradesh. Timely diagnosis by microbiological methods and antimicrobial treatment led to a successful cure for this life-threatening infection. Data on treatment of IgA vasculitis associated with HIV is sparse and limited to case reports. Highly active antiretroviral therapy (HAART) is the mainstay therapy for inducing remission. We report a case of a 41-year old HIV patient who presented with palpable purpura, melaena and oedema and was diagnosed with IgA vasculitis with severe renal (crescentic glomerulonephritis), gastrointestinal and skin involvement. He was treated with immunosuppressive drugs and HAART which led to remission of disease. Bleeding as a result of warfarin overdosage is common and risk factors seem to vary between patient populations. Predictors of bleeding in patients using warfarin were studied using a case-control design. We calculated the Hypertension, Abnormal Renal/Liver Function, Stroke, Bleeding History or Predisposition, Labile INR, Elderly, Drugs/Alcohol (HAS-BLED) score (a bleeding risk prediction tool). Cases chosen were adults prescribed warfarin presenting with bleeding and controls were likewise taking warfarin but had no bleeding in the previous six months. The most common indication for warfarin use was venous thromboembolism. Recent decrease in food intake was strongly associated with bleeding. Older age, higher warfarin doses, recent illness in the past fortnight and prior bleeding were also independent predictors. A higher HAS-BLED score was not predictive independently.
