Their service volumes must be redistributed among the remaining providers. This leads to a regional increase in the number of cases, which can also lift those providers that are below the minimum volume above the threshold. In a simulation study, we illustrate the relationship between minimum volumes and patient migration. Before the introduction of the minimum volume of 30 cases, there are 745 providers in the simulation, 510 of which are below and 235 above the minimum volume. Patient migration leads to a further 79 providers being raised above the required case number threshold. These include providers with only 12 cases initially. From this it can be concluded that, in addition to randomised controlled trials (RCTs), healthcare-related data can also be used. Questions relating to particularly rare diseases, which increasingly include ever smaller subgroups of common diseases (individualised therapies, e.g. based on certain genetic characteristics), as well as increasingly important endpoints such as Patient-Reported Outcome Measures (PROMs) and Patient-Reported Experience Measures (PREMs) cannot be clarified in an RCT. Instead of a specific study design, the focus should be on the quality of studies. It is therefore by no means a question of weakening requirements, but rather of consistently increasing the quality of studies in which new data sources can also be included. Based on the comparison of the single-arm evidence for the CAR-T Ciltacel with data from the LocoMMotion study, which was recognised in the medical community as the best prospective study on the standard of care for multiple myeloma with multiple prior treatments, the application of the concept in the AMNOG is presented. The comparison and the dramatic survival benefit shown with Ciltacel, the validity and clinical significance of which is confirmed by medical experts, contrasts with the complete rejection by the G-BA.
