According to the most recent National Cancer Register, 2283 men and 1483 women were diag nosed with renal cancer in 2006. Up to 30% of patients with RCC present with metastatic disease. M-TOR inhibitors became a new therapeutic option for patients with metastatic RCC. Two of them, temsirolimus and everolimus, are currently approved for clinical use for pa tients with advanced renal cancer. An ticancer activity of m-TOR inhibitors is re lated to cellular cycle regulation and inhibition of uncontrolled angiogenesis. Based on clinical trials, temsirolimus is indicated as the first line of chemother apy for patients with at least three poor prognostic factors. They are mostly observed in children. In adults they are more common in men in the second or third decade of life. Pineal tu mours have a diverse histopathology. Their growth is slow and the symptoms result from the compression of adjacent cerebral structures. One of the histopathological forms is germinal tumours, including mild or malignant teratoma. They have the ability to release neoplastic markers, e.g. AFP, β-HCG, which are used to evaluate the effectiveness of surgical and oncological treatment. The paper presents a case of a 24-year-old man di agnosed as teratocarcinoma. The treat ment protocol consisted of both surgery and conventional fractionation radio therapy (Df = 1.8 Gy, 5 ◊ weekly) to the total dose of 50.4 Gy. After a few months, there was a recurrence of the cancer process. The patient received 2 courses of chemotherapy, which did not yield the expected therapeutic effect. The patient’s prognosis is unfavourable due to rapid local relapse and failure after treatment. This is due to the shortage of haematology specialists, their uneven distribution and lack of clear rules regarding patient referrals to outpatient haematology clinics (OHCs). Complete blood count (CBC) is a cheap and widely performed laboratory test. However, in view of problems frequently encountered by primary care physicians with proper interpretation of CBC results, particularly with the differential diagnosis of malignant blood diseases from benign or reactive abnormalities, we hypothesized that the main reason for referring patients to OHCs might be any CBC deviation. To test this hypothesis, we analysed the structure of patient referrals to the OHC in Elbląg. Additionally, we sought to establish guidelines for primary care physicians to follow while referring patients identified with CBC abnormalities for haematology assessment. The study analyses the reasons for 986 patient referrals to the OHC in Elblag (in 2005-2009). Referrals due to CBC abnormalities were also evaluated to determine their severity. Most of the patients (690) referred to the OHC were female (70%). The most common reason for OHC referral, identified in 33% of patients with CBC abnormalities, was anaemia, followed by thrombocytopenia (25%) and leucopenia (16%). The severity of CBC abnormalities was mild in the majority of patients. At the time of their first OHC visit, only 60% of anaemia and thrombocytopenia cases in fact met the criteria for the respective diagnosis, which indicates that the abnormalities resolved spontaneously and could have been monitored by the primary care physician. Based on the above results and own experience, the authors have proposed practical guidelines on referral indications in the case of CBC abnormalities. {"title":"Assessment of intraoperative and postoperative complications during implementation of laparoscopic radical prostatectomy in patients with locally advanced prostate cancer","authors":"P. Material and methods: Within less than two years, i.e. from November 2008 to August 2010, 30 laparoscopic radical prostatectomies were performed at the Department of Urology and Uro-Oncology in Poznań. The operations were performed with the retroperitoneal approach. Lymphadenectomy was performed only in the patients with a preoperative prostate specific antigen (PSA) level greater than 10 ng/ml and/or Gleason score 7 or greater. Results: The operating time ranged from 150 to 360 minutes (mean operating time was 234 minutes). Two cases required conversion to open surgery because of highly increased prostate volume (100 ml) and due to difficult operating conditions. During one of the very first procedures rectal injury occurred and the patient developed a rectourethral fistula two weeks after surgery. Six patients had a prolonged drain leakage (> 7 days) caused by dehiscence of the vesicourethral anastomosis. One patient developed a large abdominal wall haematoma in the right and left lumbar region which was successfully treated with conservative management. One patient presented haematuria which stopped spontaneously on the 10th postoperative day. On the first post-operative days 5 patients developed a fever which lasted a few days. Conclusions: An increasing number of urology clinics and hospitals perform laparoscopic radical prostatectomy. This ablative and reconstructive surgery is a technically challenging procedure and can be successfully performed by a urological team experienced in laparoscopy and radical prostatectomy. The procedure requires consistent improvement of the operative technique in order to provide patients with the most satisfactory oncological and functional results. About 1200 new morbidity is registered in the USA annually. Langerhans cell histiocytosis has a wide range of signs and differential prognosis. Bones are the most common localization of disease (40-78%). Nowadays histiocytosis hasn’t got clinical trials and standard of treatment. Surgery is the treatment of choice in limited form of histiocytosis, but in diffuse form the chemotherapy is always used. The role of radiation therapy in histiocytosis is unknown. The most of researchers recommends the radiotherapy in following conditions: reccurence or progresion of disease, spinal cord compression, bone pain or bone’s break threat, exophthalmus, non-surgical local changes. Several endocrinopathies were found to be associated with these tu mours, with ectopic ACTH secretion re ported most frequently. Here we provide a description of the syndrome of inap propriate antidiuretic hormone secretion (SIADH) accompanying neuroendocrine thymic carcinoma. A mediastinal tumour was incidentally found on routine chest X-ray examina tion in a 55-year-old man. After detailed imaging studies, tumour excision was performed. The mediastinal tumour (11 ◊ 8 ◊ 4 cm) presented features of in vasion to the left lung and pericardium. Histopathological examination con firmed the diagnosis of thymic neu roendocrine carcinoma. The patient was treated with chemotherapy because of the recurrence. Symptoms of carcinoid syndrome and positive result of the so matostatin receptor scintigraphy justi fied subsequent introduction of so matostatin analogue therapy. At this point low serum sodium levels accom panied by decreased serum osmolarity and inadequate urine concentration were found. Having excluded other typ ical causes, SIADH syndrome due to ec topic vasopressin synthesis was as sumed. BRCA1 and BRCA2 are highpenetrant genes, but still their functions and mechanisms leading to cancer caused by their mutations remain un known. Germline alterations in BRCA1 and BRCA2 sequences are related to hereditary breast cancer syndrome (HBC), hereditary breast and ovarian can cer syndrome (HBOC), hereditary ovar ian cancer syndrome (HOC). Primary and secondary tumors prevention and choice of the best treatment are the benefits of the detection of BRCA2 mutations. Re search on correlation between the tumor phenotype and the type of mutation is continuously performed. The association of trace elements and high cancer risk has been found in many studies. The present study was performed to investigate the relationship between the concentration of magnesium, copper and chrome and the incidence and prognosis of oesophageal squamous cell carcinoma. Material and methods: The study was conducted on 36 patients with oesophageal squamous cell carcinoma and all patients underwent oesophagectomy. The cancerous tissue samples and the normal tissue samples came from the patients. The concentration of magnesium, copper and chrome was measured by flame atomic absorption spectroscopy and comparison was made using Student’s t-test. Results: The content of magnesium in cancerous tissues was significantly lower than that in the normal tissues. In contrast, copper level in cancerous tissues was significantly higher than that in the normal tissues. Copper level in patients with lymph node metastasis was significantly higher than in patients without lymph node metastasis. There was a significant increase in mean copper in cancerous tissues with a worse histological grade. Our study showed that a lower magnesium level and a higher copper level may play an important role in oesophageal squamous cell carcinoma induction.
