The ATOZ™ orthodontic distractor was invented in 2017 in Korea with the primary directive of growing the maxillary apical base without involving teeth or general anesthesia-required surgery in its application. Its 8.2 mm transverse footprint is ideal for patients who display craniofacial dysmorphism with a severely constricted maxillary apical base. ATOZ™ can withstand up to 50 kg-force of compression without permanent deformation and reverts to its original shape effortlessly. This unique property allows ATOZ™ to apply heavy orthopedic force to the median palatine suture and circummaxillary sutures in a continuous manner over a long period of time. Three exemplary clinical cases treated with ATOZ™ are presented in this article with an emphasis on its slow activation protocol, different from the conventional rapid activation protocol for Rapid Palatal Expander (RPE) or Mini-implant Assisted RPE. The purpose of the study is to compare the quality and quantity of expansion of two different Haas devices with anchorage on deciduous elements, one custom-made (HIRME), through a three-dimensional analysis of the changes in the palatine vault after expansion. The study retrospectively examined a total of 30 patients, equally divided into two groups, and showed an average increase in palatal surface area of 28.81 % for the traditional Haas expander and 24.38 % for the HIRME device. The increase in surface and volume was significant for both devices (<em>p</em> &lt; 0.001), while no significant differences in surface or volume increase were found between the two devices (Haas: <em>p</em> = 0.365; Hirme: 0.354). Bone Anchored Maxillary Protraction (BAMP) aims to correct the maxillomandibular deficiency for skeletal Class III patients in the middle or late stages of growth. The combination of mini-screw-assisted maxillary skeletal expansion (MARPE) with BAMP is the efficient solution for skeletal transverse and sagittal deficiencies, which aims to maximize skeletal effects while minimizing dental effects. This article presents a novel digital workflow for orthodontic treatment, focusing on mini-screw-assisted maxillary skeletal protraction following transverse expansion. The digital clinic workflow encompasses data acquisition, virtual design and planning, and 3D printing, which minimized the hands-on time of the dental laboratory as much as possible to improve efficiency. Additionally, the article provides a detailed description of the digital methodology for creating custom auxiliaries for maxillary protraction, including types and protocols of 3D printing auxiliaries. A case review demonstrates the application of the digital workflow in treating skeletal </span>Class III malocclusion. Radiological analysis reveals significant improvements in skeletal relationships, with successful maxillary expansion and protraction using the digital method. Digital-assisted </span>orthodontics processes described in this article enhanced the usage of </span>MARPE and BAMP treatment, aiming to enhance efficiency, accuracy, and patient comfort. Cone-beam CT scans were analyzed at baseline (T0) and after expansion (T1), comparing skeletal measurements between groups categorized by “total” and “clinical success (&gt;1 mm expansion)”. Several factors, including age, sex, and midpalatal suture maturation stage, were assessed for their impact on expansion success and patterns. The ATOZ expander achieved a 92.8 % success rate, and all skeletal measurements differed significantly post-expansion. Younger age and less mature sutures were associated with greater expansion success, while sex did not significantly influence “clinical success”. Younger age predicted greater expansion, and midpalatal suture maturation influenced the outcomes of maxillary expansion. We aimed to identify their distinct expansion patterns and potential clinical implications.</div></div><div><h3>Materials and Methods</h3><div>A total of 60 subjects, comprising 14 males and 46 females with an average age of 18.5 ± 7.36 years, were divided into two groups: the tissue-bone-borne expander group (CEXP group, <em>N</em> = 30, average age 19.16 ± 7.09 years) and the pure-bone-borne expander group (ATOZ group, <em>N</em> = 30, average age 17.83 ± 7.56 years). CBCT scans were obtained before (T1) and after expansion (T2). Zygomatic, palatal, and dentoalveolar dimensions were measured and analyzed using paired T-tests and Kruskal-Wallis tests.</div></div><div><h3>Results</h3><div>The CEXP group demonstrated significantly greater dental expansion and an increase in dentoalveolar inclination. In contrast, the ATOZ group exhibited significantly less zygomatic expansion and a decrease in dentoalveolar inclination. In horizontal plane, the CEXP group showed more parallel expansion pattern than the ATOZ group. Both groups showed similar amount of palatal expansion.</div></div><div><h3>Conclusion</h3><div>The tissue-bone-borne C-expander demonstrated more pronounced dentoalveolar effects with a vertically pyramidal and sagittally parallel pattern of expansion. In addition, it allows a reduction in maxillary anterior crowding when applied exactly during the eruption phase of the upper lateral incisors. This report presents a novel approach to treating transverse maxillary deficiency through purely bone-borne RPE using bracket plates. The method involves strategically placing four mini-implants in the palate<span> before inserting the RPE screw. Subsequently, right and left bracket plates are fixed over the mini-implant heads to connect two anteroposterior mini-implants. The RPE screw is then attached to the bracket plates, securing the RPE connecting arms into 0.032 × 0.040-inch slot brackets on the bracket plates. The primary advantage of this method is that the RPE screw is replaceable and adjustable. This enhances the freedom in selecting mini-implant placement sites, ensuring better skeletal anchorage on the palatal bone compared to tooth-bone-borne RPE, where mini-implant placement sites are limited to the periphery of the RPE screw.
