Da realistische Therapieerwartungen substanziell zum Therapieergebnis beitragen, ist eine frühe erwartungsfokussierte Intervention bei dieser Patientengruppe indiziert. Kurze Videointerventionen mit Patienten als Rollenmodelle zeigten sich bereits als sinnvolle Methode zur Veränderung von Therapieerwartungen. Patienten und Methoden: Es wurden insgesamt 92 Patienten mit einer Alkoholabhängigkeit in zwei Gruppen randomisiert: (1) eine Kontrollgruppe, die ein Video mit Patienten sah, die lediglich Informationen zu ihren Symptomen gaben, oder (2) in eine Experimentalgruppe, die ein Video mit den gleichen Patienten sah, die zusätzlich Informationen zu dem vorwiegend positiven Ausgang einer Psychotherapie gaben. Ergebnisse: Es zeigte sich kein differentieller Effekt zwischen den Gruppen, jedoch konnte eine signifikante Verbesserung der Therapieerwartung und Einstellung gegenüber Psychotherapie nach Betrachten beider Videos festgestellt werden. Nach einem Pooling der Gruppen ließen sich als Moderatoren die Selbstwirksamkeit, die Symptomschwere und die Krankheitsdauer identifizieren. Schlussfolgerung: Eine kurze Videointervention mit Patienten als Rollenmodelle verbessert Therapieerwartungen und Einstellung gegenüber Psychotherapie von Patienten mit einer Alkoholabhängigkeit, insbesondere wenn diese eine kurze Erkrankungsdauer, geringe Selbstwirksamkeit und schwere Symptomatik vorweisen. Frühe Interventionen bei therapieunerfahrenen Patienten scheinen indiziert. One possible factor for therapist effects seems to be the therapists’ interpersonal skills. Therapist assessments of interpersonal skills have demonstrated sufficient predictive validity, but little is known about underlying skills and related constructs. The purpose of the present study is to compare the different constructs of interpersonal skills with each other and to investigate possible underlying abilities. Study Population and Methods: In this exploratory study, the concept of interpersonal skills was investigated using three survey methods (questionnaires, computer tests, and assessments) and comparing two measurements each. For this purpose, 6 measures of interpersonal skills were collected from 20 prospective psychotherapists as well as 28 psychology students (total N = 48). Alongside more traditional methods, the correlations and overlaps of the constructs were examined using partial and Bayesian networks. Results: Moderate to high correlations are found within the self-report measures. The assessments, however, did not correlate with self-report measures nor with computer tests. Discussion and Conclusion: The similarities in the self-assessments indicate stable self-concepts of interpersonal skills, which, however, cannot be explained by the underlying skills nor by the observer-based evaluations. However, to date there is no systematic review on the efficacy of STAIR as initial treatment phase and STAIR/NT. <b><i>Methods:</i></b> Searching various databases, we conducted a systematic review on studies with and without control condition investigating the efficacy of STAIR, STAIR/NT, and their adaptations. <b><i>Results:</i></b> Overall, <i>k</i> = 10 studies (including 5 studies with control condition) on STAIR/-adaptations and <i>k</i> = 5 studies (including 4 studies with control condition) on STAIR/NT/-adaptations were identified. In 7 studies on STAIR/- and 4 studies on STAIR/NT/-adaptations, samples consisted of at-risk groups for complex traumatization. Both STAIR/- and STAIR/NT/-adaptations revealed positive effects on posttraumatic stress, affective dysregulation, and interpersonal problems. When compared to passive and active control conditions, STAIR/- and STAIR/NT/-adaptations were equally or more effective. <b><i>Discussion:</i></b> Findings on the efficacy of STAIR/- and STAIR/NT/-adaptations are mixed. Evidence is limited by the small number of studies, heterogeneous adaptations, often uncontrolled study designs, subclinical samples, and small sample sizes. Befunde aus der Grundlagenforschung zum inhibitorischen Lernen und deren klinische Anwendungen legen nahe, dass die Wirksamkeit von Verhaltensaktivierung zusätzlich gesteigert werden könnte, wenn bei der Durchführung ein besonderer Fokus auf das Erleben von Erwartungsverletzungen gelegt wird. So wird in diesem Artikel ein Behandlungsansatz vorgestellt, der die Verhaltensaktivierung um den Aspekt der Erwartungsverletzung ergänzt. Zentral ist dabei die Erfahrung, dass das Durchführen von geplanten Aktivitäten für Depressionspatient:innen meist weniger anstrengend als erwartet ist und mehr Freude als erwartet macht. In der Nachbesprechung wird zudem die nachhaltige Verarbeitung einer solchen erwartungsverletzenden Erfahrung besonders gefördert, indem einer nachträglichen Entwertung der Erfahrung durch die Patient:innen entgegengewirkt wird. So lernen die Patient:innen, dass sie aufgrund der Depression dazu neigen, Aktivitäten als unverhältnismäßig aversiv anzunehmen, sich diese Erwartung jedoch in aller Regel nicht bestätigt. Dadurch trauen sie sich in der Folge mehr zu und können zukünftige Aktivitäten optimistischer angehen. In einer kleinen Machbarkeitsstudie bei teilstationär behandelten Depressionspatient:innen (<i>N</i> = 7) zeigte sich, dass der hier vorgestellte Behandlungsansatz gut umsetzbar ist und von den Patient:innen gut angenommen wird. Diese Arbeit orientiert sich dabei an der Faktorenstruktur der englischsprachigen Fassung des SASII. <b><i>Ergebnisse und Schlussfolgerungen:</i></b> Die konfirmatorische Faktorenanalyse zeigte eine gute Modellpassung, während die interne Konsistenz als gering einzustufen ist. Es zeigten sich wenige positive Zusammenhänge zu Maßen aktueller Theoriemodelle suizidalen Erlebens und Verhaltens. Angesichts der in dieser Studie ermittelten geringen Reliabilität der untersuchten Faktorenstruktur wird von einer Verwendung dieser in Forschungsarbeiten abgeraten. Niemeyer","doi":"10.1159/000526594","DOIUrl":"https://doi.org/10.1159/000526594","url":null,"abstract":"The number and type of traumatic experiences (especially childhood maltreatment) appear to increase the risk of developing complex trauma symptoms. There is initial evidence that patients with complex trauma benefit from phase-based therapeutic approaches. STAIR-NT (Skills Training in Affective and Interpersonal Regulation with Narrative Therapy) is a two-phase trauma therapy approach: emotion regulation and interpersonal skills are learned in the STAIR phase, and in the narrative therapy phase (NT) the exposure to trauma aims at recognizing and changing dysfunctional, trauma-associated assumptions. Experiences with STAIR-NT in the context of a current treatment study show that the two phases provide a clear structure and more security for the treatment. First clinical experiences with STAIR-NT as well as short case vignettes are presented and recommendations for the treatment of patients with complex trauma are given. Concrete practical recommendations are offered for the implementation and individualization on interventions, which among others relate to offender contact, motivational work, handling dissociative states, therapeutic methods (especially exposure), and therapy completion. Children with ADHD require more frequent and explicit behavioral cues from others and thereby increase parental stress. Both self-control deficits and parental stress manifest depending on the situation and possess intra-individual variability. Common measures of externalizing behavior and parental stress rarely address this and target generalized impressions instead. Intraindividual differences in externalizing and stressful child behavior can be assessed with a family situations inventory including two rating scales. It rests on the HSQ and supplements the original with a parental stress scale. Method: Parents of children with (male = 204, female = 33; mean = 8.6 years) and without confirmed ADHD (male = 206, female = 155; mean = 7.6 years) took part. They were asked to rate 16 family situations concerning the extent of child externalizing behavior and their own associated stress. Three established scales (CBCL, PSI, BAP) were employed in the clinical sample to determine the external validity of the German HSQ adaption. Results: Both HSQ scales share the same two-factorial structure with satisfying explained variance (56.02–60.38%) and acceptable external and discriminant validity. “Accomplishments” (factor 2) discriminates better than “agreeableness” (factor 1) between children with and without ADHD. Discussion: “Accomplishments” refers to behavior in regularly recurring situations and demands rule following.
