It undergoes metabolic activation in the skin, liver, and kidneys to exert its systemic actions primarily through Vitamin D receptor-mediated gene expression. In recent times, the involvement of Vitamin D has been documented in the regulation of neurotrophins, neural differentiation, and maturation, and the synthesis of neuromodulators including acetylcholine, dopamine, and gamma-aminobutyric acid. Clinical studies have also highlighted the involvement of Vitamin D in neurological disorders, thereby underscoring its potential in the clinical management of such disorders. Seetha","doi":"10.25259/gjmpbu_58_2023","DOIUrl":"https://doi.org/10.25259/gjmpbu_58_2023","url":null,"abstract":"Objectives: Ventilator-associated pneumonia (VAP) is a widely recognized and potentially fatal healthcare-related infection that occurs in all high-dependency units. Mechanically ventilated patients are at an elevated risk of developing VAP, which has a high death and morbidity rate. The prevalence of VAP varies greatly depending on the location and diagnostic approach. Radiological and clinical markers impact VAP diagnosis accuracy. Reliable sampling and confirmation of microbes are highly recommended. The purpose of this study was to document the incidence, patient distribution, bacteriological profile, and antibiotic susceptibility pattern of VAP patients. Material and Methods: A prospective observational study was done between January 2016 and December 2019. Critically, ill patients on mechanical ventilation for more than 48 hours were included in the study. Based on the initial baseline, positive end-expiratory pressure, and fraction of inspired oxygen were followed by three-tier VAP criteria as per NSHN guidelines. Results: Out of 1220 VAP-suspected patients (mechanically ventilated), 49 patients developed hospital-acquired VAP. The incidence of VAP significantly reduced from 10.7 to 1.4 VAP/1000 ventilator days with continuous intervention and auditing over some time. Elderly males aged 51–66 years were found to be in higher risk groups. Klebsiella pneumoniae and Pseudomonas aeruginosa were found to be the most common pathogen. The majority of Enterobacterales (79%) were found to be resistant to third-generation cephalosporin, 69% were resistant toward fluoroquinolone and cotrimoxazole, followed by 55% resistance to beta-lactam and beta-lactamase inhibitor combination. Conclusion: Targeted strategies with implementable policies, such as the care bundle approach, will reduce the in-patient days. Histopathologically, these appear as longitudinal cleft-like spaces or may appear as a collection in cholesterol granulomata. The occurrence of CCs in odontogenic cysts is closely associated with the pathophysiology of the lesions. Despite being fairly common, not much is known about the sources of cholesterol, the mechanism of formation of these clefts, their pathogenesis, and their clinical implications. The present article focuses on describing CCs with respect to their contents, pathogenesis, and clinical implications. Our review describes the mechanism of the formation of CCs in a detailed manner along with a pictorial depiction of the cascade of events involved in the process. The formation of cholesterol granulomata is a cumulative effect of the breakdown of erythrocytes and leukocytes, intramural hemorrhage, decreased blood supply and lymphatic drainage, hypoxic conditions, increased vascular permeability, accumulation and rupture of lipid-laden macrophages, and foreign body reaction. Bibliographic materials such as systematic reviews, case studies, meta-analyses, research, and review articles were also reviewed. Yagnik","doi":"10.25259/gjmpbu_154_2022","DOIUrl":"https://doi.org/10.25259/gjmpbu_154_2022","url":null,"abstract":"Carbapenem was originally regarded as a cornerstone therapeutic for Gram-negative bacterial infections that could be fatal, but these pathogens are rapidly acquiring resistance to it by various methods such as forming biofilms, mutations in porin channels, overexpression of efflux pumps, altering or modifying antibiotics, and by horizontal gene transfer of resistance biomarkers. Among Gram-negative pathogens, the World Health Organization has prioritized Acinetobacter baumannii as a “pathogen of concern.” A. baumannii strains have a variety of antibiotics resistance pathways, making them resistant to the majority of antibiotics. Antibiotics are used indiscriminately and excessively, causing extensive natural selection pressure and allowing just the most versatile variants of A. baumannii to flourish. Evolutionary changes are unavoidable, but the widespread utilization of antimicrobials in communities, hospitals, and agriculture industries is certainly contributing to the outbreak. Antibiotic resistance has resulted in a number of adverse outcomes such as inadequacy of antimicrobials, persistent infections, and physical ailments that cause the patients to lose momentum, longer stay in the hospital, increased mortality, and morbidity, as well as increased cost that can overburden the patients and even the hospitals. Therefore, optimizing efficient alternate natural antibacterial compounds is a critical aspect of addressing the problem of antibiotic resistance. Various studies ( in vitro ) done by scientists and researchers have illustrated the efficacy of plant extracts and their essential oils against A. baumannii and other pathogens. The selective permeability of the lipid bilayer structure of the cell membrane makes it mandatory for glucose to require transport proteins for its transit into the cells. These include solute carrier integral membrane proteins such as glucose transporters (GLUTs) and sodium-glucose transporters. GLUTs belong to the major facilitator superfamily with a 12 transmembrane spanner topology, with GLUT1–13 sharing the same transmembrane sequence but variable transmembrane loops and terminal cytoplasmic ends of carbon and nitrogen. Phylogenetic analysis classifies GLUTs into three classes, with each class showing an affinity for a specific substrate. The tightly coupled relationship between glucose homeostasis and the nearly ubiquitous GLUTs has led to the investigation of their diverse roles in embryonic development, adult physiology, and clinical disorders including but not limited to inborn errors, diabetes mellitus, metabolic syndrome, and cancers. The current review is pivoted around the studies focusing on the structure and functions of members of the GLUT family, their chromosomal and organ-specific distribution, as well as the current evidence of their clinical implications and prospective therapeutic roles, specifically in cancers and metabolic disorders. It is a valid and reliable measure among those without cognitive impairment. Initially, it replaced clinical assessments in survey research and clinical settings. Materials and Methods: This study was a cross-sectional survey and the study was conducted in a community-based online survey of 6-month duration. Psychologically ill patients, pregnant women, and pediatrics were excluded from this study. The sample size was set to be >800, and the collected data were 806. The data was procured from every willing individual in the age group of 18-60 years old. To identify the similarity and variances in their responses, they were correlated with the health rating of the responders. The physicians at RDT Hospital, Bathalapalli, Anantapur, Andhra Pradesh Prepared and validated the data collection form. Results: This study was evaluated to check the impact of health literacy on one’s health. All the factors were included and taken into consideration while framing the questionnaire. The parameters and the respondent’s answers were compared and correlated they were assessed by two-tailed test and significantly correlated using Pearson’s correlation. The self-rating of the health of the responders was found to be 47%. The major factors affecting health are age, gender, body mass index (BMI), occupation, weight profile, work profile, surrounding hygiene, quality of life, physical activity, eating habits, regular sleep cycle, sleep quality, stress factor, psychological and mental health, comorbidities, side effects, nutritional supplements, self-medication or doctor’s prescription, and literacy score. Respondent’s physical activities were collected and found to be 89.7% negative and statistically significant. The BMI of the responders was classified into four categories: Underweight (16.7%), normal (51.86%), overweight (20.09%), and obese (7.94%). It was found to be negatively significant and two-tailed correlated.
