A systematic review of the literature published from January 2000 to December 2022 was performed. Selection criteria included published randomized controlled trials, prospective reviews and retrospective reviews of all human subjects undergoing hip preservation surgery. Exclusion criteria included case reports, animal studies and studies not reporting perioperative pain control protocols. Thirty-four studies included hip arthroscopy in which peripheral nerve blocks were associated with a significant reduction in pain score (P = 0.037) compared with general anesthesia alone. However, no pain control modality was associated with a significant difference in postanesthesia care unit opioid use (P = 0.127) or length of stay (P = 0.251) compared with general anesthesia alone. Falls were the most common complication reported, accounting for 37% of all complications. Five studies included periacetabular osteotomy and surgical hip dislocation in which peripheral nerve blocks were associated with an 18% reduction in pain on postoperative Day 2, a 48% reduction in cumulative opioid use on postoperative Day 2 and a 40% reduction in hospital stay. Due to the low sample size of the periacetabular osteotomy and surgical hip dislocation studies, we were unable to determine the significant difference between the means. This study describes the results of a consecutive series of patients treated with endoscopic IP tenotomy as a less invasive alternative. Twenty-eight patients with IP impingement after THA underwent endoscopic IP lengthening from 2012 to 2021 at a single-center academic institution. The follow-up of 24 of these patients was achieved with a mean follow-up of 7.6 months (range 1-28). Outcomes included the modified Harris Hip Score (mHHS), visual analog pain scale (VAS), satisfaction, component positioning and complications. Seventy-one percent of patients were satisfied or very satisfied after their operation. The median mHHS preoperatively was 57 (Interquartile range [IQR] 43-60) and postoperatively was 75 (IQR 66-92, P < 0.001). Clinically meaningful improvements in mHHS were seen in patients with VAS pain scores <5, cup prominence >8 mm, body mass index >30, and less than 2 years from their index THA. Two patients developed a deep infection 7 and 10 months postoperatively (neither related to the release), and one patient underwent open psoas release for persistent impingement. Endoscopic IP tenotomy is a safe and effective treatment for impingement after THA. The study hypothesis was that modified Dunn radiographs could be an alternative method to standing AP pelvis images in the evaluation of pincer-type femoroacetabular impingement (FAI). The study included 31 patients with a mean age of 42 years (range: 29-55 years) who had modified Dunn and standing AP pelvis radiographs taken in the radiological evaluation of non-traumatic hip pain between July 2018 and September 2022. Similar tilt ratio values were obtained on standing AP and modified Dunn images (P = 0.986). Joint space width showed a decrease in the transition from the modified Dunn position to the standing position, but the difference was not statistically significant (P = 0.161). The mean LCEA was measured as 34° ± 4° in the standing position and as 35° ± 5° in the modified Dunn position. No statistically significant difference was determined in the number of patients with a crossover sign or the crossover percentage values in these patients (P > 0.05). The data obtained in this study showed that there was no significant difference between images taken in the standing and modified Dunn positions in respect of radiological findings of pelvic tilt and pincer-type FAI. As a parameter of anterior hip coverage, the anterior center-edge angle on false-profile radiograph (ACEA FP) is associated with clinical outcomes. With the widespread application of computed tomography (CT), the anterior center-edge angle on CT (ACEA CT) has also been used to measure anterior hip coverage. Little is known about the reproducibility of the ACEA FP and ACEA CT in patients with hip dysplasia or the correlation between the ACEA CT and ACEA FP. In total, 49 hips of 49 patients who underwent periacetabular osteotomy in our center were included. The lateral center-edge angle, Tönnis angle, ACEA FP and ACEA CT were determined. We assessed the intraobserver and interobserver reliability of the ACEA FP and ACEA CT, the effect of the Tönnis angle on the reliability of the ACEA FP and ACEA CT and the correlation between the ACEA CT and ACEA FP. The intraobserver and interobserver interclass correlation coefficients of the ACEA FP were good, and those of the ACEA CT were very good. The Tönnis angle was weakly correlated with inconsistent ACEA FP measurements (P = 0.008) but not with inconsistent ACEA CT measurements (P = 0.600). No correlation between ACEA FP and ACEA CT measurements was observed (P = 0.213-0.665). The reproducibility of the ACEA CT is more consistent than that of the ACEA FP. The oblique acetabular roof had an effect on determining the ACEA FP but not on determining the ACEA CT. This retrospective review involved 98 hips of 98 consecutive patients with symptomatic acetabular dysplasia who underwent CPO from March 2016 to June 2020. The clinical outcomes were evaluated based on the Japanese Orthopaedic Association Hip Disease Evaluation Questionnaire, the Medical Outcomes Study 36-Item Short-Form Health Survey and the Harris Hip Score. Patients were divided into satisfied and unsatisfied groups according to the 85th percentile cut-off on the postoperative visual analog scale (VAS) for dissatisfaction. A multiple logistic regression analysis was performed to investigate the factors impacting patient dissatisfaction after CPO; the explanatory variables were age, body mass index, postoperative Tönnis grade of ≥2, postoperative VAS score for pain, lateral femoral cutaneous nerve (LFCN) injury and radiographic complications (non-union of the pubis, non-union of the ischium after posterior column fracture and ischial ramus stress fracture). The 85th percentile of the postoperative VAS score for dissatisfaction was 60 mm. The unsatisfied group comprised 15 patients at 1 year after CPO. The multiple logistic regression analysis results showed that the postoperative VAS scores for pain [odds ratio (OR), 1.064; 95% confidence interval (CI), 1.026-1.104; P = 0.001] and LFCN injury (OR, 6.775; 95% CI, 1.308-33.256; P = 0.018) were associated with postoperative dissatisfaction. For irreparable labral pathology, labral reconstruction is an increasingly adopted technique shown to decrease hip pain and improve function. We evaluated survivorship and clinical outcomes of allograft labral reconstruction using the mini-open anterior surgical approach. Twelve patients who underwent labral reconstruction using a semitendinosus tendon allograft (reconstruction group) were matched 1:3 based on age, gender, body mass index, year of surgery, preoperative Tönnis grade, previous hip surgery, residual hip pathology and extent of acetabular chondral lesion to a control group of 36 patients who underwent direct labral repair with anchors (refixation group). At a minimum follow-up of 2 years, patient-reported outcomes, radiological findings and failure rates were compared. The average age was 31.3 years (±13.6) for reconstruction and 34.7 (±10.2) for refixation. Both groups had similar preoperative symptomatic periods (P = 0.3), prevalence of residual hip pathology (P = 1.0) and prevalence of prior hip surgeries (P = 1.0). both groups had a significant improvement of modified Harris Hip scores and 36-Item Short-Form Health Survey physical scores. There was no statistically significant difference in conversion rates to total hip arthroplasty (25% versus 8.3%, P = 0.2); however, time to conversion was significantly longer in the reconstruction group (3.58 years ±1.04) compared to the refixation group (1.20 years± 0.93; P = 0.04). However, no bibliometric studies regarding the global research trend concerning SHD have been studied yet. This study aims to determine the research status in the field of SHD research between 2001 and 2021. The publications related to SHD from 2001 to 2021 were retrieved from the Web of Science Core Collection. Three bibliometric tools were used for this study. The main analyses include publication counts, contributions of countries, institutions, authors, journals and funding agencies, as well as analyses on clustering of references and keywords. In total, 498 articles were identified. The annual publication counts of SHD showed an ascending tendency as a whole. The United States has the most prominent contributions, with the most number of publications and the highest value of H-index. The University of Bern was the organization that produced the most literature. Professors Ganz R, Siebenrock KA, Tannast M, Steppacher SD and Leunig M were the core authors in this field. The most productive journal was Clinical Orthopaedics and Related Research. Burst keyword detection suggested that the following research directions, including 'surgical hip dislocation', 'outcome', 'fixation' and 'pain', are considered the research hotspots and deserve more attention.
