{"title":"A Cartilage-Saving Technique for Tragus and Antitragus Fabrication in Auricular Reconstruction.","authors":"Chenglong Wang, Dejin Gao, Pan Luo, Yue Wang, Qingguo Zhang","doi":"10.1089/fpsam.2024.0154","DOIUrl":"10.1089/fpsam.2024.0154","url":null,"abstract":"<p><p><b>Background:</b> For auricular reconstruction surgery using autologous costal cartilage, younger patients typically have a limited amount of costal cartilage available. <b>Objective:</b> We introduce a cartilage-saving technique for fabricating the tragus and antitragus, and evaluate its effectiveness based on aesthetic score and complications. <b>Methods:</b> For tragus fabrication, the residual part of the seventh costal cartilage was reoriented by 90° to increase its height and carved into a seagull-shaped structure. The antitragus was constructed by bending the antihelix and stabilizing it on the broadened base frame. Postoperative aesthetic outcomes and complications were documented and analyzed. <b>Results:</b> This study included a total of 658 patients with lobule-type microtia, of whom 356 (54.1%) were male and 302 (45.9%) were female. The average age was 6.5 ± 0.7 years, and the average follow-up period was 13.6 ± 3.5 months. The average aesthetic outcome score was 9.0 ± 0.7. While the forehead flap is widely utilized in nasal reconstruction, the effectiveness of a single forehead flap for subtotal/total nasal reconstruction is limited. Building on previously described fold-over flap techniques, we introduce a unique trifold forehead flap method for total nasal reconstruction utilizing a single forehead flap. (2) Positioning a simple shatterproof mirror in front of an operated facial half enables the primary surgeon to simultaneously see and stimulate at the operative site and to scrutinize the mimicry for decision-making. (3) A misconception in the literature regarding colors used for tagging \"branches for transection\" and \"branches for preservation\" is clarified, and a third tag-color is suggested for times of ambiguity. (4) The principle of distal execution of the surgery and technical considerations are underlined for maximal transection and maximal preservation. The modified selective neurectomy surgery is the current golden standard of post facial paralysis synkinesis treatment, and developing a learning curve in this type of surgery is like walking a tightrope, where the surgeon must keep pushing the limits with precise decision-making and a small margin of error. It is important to be reminded that the first aim is <i>primum non nocere</i>, and the second aim is to improve the condition of patients as much as possible. This study aims to evaluate the quality and consistency of preoperative and postoperative rhinoplasty photos on the social media platform Instagram. <b>Methods:</b> Instagram was queried on April 4, 2024 using the search term #rhinoplastybeforeandafter. Posts meeting our inclusion criteria were analyzed for inconsistencies in background, makeup, use of personal photo, and variations in face size between preoperative and postoperative images. Account demographics and time since surgery were also noted. <b>Results:</b> A total of 888 sets of before-and-after rhinoplasty photos were included, with nearly all (<i>n</i> = 882) posted by surgeon accounts. Of the posts analyzed, 23.7% had a ≥10% face size discrepancy, and 5.1% had a ≥10% face rotation discrepancy. Only 35.7% of posts were considered \"high quality,\" with no significant photo discrepancies between postop and preop images and with postoperative time interval reported. <b>Conclusion:</b> Instagram before-and-after rhinoplasty photos evaluated in this study exhibited significant heterogeneity in quality and consistency regarding image background, image source, face size, face rotation, and brightness.
