Das, Navonil Gupta","doi":"10.18231/j.ijor.2023.013","DOIUrl":"https://doi.org/10.18231/j.ijor.2023.013","url":null,"abstract":"To assess the clinical effectiveness and safety of bone marrow aspirate concentrate (BMAC) injections given intraarticularly as a potential treatment for knee osteoarthritis (OA). Data from 60 patients with knee osteoarthritis treated with BMAC injection at a single centre between December 2021 and December 2022 were retrospectively examined. We only included patients with idiopathic osteoarthritis. Post-traumatic osteoarthritis, prior knee surgery, ageless then 50 years or over 85 years, an active infection, uncontrolled diabetes mellitus, rheumatological or another systemic condition, cancer, or immunosuppressive medication use were all exclusion factors. A single-spin manual approach was used to aspirate and concentrate bone marrow from the iliac crest. The Numeric pain scale (NPS) and Oxford knee score (OKS) were used to evaluate patients both before and after the treatment. A 12-month follow-up period was used. The statistical analysis comprised a total of 60 patients. With a mean age of 67 years (range 50-85), there were 42 females and 18 males. At the end of the follow-up period, the mean NPS reduced from 8.12 to 4.31 (p< 0.001) and the mean OKS rise from 22.30 to 34.74 (p <0.001). There were no problems. We report a case of an 8 cms gap non-union patella fracture in a patient and single staged surgery with functional outcome. A 45 year female patient came with complaints of instability and defect in his right knee since 3 years. On examination we found that gap nonunion of fracture patella. Patellar fracture was approached by anterior approach. V-Y quadriceps-plasty was done to mobilize the proximal fragment. Fracture was fixed with tension band wiring fixation with k- wires. Quadriceps and retinaculum were repaired. Postoperatively physiotherapy was started and follow up of patient was done at 6 weeks, 3 months and 6 months. Improvement of knee range of movement was noticed in follow-up period. {"title":"Expert opinions on the prevalence, diagnosis and treatment choice for osteoarthritis management in Indian settings","authors":"Manjula S, Krishna Kumar M","doi":"10.18231/j.ijor.2023.014","DOIUrl":"https://doi.org/10.18231/j.ijor.2023.014","url":null,"abstract":"Studies have reported the capability of bisphosphonates to delay the progression of structural deterioration associated with osteoarthritis (OA). Limited studies exist regarding OA management in India and the determinants influencing patients' decisions regarding surgical interventions. So, this study aimed to assess the prevalence, diagnostic practices, and treatment options available for OA management in Indian clinical practice with a special focus on alendronate, a bisphosphonate medication for OA pain management. The cross-sectional, multiple-response questionnaire-based study collected a survey response using a 25-item questionnaire from 276 clinicians across India. Additionally, the survey focused on determining the diagnostic practices helpful in determining osteoporosis and the preference of treatment options for OA management. Based on the responses of 276 participants, 43% indicated that the diagnosis of OA was predominant in both genders. Moreover, about 51% of the respondents suggested that mass screening for osteoporosis in Indian patients should be conducted using the one-minute risk test developed by the International Osteoporosis Foundation. Additionally, 47% of the respondents highlighted hip fractures as the most common type of bone fracture observed in these patients. Furthermore, around 51% of the study participants emphasized that adequate calcium consumption during childhood can potentially reduce the incidence of osteoporosis. For the effective management of OA, alendronate was preferred by 49% of respondents over the other bisphosphonates.As per the expert consensus,patients who have osteoporosis risk factors and osteopenia without fragility fractures were recommended to adhere to dietary and lifestyle modifications, coupled with calcium and vitamin D supplementation, and bisphosphonate therapy. Because of ambiguous signs and symptoms, and reasonably normal laboratory investigations, these lesions pose a diagnostic challenge and there are chances of misinterpretation of radiographic images. We present a case of tubercular osteomyelitis of the first metatarsal of right foot in a 9-year-old boy. The diagnosis was initially made using plain X-ray, which was supported by magnetic resonance imaging and confirmed by the histopathological examination of the resected samples and microbiological culture combined with polymerase chain reaction. The patient was managed with open biopsy and curettage followed by a prolonged course of anti-tubercular treatment. There have been studies on the etilogical factors, however, limited attention has been given to etiology, prognostic factors, and recurrence rate. Because there are several factors that predispose to recurrence, it is imperative that medical practitioners ensure complete excision of the tumour, including removal of residual satellite nodules. This study is a focus on 0.5cm swelling over dorsal part of the interphalangeal joint of middle finger of the right hand of a 55-year old female that was initially unilobulated, which eventually increased to 3cm and was bilobed over 6 months. Many wound closure technique have been described and controversy exists as a optimal method. Healing by secondary intension used to be commonly employed technique but less popular nowadays. Most wound closure require split skin thickness grafting which increases morbidity and scarring and additional wound at donor site .gradual mechanical dermal apposition technique that is shoelace technique have gained popularity and have shown to be effective in closure of wounds .Shoelace technique for wound closure is widely used in other medico-surgical specialties, this technique is unusual in orthopedics surgery, but the simplicity of the procedure, demanded us to follow to our cases.Honey is most ancient wound dressing biomaterial .Honey is known for its anti-oxidant, anti-bacterial and anti-inflammatory properties which were used for better and faster wound healing. Both the methods combined together make it a magic formula for the wound healing.We have 21 patients presenting to VDGMC, LATUR from a period from May 2020 to December 2023, with difficult primary wound closure were included in the study. The patients underwent the surgical fracture fixation (if any), followed by application of shoelace technique and honey dressing for the patient. An infant feeding tube and skin staples were used for the technique. Patients were followed till the complete wound healing occurred. The average time required for the wound healing using this technique was 3.2 weeks.
